Book excerpt: The Archaeological Investigations Project (AIP), funded by English Heritage, systematically collected information about the nature and outcomes of more than 86,000 archaeological projects undertaken between 1990 and 2010. This volume looks at the long-term trends in archaeological investigation and reporting, places this work within wider social, political, and professional contexts, and reviews its achievements. Information was collected through visits to public and private organizations undertaking archaeological work. Planning Policy Guidance Note 16: Archaeology and Planning (known as PPG16), published in 1990, saw the formal integration of archaeological considerations with the UK town and country planning system that, and set out processes for informed decision-making and the implementation of post-determination mitigation strategies, defined a formative era in archaeological practice and established principles that underpin today’s planning policy framework. The scale of activity represented – more 1000 excavations per year for most of the PPG16 Era – is more than double the level of work undertaken at peak periods during the previous three decades. This comprehensive review of the project presents a wealth of data. A series of case studies examines the illustrate different types of development project, revealing many ways in which projects develop, how archaeology is integrated with planning and execution, and the range of outputs documenting the process, and identified a series of ten important lessons that can be learned from these investigations. Looking into the post-PPG16 Era, the volume considers anticipated developments in the changing worlds of planning, property development, and archaeological practice and proposes the monitoring of archaeological investigations in England using a two-pronged approach that involves self-reporting and periodic strategic overviews.

This work presents primary evidence from over 900 documents, interviews, and participant observations, and provides the first descriptive history of local food movement national policy achievements in the US, from 1976 to 2013, and in the UK, from 1991 to 2013, together with reviews of both the American and British local food movements. It provides a US-UK comparative context, significantly updating earlier comparisons of American, British and European farm and rural policies. The comparative perspective shows that, over time, more effective strategies for national policy change required social-movement building strategies, such as collaborative policy coalitions, capacity-building for smaller organizations, and policy entrepreneurship for joining together separate rural, farming, food, and health interests. In contrast, narrowly-defined single issue campaigns often undermined long-term policy change, even if short-term wins emerged. By profiling interviews of American and English movement leaders, policymakers, and funders, the book demonstrates that democratic participation in food policy is best supported when funders incentivize groups to work together and overcome their differences.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Social participation is defined as empowering people, communities and civil society, through inclusive participation in decision-making processes that affect health, across the policy cycle and at all levels of the system. It is core to primary health care and promotes equitable progress towards universal health coverage, producing more responsive health policies and programmes, and helping to foster population trust with the health system. While Member States have endorsed the principle of social participation, translation into practice has been inadequate. The following priority actions are identified: (a) Strengthen government capacities to design and implement social participation (b) Secure equitable, diverse and inclusive representation (c) Ensure that social participation informs decision-making for health across the policy cycle (d) Systematize and sustain regular social participation, including through legal frameworks (e) Invest adequate, stable and predictable financial resources for social participation (f) Facilitate capacity strengthening and financial resources for civil society (g) Monitor and use data and evidence routinely to evaluate participatory processes and their impact on decisions made.

When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
