Consequences of diseases, e.g. impairments and activity limitations, are addressed in rehabilitation with the overall goal to reduce disability and promote participation. Based on the available best external evidence, clinical pathways are described for stroke rehabilitation bridging the gap between clinical evidence and clinical decision-making. The clinical pathways answer the questions which rehabilitation treatment options are beneficial to overcome specific impairment constellations and activity limitations and are well acceptable to stroke survivors, as well as when and in which settings to provide rehabilitation over the course of recovery post stroke. Each chapter starts with a description of the clinical problem encountered. This is followed by a systematic, but concise review of the evidence (RCTs, systematic reviews and meta-analyses) that is relevant for clinical decision-making, and comments on assessment, therapy (training, technology, medication), and the use of technical aids as appropriate. Based on these summaries, clinical algorithms / pathways are provided and the main clinical-decision situations are portrayed. The book is invaluable for all neurorehabilitation team members, clinicians, nurses, and therapists in neurology, physical medicine and rehabilitation, and related fields. It is a World Federation for NeuroRehabilitation (WFNR) educational initiative, bridging the gap between the rapidly expanding clinical research in stroke rehabilitation and clinical practice across societies and continents. It can be used for both clinical decision-making for individuals and as well as clinical background knowledge for stroke rehabilitation service development initiatives.

Effectiveness of rehabilitation programs with patientu2019s after stroke has been proved by research, but which methods works most effectively and should be widely applied, are still in discussion. Purpose: Investigate the effect of different treadmill walking training on gait and balance in stroke patients. Materials and methods: A total of 36 stroke subjects who underwent rehabilitation at Vilnius University Hospital Santaros klinikos, Rehabilitation, Physical and Sports Medicine Centre were recruited in this study. Subjects were randomly assigned into two groups. All subjects received a routine individual physiotherapy two times per day for 30 u2013 45 minutes, 5 days per week. The subjects in the 1st group additional received 10-20 minutes gait training of walking forward on treadmill. The subjects in the 2nd group also received additional 10-20 minutes gait training of walking backward on treadmill. Balance parameters were assessed using Berg Balance Test, gait parameters were evaluated using 10 Meter Walk Test and measuring step length and width. Results: The results of balance and gait parametres have increased significantly in both groups during the research (p0,05). However, there was no significantly difference between the groups (p0,05) comparing the results of balance. Analysis of results of gait parametres tests revealed that in group which received backward training on treadmill significantly more improved gait speed and decreased the asymmetry of step length and width than in the group which received forward walking training on treadmill (p

Gait training is one of the major components in stroke rehabilitation. Treadmill walking is often used for gait training in people post stroke. Limited studies have examined the effects of a visual feedback system in combination with treadmill-based gait training. Purpose: The purpose of this study was to investigate the effects of treadmill walking with realtime visual feedback on gait outcomes in people post stroke. Methods: 6 participants (age 59.3+/-12.34 years old) participated in this case study. They were assigned to either visual feedback gait training group or control based on initial walking speed. Both groups performed 30 minutes of treadmill walking, three times a week, for eight weeks. The control group performed the training with no visual feedback (NVF). The experimental treatment group received real-time visual feedback (VF) on a LCD screen which displayed foot placement and prompts. Data collection was performed before and after the eight weeks, as well as four weeks after the completion of intervention. The kinematic and spatiotemporal variables were recorded and analyzed by using a 3D motion analysis system. (VICON Bonita System). Data process and analysis was performed using VICON Polygon software. x Results: Group averages demonstrated an increase in walking speed and cadence in the VF group. The VF group demonstrated an increase of 11.7% in cadence of the AF and NAF limb. Walking speed of the VF group increased by 21.7% from .95m/s to 1.16m/s. NVF group revealed no notable change in spatiotemporal, or kinematic variables. Neither VF nor NVF group demonstrated notable changes in the kinematic values or gait symmetry. Conclusion: The findings indicate that gait training with visual feedback can be more effective in improving gait spatiotemporal values than conventional treadmill walking.

Book excerpt: Abstract: ABSTRACT Purpose: The purpose of this study is to examine the effectiveness of treadmill walking with or without partial body-weight support (PBWS) in improving or restoring gait ability in patients post-CVA through an evidence based review of literature. Methods: A literature search was done for the years 1960 to February 2002. Selection criteria included randomized controlled trials. Study quality was assessed using four criteria and seven indicators. Treatment effect was assessed using effect sizes and confidence intervals. Results: Twenty-four studies were retrieved and reviewed. Of these, five studies were selected and outcome data were obtained for walking speed and balance. Effect sizes for walking speed were calculated for all five studies, ranging from -0.25 to 3.67. Effect sizes for balance were calculated for two studies and values ranged from 0.038 to 2.52. Discussion and Conclusions: Treadmill training with or without partial body-weight support appears to be an effective treatment for improving gait in patients post-CVA; however, the results do not conclusively indicate superiority to conventional forms of gait training. Given the evidence of little or no difference between intervention strategies, therapists need to consider cost, availability of resources, and patient preference when selecting the most appropriate therapy.
