Eleven chapters cover all aspects of research design, including: developing study objectives, defining and refining study questions, addressing the heterogeneity of treatment effect, characterizing exposure, selecting a comparator, defining and measuring outcomes, and identifying optimal data sources. Checklists of guidance and key considerations for protocols are provided at the end of each chapter. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Book excerpt: This book pursues a nonlinear approach in considering both chaotic dynamical models and agent-based simulation models of economics, as well as their dynamical behaviors. Three key concepts arising in this context are “nonlinearity,” “bounded rationality” and “heterogeneity,” which also make up the title of the book. Nonlinearity is the warp that runs throughout all models because systems that exhibit chaotic or other complex behavior in the absence of any exogenous disturbances are absolutely nonlinear. Bounded rationality constitutes the woof, because economic systems do not exhibit complex behavior if all agents are perfectly rational, as is usually assumed in neoclassical economics. Agents who are boundedly rational have to struggle to do their best with limited information and tend to adapt to their economic environment without knowing what is the best. Furthermore, the heterogeneity of firms or consumers dyes the fabric of complex dynamics woven from the warp and woof.

Book excerpt: Chronic hepatitis B (CHB) is a life-threatening liver disease affecting 257 million people worldwide, in particular in the Asia-Pacific regions. In endemic areas, hepatitis B virus (HBV) is usually transmitted from chronically infected mothers to neonates. Perinatal HBV infection causes chronic infection in more than 90% of exposed individuals. With perinatal infection, lifetime mortality risk due to complications of liver cirrhosis (LC) or hepatocellular carcinoma (HCC) reaches up to 40% in men and 15% in women. For the treatment of chronic HBV infection, nucleos(t)ide analogue antivirals have been successfully used to suppress viral replication. However, HBV exists as a cccDNA, which cannot be eliminated by nucleos(t)ide analogues. Therefore, a practical goal of novel HBV therapeutics can be HBs seroconversion (loss of HBsAg and development of HBsAg-specific antibodies), which occurs during spontaneous recovery from acute HBV infection. This HBs seroconversion is referred to as “functional cure” of HBV infection. When functional cure is reached, HBsAg-specific antibodies have virus-neutralizing activity and control HBV infection even in the presence of cccDNA. Currently, peg-IFN-a is often used to induce HBs seroconversion in patients with chronic HBV infection; however, the efficacy is not satisfactory. In future, other immunological therapeutics must be considered to achieve HBs seroconversion, including therapeutic vaccines and immune checkpoint blockers.

In this paper we study the dynamic behavior of a general equilibrium menu-cost economy where firms are heterogeneous in the shocks they perceive, and the demands and adjustment Costs they face. In this context we (i) generalize the Caplin and Spulber [1987] steady state monetary neutrality result; (ii) show that uniqueness of equilibria depends not only on the degree of strategic complementarities but also on the degree of dispersion of firms' positions in their price-cycle; (iii) characterize the path of output outside the steady state and show that as strategic complementarities become more important, expansions become longer and smoother than contractions; and (iv) show that the potential effectiveness of monetary policy is an increasing function of the distance of the economy from its steady state, but that an uninformed policy maker will have no effect on output on average.
