Book excerpt: Malaria remains an important cause of illness and death in children and adults in countries in which it is endemic. Malaria control requires an integrated approach including prevention (primarily vector control) and prompt treatment with effective antimalarial agents. Malaria case management consisting of prompt diagnosis and effective treatment remains a vital component of malaria control and elimination strategies. Since the publication of the first edition of the Guidelines for the treatment of malaria in 2006 and the second edition in 2010 all countries in which P. falciparum malaria is endemic have progressively updated their treatment policy from use of ineffective monotherapy to the currently recommended artemisinin-based combination therapies (ACT). This has contributed substantially to current reductions in global morbidity and mortality from malaria. Unfortunately resistance to artemisinins has arisen recently in P. falciparum in South-East Asia which threatens these gains. This third edition of the WHO Guidelines for the treatment of malaria contains updated recommendations based on a firmer evidence base for most antimalarial drugs and in addition include recommendation on the use of drugs to prevent malaria in groups at high risk. The Guidelines provide a framework for designing specific detailed national treatment protocols taking into account local patterns of resistance to antimalarial drugs and health service capacity. It provides recommendations on treatment of uncomplicated and severe malaria in all age groups all endemic areas in special populations and several complex situations. In addition on the use of antimalarial drugs as preventive therapy in healthy people living in malaria-endemic areas who are high risk in order to reduce morbidity and mortality from malaria. The Guidelines are designed primarily for policy-makers in ministries of health who formulate country-specific treatment guidelines. Other groups that may find them useful include health professionals and public health and policy specialists that are partners in health or malaria control and the pharmaceutical industry. The treatment recommendations in the main document are brief; for those who wish to study the evidence base in more detail a series of annexes is provided with references to the appropriate sections of the main document.

Book excerpt: Worldwide, the numbers of people suffering and dying from parasitic diseases are overwhelming, with more than 100 million cases and 1 million deaths each year from malaria alone. Despite the magnitude of the problem and the importance of the parasites that cause opportunistic infections among persons with HIV/AIDS, medical schools in the United States, Canada, and other developed countries consistently reduce the amount of time spent on parasitic diseases in the curricu lum. As a result most medical students receive limited information about these diseases, and are inadequately prepared to diagnose or treat them as physicians. This problem is too large to be resolved within the time available for parasitology in the medical school curriculum; at most, students can be acquainted with the salient features of the medically important parasites. Likewise, the traditional isolation of parasitology from the rest of the curriculum (consistent with its exclu sion from most microbiology texts) is another unresolved problem. In my opinion, this is why most physicians are unable to think about the differential diagnosis of parasitic diseases in the same way that they routinely balance the probabilities of malignancy, cardiovascular, renal, and pulmonary disease vs other infectious diseases. To resolve these problems, relevant paradigms from parasitology must be used in the teaching of cell biology, molecular biology, genetics, and immu nology.

Each of these impacts will decrease food and nutrition security, primarily by reducing access to and availability of food, and also by increasing the risk of infectious disease. Although increased biofuel demand has the potential to increase incomes among producers, it can also negatively affect food and nutrition security. Land used for cultivating food crops may be diverted to biofuel production, creating food shortages and raising prices. Accelerations in unregulated or poorly regulated foreign direct investment, deforestation and unsustainable use of chemical fertilizers may also result. Biofuel production may reduce women’s control of resources, which may in turn reduce the quality of household diets. Each of these effects increases risk of poor food and nutrition security, either through decreased physical availability of food, decreased purchasing power, or increased risk of disease. The Impact of Climate Change and Bioenergy on Nutrition articulates the links between current environmental issues and food and nutrition security. Recently d- covered diseases such as AIDS pose a major threat to the population at large, and to that threat has been added the re-emergence of established pathogens, microbes that were re- ily treatable in the past. Since infectious diseases already play a major role in the burden of illness and mortality, health care providers and planners are worried. A large proportion of the problem is man-made, arising mainly from the unnecessary overuse of antimicrobials in hospital and community settings and from the agricultural misuse of the agents in animal feed. A consequence has been a dramatic increase in resi- ant strains of bacteria that were considered conquerable several decades ago. Community infections caused by multi-resistant pneumococci serve as an example. These organisms were readily treated with penicillin, but now the spread of penicillin-resistant Streptococcus pneumoniae from continent to continent is becoming a worldwide problem. This is a major concern because pneumococcal infections are common in the community, being the le- ing cause of pneumonia, sinusitis, and meningitis. Resistant bacteria in hospitals are also becoming more prevalent. We have become accustomed to hearing about methicill- resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE), but now we have to be concerned about multidrug-resistant coliform bacteria and pseudomonads.
