They report decrease in satisfaction, quality of care and often poor outcomes. There is a strong sentiment among American institutions that immigrants should learn English and that langauge barriers should not be the problem of hospitals and staff. This project evaluated policies and procedures already in place at the clinic, benchmarked these against standards of federal and state regulations of practice, with the goal to improve operations for staff and improve access and standard of care for patients with LEP. The purpose of this capstone project was to answer the question: For the Hispanic patients of limited English proficiency at this clinic will improvements in communication lead to improvements in access to care? The primary intervention was to increase the availability of medical interpretation either face to face or through mechanical devises. Everyday operations when serving patients with LEP were benchmarked against regulation standards. The staff was surveyed for suggestions. Signage, language services and medical information handouts were made available to patients with LEP. After Institutional Review Board Approval, Hispanic patients were asked to return a satisfaction survey. Results identified medication issues and after office appointment communication as areas that still needed resolution. Easier access to appointments and better understanding of provider instructions at the time of an appointment were viewed as positive accomplished outcome goals. This project was the catalyst for the short term goal of improving communication between staff and patients with LEP at a New Jersey clinic. Continued monitoring, revisions and updating must be an organizational goal in an effort to benchmark evolving practice change. Long term goals of this project will be to improve and continually monitor cost-effective ways to communicate with patients who identify themselves as having Limited English proficiency. Failure to do so will negatively affect the quality of care accessible to patients with LEP. This in turn will put increased demands on a broken healthcare system that is unable to financially support itself. Eventually, this organization will be the basis for community support to not only Hispanic patients with LEP, but all peoples of different cultures.

Book excerpt: People with limited English proficiency (LEP) tend to be underserved and vulnerable in the United States health care system. Research has shown that LEP patients are more likely to have lower levels of education, be uninsured, have lower levels of income, and are more likely to experience discrimination during their health care encounters. This study seeks to find whether LEP patients face less favorable health care experiences than their non-LEP counterparts. Specifically, the study aims to examine the differences in health care utilization, access to health information, and satisfaction of health care services between LEP and non-LEP individuals. Based on previous use of LEP in health care literature, the limited English proficiency group in this study represents those whose primary household language is not English. The dataset used for this study was the 2007 National Survey of Children's Health. Linear and logistic regression results found that LEP patients are less likely to feel that their children's doctors and providers listen to them carefully and they are also less likely to report that they feel their providers respect their customs and values compared to non-LEP patients. Additionally, LEP patients are less likely to receive the specific health information they need and they utilize health care services less often than their non-LEP counterparts. These findings lead to the conclusion that the need for culturally diverse and culturally competent care remains in order to improve the health care utilization and encounters for LEP populations.

Book excerpt: The Centers for Medicare & Medicaid Services (CMS) have been moving from volume-based, fee-for-service payment to value-based payment (VBP), which aims to improve health care quality, health outcomes, and patient care experiences, while also controlling costs. Since the passage of the Patient Protection and Affordable Care Act of 2010, CMS has implemented a variety of VBP strategies, including incentive programs and risk-based alternative payment models. Early evidence from these programs raised concerns about potential unintended consequences for health equity. Specifically, emerging evidence suggests that providers disproportionately serving patients with social risk factors for poor health outcomes (e.g., individuals with low socioeconomic position, racial and ethnic minorities, gender and sexual minorities, socially isolated persons, and individuals residing in disadvantaged neighborhoods) may be more likely to fare poorly on quality rankings and to receive financial penalties, and less likely to receive financial rewards. The drivers of these disparities are poorly understood, and differences in interpretation have led to divergent concerns about the potential effect of VBP on health equity. Some suggest that underlying differences in patient characteristics that are out of the control of providers lead to differences in health outcomes. At the same time, others are concerned that differences in outcomes between providers serving socially at-risk populations and providers serving the general population reflect disparities in the provision of health care. Systems Practices for the Care of Socially At-Risk Populations seeks to better distinguish the drivers of variations in performance among providers disproportionately serving socially at-risk populations and identifies methods to account for social risk factors in Medicare payment programs. This report identifies best practices of high-performing hospitals, health plans, and other providers that serve disproportionately higher shares of socioeconomically disadvantaged populations and compares those best practices of low-performing providers serving similar patient populations. It is the second in a series of five brief reports that aim to inform the Office of the Assistant Secretary of Planning and Evaluation (ASPE) analyses that account for social risk factors in Medicare payment programs mandated through the Improving Medicare Post-Acute Care Transformation (IMPACT) Act.

In the United States today, over 8% of the population is of LEP, and yet no formal funding structure exists to ensure that these patients have access to interpreter services when they see their primary care provider. Research shows that without appropriate interpreter services, LEP patients not only endorse poorer subjective experiences of healthcare, but that these patients also experience objectively worse healthcare, such as less frequent visits to primary care providers, poorer management of chronic illness such as diabetes and hypertension, and higher rates of expensive testing in emergency departments, to list a few examples. This paper was inspired by firsthand experiences of a medical student learner at an urban health center witnessing frequent improper utilization of interpreter services by physicians in primary care settings. The paper traces the federal legal history addressing communication services in healthcare in the United States, reviews papers that juxtapose patient and provider opinions of interpreter services, and concludes with a discussion of potential steps forward for improving interpreter services offered to the large percentage of the US population that is currently receiving subpar care due to unsurmounted barriers to communication.

When language barriers exist, the quality of healthcare provided for patients exponentially decreases. Not using professional interpreter services in acute care settings for patients with limited English-speaking skills can lead to disparities, including decreased quality of care, errors in interpretation, and omission of important information. The key to providing efficient and accurate healthcare to the LEP population is effective communication between care providers and patients. The use of interpreting services is integral in achieving accurate communication within the healthcare system. An applied project was completed to develop an online learning module about interpreting services utilization. The online learning module was designed using VideoScribe, an animation software. There were five segments presented in this online educational module. These included: (1) A brief informational presentation about UC Davis Health Center and its community; (2) A narrative describing the hospitalization experience for non-English-speaking patients; (3) Details about the importance of professional interpreting services in caring for patients with limited English proficiency; (4) Accessing and properly using interpreting services and technology; and (5) Documenting the encounter in the patient's medical record. The learning module was sent to twelve expert reviewers. Modifications were made based on the recommendations of the expert reviewers after they completed a standardized evaluation form. This educational module provides healthcare professionals with the knowledge necessary to effectively communicate with non-English-speaking patients. The goal is to house the learning module on the UC Davis Learning Center and provide a quick access link on the UC Davis Health Interpreting Services website. It is important that healthcare providers address possible communication disparities for people with limited English skills in order to improve patient outcomes and maintain high levels of patient satisfaction. Training, such as the online education module, can be the first step in the process.
