This book introduces and explicates the end-to-end resource tracking and management (RTM) framework, which includes five components that determine effective and efficient financing for PHC: resource mobilization, allocation, utilization, productivity, and targeting.In addition, this book compiles detailed results from the most recent RTM-based resource tracking efforts for PHC in selected countries. This is to demonstrate how the RTM framework can be used to bring a set of separate resource tracking efforts at different stages of flow of funds into a comprehensive process with an end-to-end 'storyline'. In order to build a functional PHC system that addresses access, quality, and equity issues, this book highlights the key (public) financing issues that researchers, technical advisors, and policy makers would need to address in addition to more resources.

PHCs essential attributes (people and family centered care, comprehensiveness, continuity, longitudinality) position this approach as one of the key strategies for countries to meet the aspiration of achieving universal health coverage. Creating care networks has been a common thread running through Latin America and the Caribbeans health policy agendas. In terms of actually putting the IHSDN model in action, there is a wide range of interpretations and experiences, with designs, scales, organizational methods, and maturity levels that vary within and between countries. This book shares evidence of the progress made in forming and launching IHSDN in Latin America based on four case studies conducted in Argentina, Brazil, Colombia, and Mexico. The results were found by systematically applying an instrument that collects regional information on the context and features of the IHSDNs governance, funding, care models, and IHSDN management models. The books chapters describe the characteristics of IHSDN in the four studied countries, lessons are drawn from how these IHSDN have been designed and implemented, challenges for the future are identified and recommendations are provided on what will it take to consolidate the IHSDN model in Latin America. The hypothetical story of Dioselina, illustrates throughout the book the obstacles and difficulties that arise for a diabetic patient when using health services that are not people-centered. The results shed light on how prepared IHSDN in this region are to provide patient-centered care and where to focus efforts for improvement. The evidence found in this study will help develop and advance PHC in Latin America.

Regardless of culture or context, persons with disabilities are almost always pushed to the bottom of the social hierarchy. With the advent of the Convention on the Rights of Persons with Disabilities (2006), disability human rights seemingly provided a path forward for tearing down ableist social hierarchies and ensuring that all persons with disabilities everywhere were treated equally. Despite important progress, the disability human rights project not only remains incomplete, but has often created new hierarchies among persons with disabilities themselves or across the human rights it promotes. Certain groups of persons with disabilities have gained new voices while others remain silenced and certain rights are prioritized over others depending on what states, international organizations, or advocates want rather than what those on the ground need most. This volume was inspired both by the continued need to expose human rights violations against persons with disabilities, but to also explore the nuanced role that hierarchies play in the spread, implementation, and protection of disability human rights. The enjoyment of human rights is not equal nor is the recognition of specific individuals and groups’ rights. Los compromisos polfticos se han traducido en general en presupuestos mas elevados yen leyes aprobadas que circunscriben los fondos destinados al sector salud. En la mayorfa de los pafses se ha priorizado la atenci6n primaria en salud por su costo efectividad y han adoptado metodos de adquisici6n que incentivan la eficiencia y la transparencia en los resultados y que les dan a los administradores del sector salud un mayor apalancamiento para dirigir a los proveedores hacia la . consecuci6n de las prioridades de salud publica. Sin embargo, a pesar de los avances. aun subsisten las disparidades en el financiamiento y calidad de los servicios de los subsistemas de salud. Cumplir con el compromiso de la cobertura universal de salud exigira esfuerzos concertados para mejorar la generaci6n de los ingresos fiscales de manera sostenible y de elevar la productividad y efectividad del gasto. En el re po rte Hacia la cobertura universal en salud v la equidad en America Latina v el Caribe: Evidencia de poises seleccionados. los autores sefialan que la evidencia tomada de un analisis de 54 encuestas de hogares corrobora que las inversiones en la ampliaci6n de la cobertura estan generando resultados. A pesar que los pobres aun presentan los peores fndices de salud en comparaci6n con los ricos, las disparidades se han reducido en gran medida, especialmente durante las primeras etapas de vida. Los pafses han alcanzado elevados niveles de cobertura y equidad en la utilizaci6n de los servicios de salud maternoinfantil. El panorama se torna mas lleno de matices y no tan positivo cuando se trata de la salud en adultos y de la prevalencia de condiciones y enfermedades cr6nicas. La cobertura de las intervenciones de enfermedades no transmisibles no es muy alta y la utilizaci6n de los servicios se inclina hacia aquellos que gozan de una mejor posici6n. La prevalencia de las enfermedades no transmisibles no ha mostrado el comportamiento esperado en vista de la cafda en las tasas de mortalidad: un mayor acceso a los servicios de diagn6stico por pa rte de los segmentos mas ricos podrfa encubrir cambios en la prevalencia real. Los gastos de salud provocados por situaciones catastr6ficas han ido en descenso en la mayorfa de los pafses. El panorama en torno a la equidad, sin embargo es mixto, apuntando hacia limitaciones en la medida. Si bien la tasa de empobrecimiento a causa de gastos de salud es baja, con tendencia decreciente en terminos generales, entre dos y cuatro millones de personas en los pafses estudiados aun caen por debajo de la lfnea de la pobreza despues de haber incurrido en gastos de salud. Los esfuerzos para darle un seguimiento sistematico a la calidad de la atenci6n en la region, apenas dan sus primeros pasos. Sin embargo, un repaso a la literatura revela graves deficiencias en la calidad de la atenci6n de salud, asi como sustanciales diferencias entre los diversos subsistemas. Elevar la calidad de la atenci6n y asegurar la sostenibilidad de las inversiones en salud siguen siendo una agenda inconclusa.

Why does the ideology of health promotion and health in all policies mainly permeate structures of the health sector, but not other sectors? The main hypothesis of this approach is that, the greater the interactions between organisms of different trophic levels (edaphic organisms, multiple crops, weed plants, herbivores, carnivores, plants in living fences, corridors or forest patches within agroecosystems), the greater will be the possibilities of obtaining abundant and varied harvests, with fewer external inputs (pesticides, fertilizers) and better environmental performance. The agrobiodiversity is meant as the variety and the disposition of the cultivations, pastures, farms, that affect the soil properties and create habitat diversity, landscape diversity and connectivity. At the level of landscapes or territories, the set of biodiverse agroecosystems generate natural matrices that have a powerful impact on the recovery and restoration of forest corridors, which, in turn, positively influence many ecosystem services for nature conservation and free movement and recovery of many populations of animal species, including those in danger of extinction.

Its performance can be measured based on its ability to respond to the needs of the population through actions such as increasing equality in access to health, reducing health risks and protecting individuals from the financial consequences caused by hight costs deseases. In this way, it is imperative that health systems have an efficient design that allows guaranteeing the protection of the right to health.

This Element proposes a conceptualization of programmatic participation and distinguishes between two types, monitoring and policy-making. Falleti and Cunial review the origins of state-sanctioned institutions that mandate community participation in health in the two world regions with most advanced social welfare systems, Western Europe and Latin America, implying a comparative analysis of eleven health care systems. They argue that the origins of participatory institutions help account for the resulting types of programmatic participation. They delve deeper into the study of the experience of participation for policy making and analyze two hundred local participatory projects in public health.
