Book excerpt: Les effets indésirables liés aux AVK (antivitamines K) sont un problème de santé publique en France. Leur usage est en constante augmentation depuis les années 90 et ils étaient responsables de 37 % des événements indésirables graves rapportés liés au médicament en 2003. Malgré les efforts déployés par les autorités sanitaires, les AVK représentaient toujours la plus forte incidence d'hospitalisation pour effets indésirables en 2008. Nous avons réalisé une étude rétrospective, observationnelle des patients sous AVK hospitalisées à l'hôpital de Pontoise entre le 1er décembre 2006 et le 31 octobre 2008. Nous leur avons proposé un protocole d'éducation thérapeutique. Nous avons aussi interrogé leurs médecins traitants sur leur vision de cette éducation. 88 patients ont été inclus. 60 patients ont accepté le protocole (durée médiane de suivi 9 mois), 28 patients l'ont refusé. 28 médecins ont été inclus. Les patients éduqués ont fait preuve d'une meilleure connaissance de l'INR (International Normalized Ratio) cible, et d'un taux plus faible de saignements mineurs (p=0,03). 86% des médecins recommandaient fortement le protocole. Nos résultats sont encourageants et confirmés par des études réalisées à plus grande échelle. La commercialisation de nouvelles molécules pourrait remettre en question l'intérêt d'une telle éducation.

Aujourd'hui on sait que l'intégration d'actions éducatives centrées sur le patient dans la stratégie de prévention contribue à l'amélioration de l'éfficacité et de la sécurité du traitement. Dans ce contexte , l'éducation thérapeutique du patient (ETP) traité par AVK prend toute son importance. Le pharmacien est plus que jamais l'un des acteurs de santé au cœur de l'ETP. L'objectif de ce travail de thèse était de mettre en place une démarche éducative structurée dans le service de cardiologie du Centre Hospitalier de Saint-Quentin (02), en s'inspirant des recommandations actuelles de l'ANSM. Pour cela j'ai réalisé une étude ayant pour objectif la conception d'un questionnaire de sortie hospitalière répondant aux exigeances d'un programme d'ETP. Un protocole de prise en charge des patients sous traitement AVK admisdans le service a également vu le jour. Si les fondations d'une démarche éducative de qualité ont été posées, il reste maintenant à poursuivre le travail pour tendre vers la création d'un véritable programme d'ETP reconnu et autorisé par l'Agence Régionale de Santé.
