Available in PDF, EPUB and Kindle. Book excerpt: Background: The pursuit of equity in health and healthcare has been the key feature of health policy in India. However, despite the policy significance, the volume of literature available on this issue is scarce. Therefore, this paper is an attempt to examine the horizontal inequities in healthcare utilization, consisting of outpatient and inpatient care in 15 major states and north-eastern region of India.Methods: Cross-sectional data were taken from the National Sample Survey Organization (NSSO) 60th round (2004), the survey on 'morbidity and healthcare'. While outpatient care was assessed using the probability of outpatient visit 15 days prior to the survey date, the indicators of inpatient care utilization were based on the following variables: the probability of hospital admission and length of stay in hospital over a 12-month period. All these measures of healthcare utilization were standardized for need differences and controlled for socio-economic factors. Need standardized concentration indices were used to measure interstate and intrastate income-related inequities in healthcare utilization.Results: Absolute inequalities were found between states in the proportion of the population reporting a visit to an outpatient provider, in the range of 4.42% to 21.72%. Similarly, inpatient care varied from 1% to 10%. The magnitude of inequity for both outpatient and inpatient care was pro-rich across rural and urban areas of India and in majority of the states. In fact, in majority of the states, the horizontal inequity across types of curative care was noticeably higher within the rural population than in the urban population. The analysis demonstrated that high per capita government health spending was significantly associated with low inequity in utilization of inpatient care.Conclusion: The study concludes that it would be necessary to address the prevailing inequities in healthcare by substantially scaling up the public spending on health, and achieving effective universal coverage of healthcare in India.

Book excerpt: Globally estimates of maternal mortality indicate that more than half a million women died due to pregnancy related causes and these deaths are unjust and can be avoided with key health interventions, like provision of antenatal care and medically assisted delivery. The prime objective of book is to examine the trend of economic inequalities in utilization of RCH services in India and to quantify the contribution of selected factors explaining the rural-urban gap in use of RCH care utilization by using the NFHS-1,2,3 data. To fulfill the aforesaid objectives concentration index and non-linear decomposition technique have been used. The study reveals a sluggish increase in the utilization of RCH services in India with respect of place of residence. The trends of economic inequalities in utilization of RCH services are decreasing in both urban and rural areas but the pace of declining is more in rural areas than urban areas during the study period. Decomposition results indicates that economic status of the household is the main contributor in urban-rural differential in receiving the reproductive and child health care services followed by mother education level and media exposure.

Book excerpt: While the global maternal mortality ratio has significantly declined since 1990, a large number of women still die in child birth, with most of these deaths occurring in sub-Saharan Africa and South Asia. Evidence suggests that use of maternal health care services during delivery can substantially avert maternal mortality. This paper uses India's 2005-06 National Family Health Survey (NFHS-3) household data to investigate the economic, demographic and socio-cultural factors that determine a woman's use of professional delivery care services in India. Recognizing evidence which suggests that men are the key decision makers in India, this paper tests the impact of a husband's involvement in a woman's pregnancy on her use of these services, as its main variable of interest. A husband's involvement is measured by his presence at a woman's antenatal visit to the health clinic. The multivariate regression analysis undertaken in this paper finds that a woman whose husband is present at her antenatal visit is 17.06 percentage points more likely to deliver in a health facility. Further, the analysis also finds factors such as a mother's education level, place of residence (urban/rural) and household wealth to be significant in determining a woman's use of professional delivery care services. These results are important for designing policies aimed at increasing the use of maternal health care services in India and highlight the importance of involving men (husbands) as an integral part of reproductive health care interventions.

It is against this backdrop that the Division of Behavioral and Social Research at the U.S. National Institute on Aging (NIA) asked the National Research Council (NRC), through the Committee on Population, to undertake a project on advancing behavioral and social research on aging in Asia. Book excerpt: The quality of medical care received by patients varies for two reasons: differences in doctors' competence or differences in doctors' incentives. Using medical vignettes, the authors evaluated competence for a sample of doctors in Delhi. One month later, they observed the same doctors in their practice. The authors find three patterns in the data. First, what doctors do is less than what they know they should do-doctors operate well inside their knowledge frontier. Second, competence and effort are complementary so that doctors who know more also do more. Third, the gap between what doctors do and what they know responds to incentives: doctors in the fee-for-service private sector are closer in practice to their knowledge frontier than those in the fixed-salary public sector. Under-qualified private sector doctors, even though they know less, provide better care on average than their better-qualified counterparts in the public sector. These results indicate that to improve medical services, at least for poor people, there should be greater emphasis on changing the incentives of public providers rather than increasing provider competence through training.
