PEPFAR supports HIV/AIDS prevention, treatment, and care services, primarily in Africa as well as in Asia and the Caribbean. The Office of the U.S. Global AIDS Coordinator leads implementation of PEPFAR. The Dept. of Health and Human Services' Centers for Disease Control and Prevention and the U.S. Agency for International Development are among PEPFAR's primary implementing agencies. This report examined practices used in: (1) selecting organizations to implement PEPFAR activities; and (2) overseeing these organizations' PEPFAR activities. Illustrations.

The Pres. intends to ask Congress to authorize $30 billion for these efforts for the next 5 years. In 2007, the Institute of Med. recommended eliminating the directives. This report describes: (1) the views of HIV/AIDS experts on these directives; (2) an alternative approach to allocating funds; and (3) potential challenges related to this approach.
