Book excerpt: Over the past three decades, many countries of Latin America and the Caribbean have recognized health as a human right. Since the early 2000s, 46 million more people in the countries studied are covered by health programs with explicit guarantees of affordable care. Reforms have been accompanied by a rise in public spending for health, financed largely from general revenues that prioritized or explicitly target the population without capacity to pay. Political commitment has generally translated into larger budgets as well as passage of legislation that ring-fenced funding for health. Most countries have prioritized cost-effective primary care and adopted purchasing methods that incentivize efficiency and accountability for results, and that give stewards of the health sector greater leverage to steer providers to deliver on public health priorities. Evidence from the analysis of 54 household surveys corroborates that investments in extending coverage are yielding results. Though the poor still have worse health outcomes than the rich, disparities have narrowed considerably - particularly in the early stage of the life course. Countries have reached high levels of coverage and equity in utilization of maternal and child health services; coverage of noncommunicable disease interventions is not as high and service utilization is still skewed toward the better off. Catastrophic health expenditures have declined in most countries; the picture regarding equity, however, is mixed. While the rate of impoverishment owing to health-care expenditures is low and generally declining, 2-4 million people in the countries studied still fall below the poverty line after health spending. Efforts to systematically monitor quality of care in the region are still in their infancy. Nonetheless, a review of the literature reveals important shortcomings in quality of care, as well as substantial differences across subsystems. Improving quality of care and ensuring sustainability of investments in health remain an unfinished agenda.

Murillo's analysis of the Latin American electricity and telecommunications sectors shows that different degrees of electoral competition and the partisan composition of the government were crucial in resolving policymakers' tension between the interests of voters and the economic incentives generated by international financial markets and private corporations in the context of capital scarcity. Electoral competition by credible challengers dissuaded politicians from adopting policies deemed necessary to attract capital inflows. When electoral competition was low, financial pressures prevailed, but the partisan orientation of reformers shaped the regulatory design of market-friendly reforms. In the post-reform period, moreover, electoral competition and policymakers' partisanship shaped regulatory redistribution between residential consumers, large users, and privatized providers.

It concludes that participatory budgeting in selected European countries is far away from the level of ‘best practice’, but that all experiences are not just trivial pursuits. The information collected serves to check, to what extent participatory budgeting as practiced in the countries involved presents a real attempt to change municipal budgets towards addressing the needs of marginalized groups and to improve decision-making based on local democracy and participation, or whether these processes as such are to be judged to be more important than any output and outcomes. The practices can neither be seen as a process of policy diffusion nor as a process of policy mimesis. The terminology of participatory budgeting remains, but the tools to achieve the goals resulted only in marginal changes in the status quo in municipalities in European countries practicing participatory budgeting, instead of resulting in radical changes to increase spending in favor of marginalized groups. Book excerpt: Due to the lack of systematic policy evaluations, identifying successful policies in the Latin American and the Caribbean (LAC) region is a hunting task. Nevertheless, this type of analysis is crucial to inform policymakers in their decision-making process. Herein we contribute to filling this gap by assessing the progress in terms of energy services since the year 2000 and reviewing the policies that have led us to where we are.We focus on three fundamental dimensions in the definition of energy services: access to electricity and clean fuels for cooking, affordability of those energy services and quality considering both service’s interruptions and energy losses. We find that countries that have improved in all these dimensions simultaneously, catching up with the best performers in the region, are the ones that have implemented integrated policies that are part of a pluriannual plan, implemented in a strong institutional environment.Aside from macroeconomic differences, successful energy policies have in common the following characteristics: (i) important institutional reforms already in place by the end of the century; (ii) state-led plans that measure performance accounting for most dimensions simultaneously and including mechanisms to enforce preestablished objectives in targeted population; (iii) appropriate financing mechanisms to ensure affordability; and (iv) partnership with private stakeholders when their participation increases efficiency, adoption of innovation and ensure maintenance, either directly or through the involvement of local communities.Even if the discussion is presented dimension by dimension, two countries appear like having implemented successful integrated policies: Peru and Ecuador. Despite their institutional differences, they have succeeded in increasing access to affordable electricity that is more reliable (with less frequency and duration of interruptions). Additionally, Ecuador has also enhanced efficiency through electricity loss reduction thanks to enforceable performance-based regulation.Aside from these two countries, other countries have also implemented successful policies that have greatly improved one or two of the dimensions mentioned but fail to tackle all the issues simultaneously. This is the case of El Salvador with access to clean cooking and fuel subsidy reform, for example. Similarly, Nicaragua appears as an example in bridging the gap in access and Brazil, Colombia and more recently Argentina are implementing a mechanism to target demand-side subsidies based on family economic means.
