The updated information in this publication will help readers understand what histoplasmosis is and recognize activities that may expose workers to the disease-causing fungus Histoplasma capsulatum. The publication also informs readers about methods they can use to protect themselves and others from exposure. Outbreaks of histoplasmosis have shared similar circumstances: People who did not know the health risks of breathing in the spores of H. capsulatum became ill and sometimes caused others nearby to become ill when they disturbed contaminated soil or accumulations of bird or bat manure. Because they were unaware of the hazard, they did not take protective measures that could have prevented illness. This publication will help prevent such exposures by serving as a guide for safety and health professionals, environmental consultants, supervisors, and others responsible for the safety and health of those working near material contaminated with H. capsulatum. Activities that pose a health risk to workers at these sites include disturbance of soil at an active or inactive bird roost or poultry house, excavation in regions where this fungus is endemic, and removal of bat or bird manure from buildings. Local, State, and national public health professionals may also find this publication useful for understanding the health risks of exposure to H. capsulatum so that they can provide guidance about work practices and personal protective equipment.

The National Institute for Occupational Safety and Health (NIOSH) estimates that deaths from work-related respiratory diseases and cancers account for about 70% of all occupational disease deaths. NIOSH conducts research in order to detect and reduce work-related hazardous exposures, injuries, and diseases; its Respiratory Disease Research Program (RDRP) focuses on respiratory diseases. This National Research Council book reviews the RDRP to evaluate the 1) relevance of its work to improvements in occupational safety and health and 2) the impact of research in reducing workplace respiratory illnesses. The assessment reveals that the program has made essential contributions to preventing occupational respiratory disease. The National Research Council has rated the Program a 5 out of 5 for relevance, and a 4 out of 5 for impact. This disease is highly endemic in some regions of North America, Central America, and South America and is also reported in certain countries of Asia and Africa. It often affects people with impaired immunity, including people living with HIV, among whom the most frequent clinical presentation is disseminated histoplasmosis. The symptoms of disseminated histoplasmosis are non-specific and may be indistinguishable from those of other infectious diseases, especially disseminated tuberculosis (TB), thus complicating diagnosis and treatment. Histoplasmosis is one of the most frequent opportunistic infections caused by fungal pathogens among people living with HIV in the Americas and may be responsible for 5-15% of AIDS-related deaths every year in this Region. These guidelines aim to provide recommendations for the diagnosis, treatment, and management of disseminated histoplasmosis in persons living with HIV. Although the burden of disease is concentrated in the Americas, the recommendations presented within these guidelines are applicable globally. These guidelines were produced in accordance with the World Health Organization (WHO) handbook for guideline development. The Guideline Development Group elaborated the final recommendations based on a systematic review of scientific literature and critical evaluation of the evidence available using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach. These guidelines are intended for health-care providers, HIV program managers, policy-makers, national treatment advisory boards, researchers, and other professionals involved in caring for people who either have or may be at risk of developing disseminated histoplasmosis.
