Book excerpt: On May 14-16, 1982, a group of scientists met in Viareggio, Italy to present ideas, and exchange relevant data on current approaches on immunomodulators. We felt that the embodiment of the substance of that international symposium into this volume will be beneficial in keeping both clinicians and basic scientists abreast of the latest exciting developments in this rapidly accelerating field. We hope that the publication of this series of papers by an international panel of experts will enhance the nature of future inves t igat ive stud ies with such ent it ies . Under the rubric of immunology, it is obvious that such a term as "immunomodulation" could be construed as being all encompassing. However, it is intriguing that the immune modulators touched on in these sessions can be subcategorized into about five broad groups. Almost all substances, excepting those entities that are immuno logically inert, will have some modulating effects on immunity. From the coarse adjustment of antibody feedback inhibition upon cellular limnunity to the subtle finesse envisioned in Jerne's idiotypic network and s'lppressor T cells, immunomodulators attempt to either magnify or diminish those responses that are normally elicited by antigen. The broad groups - thymic hormones, D.L.E., Interferon, drugs, and the use of bacterial products appear to be at the forefront of much of the pioneering work on immunomodulation.

It is in this area that the current view of the immune system has arisen and developed. There is indeed little doubt that immune reactions are primarily determined by messages which are genera ted within the immune system and passed among different types of immunologie cells. This cell communication not only determines the type, intensity and duration of the response after perturbation of the immune system by exogenous antigens, but it is also essential for preventing autoimmune reactions and their clinical conse quences. In order to assure aperfect balance within the enormous com plexity of the immune system, it is not surprising that multiple self-regulatory mechanisms are organized at different levels, such as antibody feedback, idiotypic-anti-idiotypic responses, suppres sor and helper T cells, lymphokine signals and genetic require ments. A nu mb er of observations in recent years have, however, demonstrated that consistent contributions to the immunological homeostasis are given also by signals generated outside of the immune system, namely,in the central and autonomous nervous system as weIl as in the endocrine apparatus. Furthermore, the interactions between the immune system and the other body homestatic mechanisms seem to be bidirectional: if immunological cells may be targets of neuroendocrinological factors, immunological products seem in turn to contribute to the neuro endocrine homeostasis.

However, to date the origin of this chronic inflammatory disorder has not been elucidated. Evidence for the involvement of microbial agents is still tenuous, the rea son for the defective control of inflammation is still unknown, and the disease all too of ten progresses to crippling stages despite long-term treatment. As T. McKeown recently stated with regard to rheumatoid arthritis, "not enough is even known to indicate which is the more promising approach, and the pru dent course is to investigate both origins and mechanisms". While working on animal model diseases as well as on aspects of rheumatoid arthritis and related disorders in man, the members of the Special Research Area on patho physiology of rheumatoid inflammation in man and. animals (Sonderforschungsbereich 54 "Pa thomechanismen der rheumatoiden Entztindung bei Mensch und Tier"; maintained by the Deut sche Forschungsgemeinschaft) at the Veterinary and Medical Schools, Hannover, have been guided by the concept that by studying clinically and morphologically similar diseases in several species, mutually stimulating results might lead to improved knowledge of the pathophysio logy of rheumatoid arthritis. In particular, from working with model diseases in animals new approaches to the etiology of rheumatoid arthritis might be derived, while detailed knowledge of pathogenetic processes in rheu matoid arthritis could further research in chronic stages of pertinent animal diseases

Book excerpt: These two volumes, entitled "Purine Metabolism in Man IV" con tain the papers presented at the Fourth International Symposium on Human Purine and Pyrimidine Metabolism", held in Maastricht (The Netherlands), June 1982. The proceedings of the three previous meet ings in Tel Aviv (Israel, 1973), Baden (Austria, 1976) and Madrid (Spain, 1979) were also published by Plenum Press. In the past few years interest in purine and pyrimidine metabo lism under normal and pathological conditions has been growing rapid ly. Apart from the more or less classical topics such as hyperuricae mia, clinical gout and urolithiasis, an increasing number of papers relating to other fields have been presented at successive meetings. Knowledge derived from the study of purine metabolism in relation to lymphocyte function, for instance, has opened up new possibilities for immunomodulation and leukaemia chemotherapy, with eventual conse quences for other types of cancer. At previous meetings there have been pointers implicating purine metabolism in relation to normal cardiac and skeletal muscle function. During the present meeting mych new data on both issues have been re ported which indicate clear differences in the pathways of ATP metabo lism. The widening of the field of interest is also illustrated by the recent work on infectious disease: exploitation of the differences in purine metabolic pathways in certain parasites compared with those in human cells has resulted in new rationales for therapy being devel oped.

Book excerpt: It is widely recognized that "immunotherapy" was practiced at the end of the last century and the beginning of this century when it was recognized that microbial infections were caused by distinct species of bacteria and that passive administration of serum containing antibody to these microbes or their products could, in many cases, favorably influence the outcome of an infectious process. Furthermore, in the area of infectious disease it became quite apparent that "vaccines" prepared from killed microorganisms, or products thereof, could render an individual specifically resistant to that microorganism and, in many cases, increase in a nonspecific manner resistance to other organisms. This became quite evident with the advent of the use of attenuated mycobacteria for vaccination against tuberculosis.
