The initial papers were concerned with descriptions of endemic cretinism by investigators who have had direct experience with field studies in various parts of the world. Reports were presented from Andean South America, the Himalayas, New Guinea, Switzerland, Italy, and Central Africa. Resolution of some of the seemingly conflicting information was achieved. The conference then addressed the problems of definition, assessment, and field ascertainment of cretinism.

Even though the specific preventative agent is known, we do not totally understand the neurobiological questions raised.

Virtually all studies report higher prevalence rates for hypothyroidism in women and with advancing age. Abnormal thyroid functions have important public health consequences. Suppressed TSH levels have been associated with decreased bone density and with an increased risk of atrial fibrillation and premature atrial beat. It has been known for decades that overt hypothyroidism contributes to elevated serum cholesterol levels and cardiovascular risk and recent studies suggest this may also be true with subclinical hypothyroidism.

Differential growth creates form: external form through growth rates which vary from one part of the body to another and one tissue to another; and internal form through the series of time-entrained events which build up in each cell the special ized complexity of its particular function. We make no distinction, then, between growth and development, and if we have not included accounts of differentiation it is simply because we had to draw a quite arbitrary line somewhere. It is only rather recently that those involved in pediatrics and child health have come to realize that growth is the basic science peculiar to their art. It is a science which uses and incorporates the traditional disciplines of anatomy, physiology, biophysics, biochemistry, and biology. It is indeed a part of biology, and the study of human growth is a part of the curriculum of the rejuvenated science of Human Biology. What growth is not is a series of charts of height and weight. Growth standards are useful and necessary, and their construction is by no means void of intellectual challenge. They are a basic instrument in pediatric epidemiology. But they do not appear in this book, any more than clinical accounts of growth disorders. This appears to be the first large handbook-in three volumes-devoted to Human Growth.

However, fluoride can also enter public water systems from natural sources, including runoff from the weathering of fluoride-containing rocks and soils and leaching from soil into groundwater. Fluoride pollution from various industrial emissions can also contaminate water supplies. In a few areas of the United States fluoride concentrations in water are much higher than normal, mostly from natural sources. Fluoride is one of the drinking water contaminants regulated by the U.S. Environmental Protection Agency (EPA) because it can occur at these toxic levels. In 1986, the EPA established a maximum allowable concentration for fluoride in drinking water of 4 milligrams per liter, a guideline designed to prevent the public from being exposed to harmful levels of fluoride. Fluoride in Drinking Water reviews research on various health effects from exposure to fluoride, including studies conducted in the last 10 years.
