Dubbed "the Royal Disease" because of its identification with Queen Victoria, the world's most renowned carrier, hemophilia is a genetic disease whose sufferers had little recourse until the mid-twentieth century. In the first book to chronicle the emergence and transformation of the hemophilia community, Susan Resnik sets her story within our national political landscape—where the disease is also a social, psychological, and economic experience. Blood Saga includes many players and domains: men with hemophilia and their families, medical personnel, science researchers, and the author herself, who was Director of Education of the National Hemophilia Foundation in the early 1980s. At that time the "miracle treatment" of freeze-dried pooled plasma blood products enabled men with hemophilia to lead full, normal lives. Then the AIDS virus infiltrated the treatment system and over fifty percent of the hemophilia community became HIV-positive. But rather than collapsing, this community refocused its priorities, extended its reach, and helped shape blood safety policies to prevent further tragedy. The hemophilia community includes people from every socioeconomic and ethnic group, and Resnik's narrative and use of oral histories never lose touch with those affected by the disease. Her extensive informant interviewing allows much of this social history to be told by participants on all levels: parents, wives, nurses, doctors, government agency directors, health care providers, and many others. Gene insertion therapy now holds the promise of a cure for hemophilia in the near future. Scientific breakthroughs inevitably become intertwined with the industry and academic medical centers that govern the national health care system. This book provides a comparative perspective on the political, legal, and social struggles that emerged in response to the HIV contamination of the industrialized worlds blood supply. It describes how eight nations responded to the first signs that AIDS might be transmitted through blood, and how they falteringly arrived at and finally implemented measures to secure the blood supply. The authors detail the remarkable saga of the mobilization of hemophiliacs who challenged the state, the medical establishment, and even their own caregivers as they sought recompense and justice. In the end, the blood establishments in almost every advanced industrial nation were shaken. In Canada, the Red Cross was forced to withdraw from blood collection and distribution. In Japan, pharmaceutical firms that manufactured clotting factor agreed to massive compensation -- $500,000 per hemophiliac infected. In France, blood officials went to prison. In spite of the dramatically improved life situation of hemophiliacs, hemorrhagic complications remain a threat. The hemorrhagic disorder may influence and/or aggravate the course of trauma or other diseases in these patients. Hemophiliacs suffering from hemorrhagic complications or eligible for elective surgery should be referred to Hemophilia Treatment and Training Centers, where evaluation and examination are performed by a multidisciplinary team with experience and interest in the disease. The radio10gist is an important member ofthis team. In the past diagnostic imaging has been based mainly on conventional radiography, and this is still very important for the diagnosis ofhemophilic complications, but the diagnostic imaging of today offers a wide range of modalities-conventional radiography, computed tomography, sonography, radionuc1ide imaging, and, in its infancy, magnetic resonance imaging. Thus there is a need for a thorough description of the potential and limitations ofthese modern diagnostic techniques.
