Book excerpt: Dans un contexte de forte évolution des recommandations de bon usage et d'expansion des utilisations du complexe prothrombinique (CCP), nous avons réalisé une évaluation des pratiques de prescription dans l'ensemble des services du CHU de Besançon en 2006-2007. Ainsi 9 paramètres ont été évalués sur les deux périodes d'étude avec pour chaque critère la détermination d'un taux de non-conformité absolu (par critère) et cumulé (tenant compte du critère précédent) : données complètes, indication déclarée incluse dans le RBU, données du dossier conformes à la définition clinique/biologique de l'indication selon les recommandations nationales, alternative de première intention, posologie conforme, traitements associés conformes, timing du bilan de contrôle conforme, efficacité du traitement, sécurité du traitement. Au total, 98 patients ont été inclus (37 patients avant et 61 patients après diffusion des recommandations). Le taux de conformité cumulé final est faible et n'est pas amélioré entre les deux périodes d'étude (8,1% avant versus 1,6% après, p=0,149). Le principal critère de non-conformité est le non-respect de la posologie : le taux de conformité absolu de ce critère diminue de 45,9% (avant) à 16,4% (selon le RCP) après diffusion des recommandations (p=0,002). Notre étude souligne la nécessité d'une homogénéité des recommandations de bon usage dès la commercialisation des médicaments, basées sur des études à haut niveau de preuve. Le travail pluridisciplinaire sur le bon usage du CCP devra être poursuivi, en prenant en compte ses nouvelles utilisations avec les anticoagulants oraux directs
