Book excerpt: An image registration technique is presented for the registration of medical images using a hybrid combination of coarse-scale landmark and B-splines deformable registration techniques. The technique is particularly effective for registration problems in which the images to be registered contain large localized deformations. A brief overview of landmark and deformable registration techniques is presented. The hierarchical multiscale image decomposition of E. Tadmor, S. Nezzar, and L. Vese, A multiscale image representation using hierarchical (BV;L2) decompositions, Multiscale Modeling and Simulations, vol. 2, no. 4, pp. 554{579, 2004, is reviewed, and an image registration algorithm is developed based on combining the multiscale decomposition with landmark and deformable techniques. Successful registration of medical images is achieved by first obtaining a hierarchical multiscale decomposition of the images and then using landmark-based registration to register the resulting coarse scales. Corresponding bony structure landmarks are easily identified in the coarse scales, which contain only the large shapes and main features of the image. This registration is then fine tuned by using the resulting transformation as the starting point to deformably register the original images with each other using an iterated multiscale B-splines deformable registration technique. The accuracy and efficiency of the hybrid technique is demonstrated with several image registration case studies in two and three dimensions. Additionally, the hybrid technique is shown to be very robust with respect to the location of landmarks and presence of noise.

This series of conference began in 1983 and is jointly organized by the YLL School of Medicine and Faculty of Engineering of the National University of Singapore and the Biomedical Engineering Society (Singapore). First of all, I want to thank Mr Lim Chuan Poh, Chairman A*STAR who kindly agreed to be our Guest of Honour to give th the Opening Address amidst his busy schedule. I am delighted to report that the 13 ICBME has more than 600 participants from 40 countries. We have received very high quality papers and inevitably we had to turndown some papers. We have invited very prominent speakers and each one is an authority in their field of expertise. I am grateful to each one of them for setting aside their valuable time to participate in this conference. For the first time, the Biomedical Engineering Society (USA) will be sponsoring two symposia, ie “Drug Delivery S- tems” and “Systems Biology and Computational Bioengineering”. I am thankful to Prof Tom Skalak for his leadership in this initiative. I would also like to acknowledge the contribution of Prof Takami Yamaguchi for organizing the NUS-Tohoku’s Global COE workshop within this conference. Thanks also to Prof Fritz Bodem for organizing the symposium, “Space Flight Bioengineering”. This year’s conference proceedings will be published by Springer as an IFMBE Proceedings Series.

Book excerpt: Deformable image registration (DIR) has various applications in medical image analysis such as in adaptive radiotherapy (ART) and multi-atlas segmentation. ART uses DIR to warp the dose from the daily cone-beam CT (CBCT) image to the planning computed tomography (CT) image to accommodate anatomical changes that occur between dose planning and dose delivery. DIR aims to find a geometric transformation between corresponding image data and brings them into a common coordinate frame. DIR is an inherently ill-posed problem that lacks a unique mapping between the voxels of the two images being registered. As such, one must regularize the registration to achieve physically meaningful transforms. The regularization penalty is usually a function of derivatives of the displacement vector field and can be calculated either analytically or numerically. The numerical approach, however, is computationally expensive depending on the image size, and therefore a computationally efficient analytical framework has been developed. Cubic B-splines were used to model the displacement vector field. A generalized mathematical framework was developed that supports five distinct regularizers: diffusion, curvature, linear elastic, third-order, and total displacement. The approach was validated by comparing each regularizer with its numerical counterpart in terms of accuracy. Benchmarking results show that the analytic solutions run significantly faster -- up to two orders of magnitude -- than finite differencing-based numerical implementations. Additionally, the cost function used in the DIR optimization process is a global, low-level cost function. It fails to accurately align high-level image structures such as organs at risk (OARs). DIR accuracy can be improved locally by adding high-level structures such as contours and landmark points. A hybrid image similarity metric that incorporates a point-to-distance map (PD) metric was developed. Given a pair of segmented images, structures on the fixed image are represented as sets of points while structures on the moving image are described as distance maps. The total distance of all fixed points to their associated boundaries on the moving image constitutes the PD metric, which is combined with the more traditional intensity similarity metric between the fixed and moving images. The approach was validated using the pelvic reference dataset wherein the prostate, bladder, and rectum were manually segmented from the CT and CBCT images by a medical expert to obtain the segmented fixed and moving images. The accuracy of the deformable registration was quantified using the Dice Similarity Coefficient (DSC) and 95% Hausdorff Distance (95% HD), with and without the PD metric. Results demonstrate a much-improved overlap between the fixed and warped contours once the PD metric was applied. Moreover, the computational overhead associated with adding the PD metric was minimal. Finally, to circumvent the need for a medical expert to manually segment the structures, a 3-D U-Net architecture was used to generate structures to be used with the PD Metric calculations. An image-to-image translation network is trained using a CycleGAN to domain adapt between CT and CBCT images to assist the auto-segmentation process. Experiments are performed to compare the DIR accuracy with the 3-D U-Net generated structures and the manually segmented structures. Results demonstrate that the auto-segmented images improve the DIR accuracy equally when using manually segmented images. This helps in reducing manual overhead associated with segmenting the images and thus increases the end-to-end performance of the DIR algorithm making it suitable for clinical use.

Chapters have been contributed by selected authors in the relevant domains of tissue engineering, medical image acquisition and processing, visualization, modeling, computer aided diagnosis and knowledge management. The multi-scale and multi-disciplinary research aspects of articulations in humans are highlighted, with a particular emphasis on medical diagnosis and treatment of musculoskeletal diseases and related disorders. The need for multi-scale modalities and multi-disciplinary research is an emerging paradigm in the search for a better biological and medical understanding of the human musculoskeletal system. This is particularly motivated by the increasing socio-economic burden of disability and musculoskeletal diseases, especially in the increasing population of elderly people. Human movement is generated through a complex web of interactions between embedded physiological systems on different spatiotemporal scales, ranging from the molecular to the organ level. Much research is dedicated to the understanding of each of these systems, using methods and modalities tailored for each scale. Nevertheless, combining knowledge from different perspectives opens new venues of scientific thinking and stimulates innovation. Integration of this mosaic of multifaceted data across multiple scales and modalities requires further exploration of methods in simulations and visualization to obtain a comprehensive synthesis. However, this integrative approach cannot be achieved without a broad appreciation for the multiple research disciplines involved.

Previous WBIRs took place in Bled, Slovenia (1999), at the UniversityofPennsylvania,USA(2003)andinUtrecht,TheNetherlands(2006). This year, WBIR was hosted by the Institute Mathematics and Image Proce- ing and the Fraunhofer Project Group on Image Registration and it was held in Lub ̈ eck, Germany. It provided the opportunity to bring together researchers from all over the world to discuss some of the most recent advances in image registration and its applications. We had an excellent collection of papers that were reviewed by at least three reviewers each from a 35-member Program Committee assembled from a wor- wide community of registration experts. This year 17 papers were accepted for oral presentation, while another 7 papers were accepted as poster papers. We believe all of the conference papers were of excellent quality. Registration is a fundamental task in image processing used to match two or more pictures taken, for example, at di?erent times, from di?erent sensors, or from di?erent viewpoints. Establishing the correspondence of structures within medical images is fundamental to diagnosis, treatment planning, and surgical guidance. The conference papers address state-of-the-art techniques for prov- ing reliable and e?cient registration techniques, thereby imposing relationships between speci?c application areas and appropriate registration schemes. We are grateful to all those who contributed to the success of WBIR 2010.

Anatomical data from CT and MRI are useful for analyzing or further processing functional data from techniques such as PET and SPECT. When data are not acquired simultaneously, even when these data are acquired on a dual-imaging device using the same bed, motion can occur that requires registration between the reconstructed image volumes. As the human torso can allow non-rigid motion, this type of motion should be estimated and corrected. We report a deformation registration technique that utilizes rigid registration for bony structures, while allowing elastic transformation of soft tissue to more accurately register the entire image volume. The technique is applied to the registration of CT and MR images of the lumbar spine. First a global rigid registration is performed to approximately align features. Bony structures are then segmented from the CT data using semi-automated process, and bounding boxes for each vertebra are established. Each CT subvolume is then individually registered to the MRI data using a piece-wise rigid registration algorithm and a mutual information image similarity measure. The resulting set of rigid transformations allows for accurate registration of the parts of the CT and MRI data representing the vertebrae, but not the adjacent soft tissue. To align the soft tissue, a smoothly-varying deformation is computed using a thin platespline(TPS) algorithm. The TPS technique requires a sparse set of landmarks that are to be brought into correspondence. These landmarks are automatically obtained from the segmented data using simple edge-detection techniques and random sampling from the edge candidates. A smoothness parameter is also included in the TPS formulation for characterization of the stiffness of the soft tissue. Estimation of an appropriate stiffness factor is obtained iteratively by using the mutual information cost function on the result of the global deformable transformation.

Book excerpt: With the availability of big image datasets and state-of-the-art computing hardware, data-driven machine learning approaches, particularly deep learning, have been used in numerous medical image (CT-scans, MRI, PET, SPECT, etc..) computing tasks, ranging from image reconstruction, super-resolution, segmentation, registration all the way to disease classification and survival prediction. However, training such high-precision approaches often require large amounts of data to be collected and labelled and high-capacity graphics processing units (GPUs) installed, which are resource intensive and hence not always practical. Other hurdles such as the generalization ability to unseen new data and difficulty to interpret and explain can prevent their deployment to those clinical applications which deem such abilities imperative.
