The meeting was organized to review recent advances in basic and clinical research work on hormone-responsive tumors of the reproductive system. The association between protracted hormone action and cancer of the reproductive system is now irrefutable. Yet we still do not know how hormones initiate and promote neoplastic transformation of their target cells. A major effort is cur rently being directed at understanding the hormonal regulation of neoplastic cells, especially those arising from the breast, uterus, pituitary gland and prostate. The symposium brought together leading experts whose research is focused on this impor tant area of human health. Although many questions remain to be answered by future studies, the material covered in this volume provides an up-to-date overview of the current status of work in this area. The subjects range from the mechanism of hormone action to the treatment of hormone-dependent tumors, and topics include estrogen, progestin, androgen and glucocorticoid action~ hormone receptors~ specific protein responses~ developmental effects of estrogens and antiestrogens~ the DES syndrome~ regu lation of the cell cycle~ therapeutic effects of antiestrogens, aromatase inhibitors, iodoestrogens, progestins on estrogen dependent tumors~ characterization of retinoid-binding sites~ and the utility of various tumor markers.

Progress in our understanding of hormonal carcinogenic processes has been a direct result of the advances made in cell biology, endocrinology, and carcinogenesis at the molecular level. The newer fields of molecular genetics and cytogenetics already have and are expected to continue to playa major role in furthering our understanding of the cellular and molecular events in hormonal carcinogenesis. It has become increasingly clear that the risk of naturally occurring sex hormones in carcinogenic processes, both in human and in animal models, requires only minute quantities of hormones, at both the serum and tissue levels. Moreover, hormone target tissues for neoplastic transformation, perhaps with the exception of the liver, generally have relatively modest ability to metabolize sex hormones, such as the breast and prostate. Table 1 summarizes the serum, and in most cases, the tissue levels of sex hormones, both endogenously and exogenously ingested, which are associated with increased risk for endocrine-associated cancers such as breast, endometrium, and prostate, as well as the hormone levels of four experimental models that have been shown to elicit high tumor incidences. In contrast to the human, in which the hormone levels are cyclic, however, the latter require continuous hormone exposure at these relatively low levels.

Although most of breast and prostate cancers initially respond to hormone therapy, most tumors reinitiate to growth. Finally, hormone-resistant and metastatic breast and prostate cancers may develop. Thus, the challenge is the dissection of mechanisms by which steroid receptor signaling pathways continue to influence cell growth and invasiveness. Compelling evidence indicates that steroid hormones elicit non-genomic responses in extra-nuclear compartment of target cells. In this cellular location, steroid-coupled receptors rapidly recruit signaling effectors or scaffold proteins and activate multiple pathways leading to proliferation, survival, migration and invasiveness. The immediate challenge is the dissection of key events regulating the steroid response of target tissues to prevent progression and improve treatment of breast and prostate cancers.

Book excerpt: The concept that hormones influence tumor growth originated in 1889 with the proposal of Albert Schinzinger who suggested that breast cancer is related to the ovaries. Several years later, Sir George Beatson observed that remission of disseminated breast cancer could be achieved in premenopausal patients by performing bilateral oophorectomy. As a result of the contri butions of Hedley Atkins, Charles Huggins and others, additive and ablative hormonal therapies have been widely used for the treatment of advanced breast cancers for several decades. Model systems to study the effects of hormones on growth and regression of breast tumors have been available for many years; however, the complexities of the hormonal environment have rendered in vivo studies difficult in man and experimental animals. Recently, the availability of long-term cultures of breast cancer cells has stimulated many investigators to use these cell lines to unravel the mechanisms of hormone action. Because of the extreme diversity and complexity of advances regarding the endocrinology of the breast and breast cancers, a multi-authored review was deemed necessary. It has been gratifying to receive contributions from many noted scholars. In Volume I of this monograph, the influence of steroid hormones and their antagonists upon normal and neoplastic tissues of the mammary gland are presented. In Volume II, the effects of peptide and other hormones are reviewed.

This symposium was organized to bring together basic scientists and clinicians for the purpose of exchanging new ideas and the latest information in the area of Steroid Hormone Action and Cancer. The design of the symposium included both formal plenary sessions and informal roundtable discussion groups, the chapters of this volume being drawn primarily from these proceedings. During the last quarter of a century considerable progress has been made toward understanding the molecular mechanisms in volved in steroid hormone action. It now appears that the mechan sim of action of the four major classes of steroid hormones is qualitatively similar. Research during the past decade has demonstrated steroid hormone receptors in a variety of normal and neoplastic tissues. Receptor-containing neoplasms have been shown to be hormone-dependent and undergo regression when treated with hormone antagonists. Natural and synthetic steroids also have been employed for many years to successfully treat various types of cancer, for example: estrogens, androgens and progestagens for breast cancer; estrogens and progestagens for prostatic carcinoma; progestagens for endometrial carcinoma; and corticoids for leu kemias. All of these neoplasms have now been found to possess receptors for the steroids empirically used in their treatment.
