When coupled with appropriate treatment, early detection can reduce breast cancer mortality. At the request of Congress, the Food and Drug Administration (FDA) commissioned a study to examine the current practice of mammography and breast cancer detection, with a focus on the FDA's oversight via the Mammography Quality Standards Act (MQSA), to identify areas in need of improvement. Enacted in 1993, MQSA provides a general framework for ensuring national quality standards in facilities performing screening mammography, requires that each mammography facility be accredited and certified, and mandates that facilities will undergo annual inspections. This book recommends strategies for achieving continued progress in assuring mammography quality, including changes to MQSA regulation, as well as approaches that do not fall within the purview of MQSA. Specifically, this book provides recommendations aimed at improving mammography interpretation; revising MQSA regulations, inspections, and enforcement; ensuring an adequate workforce for breast cancer screening and diagnosis; and improving breast imaging quality beyond mammography.

Book excerpt: In November 1999, the Institute of Medicine, in consultation with the Commission on Life Sciences, the Commission on Physical Sciences, Mathematics, and Applications, and the Board on Science, Technology and Economic Policy launched a one year study on technologies for early detection of breast cancer. The committee was asked to examine technologies under development for early breast cancer detection, and to scrutinize the process of medical technology development, adoption, and dissemination. The committee is gathering information on these topics for its report in a number of ways, including two public workshops that bring in outside expertise. The first workshop on "Developing Technologies for Early Breast Cancer Detection" was held in Washington DC in February 2000. The content of the presentations at the workshop is summarized here. A second workshop, which will focus on the process of technology development and adoption, will be held in Washington, DC on June 19-20. A formal report on these topics, including conclusions and recommendations, will be prepared by the committee upon completion of the one-year study.

Due to the combination of improved treatments and the benefits of mammography screening, breast cancer mortality has decreased steadily since 1989. Yet breast cancer remains a major problem, second only to lung cancer as a leading cause of death from cancer for women. To date, no means to prevent breast cancer has been discovered and experience has shown that treatments are most effective when a cancer is detected early, before it has spread to other tissues. These two facts suggest that the most effective way to continue reducing the death toll from breast cancer is improved early detection and diagnosis. Building on the 2001 report Mammography and Beyond, this new book not only examines ways to improve implementation and use of new and current breast cancer detection technologies but also evaluates the need to develop tools that identify women who would benefit most from early detection screening. Saving Women's Lives: Strategies for Improving Breast Cancer Detection and Diagnosis encourages more research that integrates the development, validation, and analysis of the types of technologies in clinical practice that promote improved risk identification techniques. In this way, methods and technologies that improve detection and diagnosis can be more effectively developed and implemented.

Book excerpt: Millions of women undergo screening mammography regularly with the hope of detecting breast cancer at an earlier and more curable stage. But the ability of such screening to accurately detect early cancers depends on the quality of mammography, including high-quality image acquisition and interpretation. To help ensure the quality of mammography, Congress passed the Mammography Quality Standards Act (MQSA) in 1994 and last reauthorized it in 2004. In advance of its expected reauthorization in 2007, Congress requested a consensus study from the Institute of Medicine (IOM) recommending ways to improve the quality of mammography, with an emphasis on image interpretation. The resulting report, Improving Breast Imaging Quality Standards, highlighted the need to decrease variability in mammography interpretation in the United States and identified gaps in the evidence needed to develop best practices. The consensus committee found that mammography interpretation remained quite variable, and that this variability limited the full potential of mammography to reduce breast cancer mortality by detecting breast cancers at an early stage. In May 2015, the IOM convened a workshop to address this issue. The participants discussed challenges in the delivery of high-quality mammography, the impact of training and experience on interpretive performance, how best to measure interpretive performance, and the potential impact of new technologies and supplemental imaging on interpretation of breast screening and diagnostic images. Assessing and Improving the Interpretation of Breast Images summarizes the presentations and discussions from this workshop.

A User's Guide has been designed to assist you in utilizing the Resource Manual. It is Prudential HealthCare's priority to increase mammography screening rates among eligible members of the plan. The goal of the User's Guide and the Resource Manual is to assist you in increasing the mammography screening rates among the eligible members of your health plan. The Resource Manual is a source of basic information on mammography and the intervention strategies and tools that health plans may use to help increase the screening rates. The User's Guide outlines major points from the Resource Manual, and will help you find the information in the larger manual that is most relevant for your needs. The manuals are designed to be user-friendly, practical and adaptable to the needs and capacity of different health plans. Together with the resources suggested in the manuals, a health plan will have access to the basic information needed to successfully plan, implement and evaluate the effectiveness of mammography intervention strategies. Book excerpt: The objective of this program was to increase mammography screening rates among Hispanic women through a series of targeted community-wide interventions. A diverse array of outreach efforts was offered by the program to increase awareness and use of screening mammography. Before the program, 12 percent of the Hispanic women surveyed in the intervention community had been screened, compared with 27 percent after the program. There was no change in screening among Hispanic women in the control community (23 percent before and 24 percent after the program). The program demonstrated that the awareness and behavior of "hard-to-reach" underscreened Hispanic women can be changed through intensive targeted outreach and that a church-based cancer control program can play an effective role in the process. This finding has national health policy implications.

Book excerpt: This paper explores the efforts of the San Francisco Department of Public Health (SFDPH) and San Francisco General Hospital (SFGH) to increase their overall mammogram screening rate to 76.7% by June 2015 in order to receive funding from the federal government. The target population is women within SF DPH's primary care program, 50-74 years old, and overdue for a mammogram. The literature reports there are health disparities among women who receive mammograms and how breast cancer affects them. African American women have the highest mortality rates and women of color are more likely to be diagnosed at a later stage, when treatment options are limited (National Cancer Institute, 2014). There were disparities found within the mammogram rates at SFDPH which varied by ethnicity, insurance type, and socioeconomic status. The literature also supports this finding with The National Cancer Institute (2014) stating that lower screening rates are associated with lower education levels and incomes, the uninsured, and women of color. A pilot study was conducted at the SFGH Avon Breast Center, which included extending evening hours and opening a Saturday clinic for mammograms, from February to June 2014, to increase appointment accessibility for patients. The overall mammogram rate was calculated using data from just over 13,000 eligible women in the SFDPH system. After the pilot study ended, the mammogram rate for every ethnicity group had increased and the overall rate had increased by 5%. Using a chi-square test, this increase was shown to be statistically significant. The appoinment show rate also increased by 16% during the pilot study, as compared to the regular hour appointments, and this increase was shown to be statistically significant as well. Patient and staff surveys were distributed to obtain quantitative and qualitative feedback directly from involved stakeholders. It was noted that women enjoyed the pilot study hours and were especially interested in having other services offered on Saturdays as well. By offering evening and Saturday hours, the mammogram and show rates experienced a significant increase, and the patients awarded high satisfaction scores for their experience during the pilot study. SFDPH and SFGH are collaborating together to address this issue in addition to discovering other methods to educate women on the importance of getting screened for breast cancer and to make mammograms more accessible to women from diverse backgrounds.
