Afin d'évaluer les risques et les résultats de cette chirurgie, nous réalisons une étude rétrospective, portant sur 16 enfants sourds profonds et porteurs d'une malformation bilatérale d'oreille interne, parmis 260 patients implantés entre 1991 et 2003. L'intervention n'est pas dénuée de risques chirurgicaux et infectieux inhérents à la malformation. Nous avons constaté 7 cas de geysers per-opératoires dont un cas avec liquorrhé persistante et un cas de malposition du porte électrode. Aucune autre complication n'a été observée. Certaines précautions sont essentielles pour optimiser la prise en charge de ces enfants et limiter les risques : un examen tomodensitométrique des rochers en coupes millimétriques, la vaccination systématique et une préparation péri-opératoire, permettant de diminuer la pression intralabyrinthique. Les résultats sur la perception et le développement du langage sont bons. Une grande majorité des enfants (92 %) ont une amélioration de leur perception, à un an de recul. La progression dépend de l'histoire de la surdité et de l'âge d'implantation. Parmi les enfants atteints de surdité évolutive, 87,5 % ont un score perceptif en liste ouverte supérieur ou égal à 50 % alors que dans le groupe des enfants sourds congénitaux, les résultats sont de 20 % ceci un an après l'implantation cochléaire. Malgré les risques possibles qu'il ne faut pas sous-éstimer, les résultats orthophoniques encourageant permettent de justifier l'indication opératoire qui doit néanmoins être pesée au cas par cas.

Book excerpt: La malformation cochléo-vestibulaire est un facteur de variabilité des résultats de l'implant cochléaire et majore les risques chirurgicaux. Certains auteurs ont proposé la mise en place d'un implant du tronc cérébral dans les malformations majeures avec les arguments du risque d'échec de l'implant cochléaire et de méningite. Cette étude a examiné de façon rétrospective les anomalies radiologiques, les caractéristiques chirurgicales, les résultats auditifs et sur le langage des enfants présentant une malformation cochléo-vestibulaire porteurs d'un implant cochléaire sur trois centres d'implantation de janvier 1998 à mai 2015. 102 enfants implantés cochléaires (108 oreilles) ont été inclus, 49 enfants avec malformations cochléo-vestibulaires (54 oreilles) formant le groupe malformation, et 53 enfants (54 oreilles) avec anatomie cochléo-vestibulaire normale formant le groupe contrôle. Le suivi moyen était de 78.3 mois. Le sous-groupe malformation cochléaire majeure était constitué de 5 hypoplasies cochléaires de type I, de 3 cavités communes et d'une partition incomplète de type I. Le sous-groupe malformation cochléaire mineure était constitué de 26 partitions incomplètes de type II. Les enfants porteurs de malformations cochléaires mineures et vestibulaires isolées implantés cochléaires présentaient un développement du langage équivalent à la population contrôle. Les enfants porteurs d'une malformation cochléaire sévère présentaient des résultats moins bons et une progression plus lente par rapport au groupe contrôle. De plus ils présentaient une association fréquente à d'autres facteurs de mauvaises performances de l'implant cochléaire. La chirurgie a été compliquée par la présence d'une oreille geyser dans 22.2% des cas. Il n'y a eu aucun cas de paralysie faciale ni de méningite. L'implantation cochléaire semble bénéfique pour tous les enfants présentant une malformation cochléo-vestibulaire, avec une variabilité des résultats dans les malformations cochléaires majeures. Cette étude incite à proposer un implant cochléaire dans les malformations majeures et de n'envisager l'implant du tronc cérébral que pour les échecs de l'implant cochléaire.

Book excerpt: Introduction : le risque de perte complète de la fonction vestibulaire (FV) à la suite d'une implantation cochléaire (IC) chez l'enfant sourd profond, toute étiologie confondue, a été évalué à 10% Ce risque n'est pas connu pour les enfants porteurs de malformation d'oreille interne (MOI), dont la FV est souvent soumise spontanément à des fluctuations. Cette fonction est d'ailleurs peu étudiée dans la littérature. Le but de cette étude est donc d'évaluer l'état de la FV dans une large cohorte d'enfants atteints de MOI et de préciser le risque d'atteinte vestibulaire due à l'IC à court terme en tenant compte du potentiel évolutif spontané de la MOI. Matériels et méthodes : il s'agit d'une étude rétrospective reprenant les examens vestibulaires réalisés à l'hôpital Robert Debré entre 1993 et 2020 chez les enfants porteurs de MOI (malformation cochléaire et/ou malformation du labyrinthe postérieur et/ou dilatation de l'aqueduc du vestibule). Parmi les 222 porteurs de MOI, 199 enfants ont bénéficié d'un bilan vestibulaire (BV) initial complet et 47 d'entre eux un BV complet avant et après une IC unilatérale. Nous avons analysé la FV avant toute implantation sur les 199 patients et le risque d'atteinte vestibulaire a été évalué sur le groupe de 47 patients implantés d'un seul côté, en comparant l'évolution de la FV des oreilles implantées par rapport aux oreilles appariées non implantées (à l'aide d'un paired ttest). A partir des résultats au test calorique, aux Potentiels Evoqués Otolithiques Myogènes (PEOM) et au Video Head Impulse Test (VHIT), la FV a été classée en trois groupes : FV normale (réponse normale à tous les tests), FV partielle (atteinte partielle de la fonction canalaire et/ou otolithique), aréflexie de la FV (absence de réponse à tous les tests). Résultats : la FV des patients porteurs de MOI non implantés (n=199) était normale pour 134 enfants (67,3 %), partiellement atteinte pour 61 (30,7%) et 4 enfants (2%) avaient une aréflexie vestibulaire complète bilatérale. Parmi les enfants avec une fonction partielle (n=61), 48 (78,6%) avaient un vestibule moins fonctionnel, dont 7 (11,4%) avaient une aréflexie unilatérale. Des signes d'évolutivité récente (nystagmus et/ou prépondérance directionnelle >20%), ont été retrouvés pour 46 patients (23,1%), dont 23 (50%) avaient une fonction normale. Dans le groupe d'enfants implantés en unilatéral (n=47), au bilan post-IC, 66% des bilans étaient modifiés pour les oreilles implantées et 49% pour les oreilles non implantées, avec pour les oreilles implantées une réflectivité au calorique et un ratio aux PEOM significativement plus bas (paired-test, p
