Book excerpt: Injuries sustained at work represent large income and welfare losses to households and there is a significant policy interest in reducing these burdens. Workers' compensation program is a large government program which provides monetary and medical benefits to injured workers. Despite the potential importance of medical care in improving the health and labor productivity of injured workers, little research has addressed the relationship between medical care provided through workers' compensation and post-injury labor outcomes. This paper exploits the 2003-2004 California workers' compensation reforms which reduced medical care spending for injured workers with a disproportionate effect on workers suffering low back injuries. We study the differential impact of this reduction in medical care generosity on post-injury outcomes, using administrative data which includes claim-level medical costs, pre- and post-injury labor earnings, and earnings information for matched (uninjured) workers at the same preinjury firm. Our focus on labor outcomes is motivated by the importance of understanding the relationship between health and labor productivity more broadly and by the policy interest in mechanisms to improve the labor outcomes of injured workers. Adjusting for injury severity and selection into workers' compensation, we find that workers with lower back injuries experienced a 7.3% greater decline in medical care after the reforms, and that this led to an 8.3% reduction in post-injury earnings relative to other injured workers. We estimate that this earnings decline is due both to an increase in injury duration and to lower earnings conditional on working.

In reality, how- ever, disputes often arise over issues such as whether an injury in fact occurred at work, whether medical treatment is necessary, and the extent to which an injury poses long-term consequences for the worker. All such disputes are resolved in a single forum: the Workers' Compensation Appeals Board (WCAB). Of the one million workers' compensation claims filed in California every year, about 200,000 end up at the WCAB.

Losses from injuries to employees result in disruption of the educational process, lost days from work increased insurance premiums and consequent loss of funding for core educational initiatives. Injuries can diminish in frequency and severity or be avoided almost entirely if administrators implement health and safety systems. The cost of providing medical care to injured employees establishes baseline data on which worker compensation premiums are assessed. Administrators could reinvest costs saved in workers compensation insurance premiums into educational programs for the direct benefit of students. A comprehensive safety system that confirms administration commitment to a safe working environment assures management commitment as a legal and ethical responsibility, promotes employee involvement, pinpoints and eliminates potential hazards, and encourages employees to prevent potential harm to other employees, teaching staff, and students. This text focuses on developing a successful workers compensation program and safety system for school district employees. The models and strategies may be adapted for use in school districts for reducing school injuries. School district administrators are frequently forced to do more with less funding, particularly considering the requirement to implement many unfunded mandates from state and federal governments. Dollars saved from improved worker safety and reduction of insurance premiums may directly benefit educational initiatives in school districts. Primarily, the funds for educating students may originate from saved workers compensation dollars.

Book excerpt: Over the past few years, nonoccupational health-insurance programs and group health plans have implemented initiatives to improve the quality and efficiency of care through incentive programs, typically called "pay for performance," or P4P. In addition, Medicare program administrators are evaluating how P4P incentives might be incorporated into Medicare payment systems. A workers' compensation (WC) P4P program could be an effective strategy to improve the quality and efficiency of care, increase patient satisfaction, and improve work-related outcomes provided it is integrated with other initiatives to improve the WC medical treatment system. The program could reward only high-performing physicians or reward improvement from any physician, and payer and provider participation could be voluntary or mandatory. Challenges to implementation include lack of clinical measures for WC conditions, multiple payers, and multiple physicians treating few WC patients and the high level of contention among stakeholders. This paper assesses the options, challenges, and potential benefits of adopting P4P incentives for physician services in California2s WC program, offering three models of programs for consideration.
