Poor pain management is a result of health care professionals lack of knowledge and access to the training programs which brings necessary skills, knowledge and attitude towards pain management. The study analyze the current pain management situation in the hospital and how the training and knowledge can bring effective pain management to be implemented in the clinical health facility. The pain management training was provided to nursing professionals and the level of knowledge was analyzed before training and immediately after training. The evaluation of knowledge shows improved nursing practices and attitudes after educational program. Pain education helps nurse to better understand and assess the pain for the patient who underwent surgery. Study also reiterate the need of continued education programs for nursing staff in order to provide evidenced based practice and policies while providing pain management to the patient community.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

These numbers have increased considerably for a variety of reasons, and the growing role of prescription drug abuse has been widely recognized. There has also been increasing consciousness of the lack of dedicated and formalized training dedicated to prescription of analgesics, particularly among surgical trainees and practicing surgeons. Recent studies demonstrate that only a small minority of training programs mandates any opioid prescribing education, despite the fact that nearly all programs allow trainee prescription of opioids on an outpatient basis. Therefore, it is not surprising that despite a growing body of literature illustrating the value of opioid alternatives and adjuncts, practicing surgeons in all specialties largely ignore these evidence-based practices and continue to prescribe opioids in situations for which alternatives are proven, safe, and effective. There are many reasons for preparing a concise and practical resource describing evidence-based practices targeting surgeons and surgical subspecialists, including but not limited to: (1) The exponential increase in prescription drug abuse associated with the “opioid epidemic,” as surgeons play an important role in managing perioperative and chronic pain in a variety of practice setting; this issue bears importance from the patient, physician, and policymaker standpoint; (2) With increasing recognition of the role of quality improvement in patient safety initiatives, and literature demonstrating inadequate opioid prescribing education (OPE) in surgical training programs, it is only a matter of time before accreditation bodies including the ACGME incorporate strong recommendations and mandates for OPE; (3) Myriad literature demonstrating the existence of evidence-based alternatives to opioids – All surgeons and surgical subspecialists would benefit from a practical guide concisely laying this literature out. We feel that surgeons of all experience levels may benefit, ranging from the first year surgical intern to an experienced practitioner trying to remain compliant with the evolving opioid regulatory environment. Hence, the appropriate target audience for this text would be for the tens of thousands of current surgical trainees in many specialties, including general surgery, thoracic surgery, otolaryngology, plastic surgery, urology, gynecology, vascular surgery, neurosurgery, and orthopedic surgery. Furthermore, surgeons out of training would be another audience of interest, as there are increasing courses for continuing medical education based on perioperative and chronic pain management. Currently, there are no comparable and competitive guides with these target audiences; all of the existing literature about perioperative and chronic pain is targeted either specifically for patients, anesthesiologists, or pain medicine physicians. This will be the only up to date guide focusing on evidence-based practices for perioperative pain control, and each section will also include information on chronic pain sequelae relevant to each surgical specialty. The editors envision this text being an interdisciplinary endeavor, incorporating surgeons from multiple specialties, anesthesiologists, pain medicine physicians, and palliative physicians as appropriate.

Due to the identified need of postoperative pain management after a total hip arthroplasty (THA) and a total knee arthroplasty (TKA), an evidence -based change project was conducted utilizing educational session for orthopedic surgeons and day surgery and PACU nurses to improve knowledge regarding the utilization of multimodal analgesia for postoperative pain management. A Pain Management survey tool that was created by the PI was utilized to assess the current practices of the orthopedic surgeons and the knowledge of the day surgery and PACU nurses prior to educational sessions. Analysis of the data through descriptive statistics to compare the means of pre and post intervention that was performed. A 32.3% decrease was seen in the pain scores immediately postoperatively and upon discharge. A 55.7% increase was seen the patient satisfaction pain scores. ..." -- from Abstract.

Acute postoperative pain management is one of the most important roles and functions of the perioperative nurse. Healthcare providers that integrate evidence-based guidelines into practice make informed decisions that are more concise and predictable. Although clinical guidelines on acute postoperative pain management have been published by the American Society of PeriAnesthesia Nurses, the Association of Perioperative Registered Nurses, the American Pain Society and the American Society of Anesthesiologist Task Force on Acute Pain Management, patients continue to experience inadequate pain control due to inconsistent pain management practices. Educational interventions can increase clinical knowledge and decrease negative patient outcomes post-surgically. The purpose of this EBP change project was to reduce acute postoperative patient pain through the implementation of an educational intervention aimed to educate the staff on evidence-based standardized pain management guidelines in an Ambulatory Surgery Center and Post Anesthesia Care Unit. The benchmark of 80% post intervention and 70% post implementation was achieved. There was an overall 22% reduction rate in pain scores from 34.91% in August 2017 to 27.36% in October 2017. Although the rate in October 2017 did not meet the 22% internal benchmark, there was a decrease in the rate of pain.
