Potential immunologic and aesthetic drawbacks associated with titanium implants have resulted in the development of alternatives like zirconia-based dental implants. Zirconia seems to be a suitable implant material because of its tooth-like color, mechanical properties, biocompatibility, and low plaque affinity. However, the use of zirconia in clinical implant dentistry is still controversial. The aim of this chapter is to review clinical and research articles conducted on zirconia dental implants, and to provide information on zirconia dental implant osseointegration, mechanical strength, and microbiology. Compared to titanium-based dental implants zirconia implants show promising results in clinical studies. However, there are a limited number of long-term studies on the outcome of zirconia implants and additional clinical research needs to be done to fully appraise zirconia-based dental implants.

Available in PDF, EPUB and Kindle. Book excerpt: The demand for developing biomaterials with the optimum mechanical and corrosion properties for dental prostheses is increasing. Zirconia, a ceramic material, is getting more attention for its favorable physical, biological, and corrosion resistance attributes. Zirconia demonstrates polymorphism, and upon cooling, it exhibits martensitic transformation. This results in the uncontrolled expansion of the volume of about 5% and makes it suitable for clinical load bearing application. However, it can result in material catastrophic failure. By introducing a dopant into its crystal structure, this transformation can be stabilized, controlled, and the material becomes fracture-resistant under load-bearing situations. Ceria is isovalent with zirconia and does not create any vacancies upon substitution. In this study, 20 mol % Ceria Stabilized Zirconia (Ce-TZP) has been characterized upon sintering at three different temperatures, such as 1400°C, 1500°C, and 1600°C for 2 hours. X-ray Diffraction Pattern and Raman spectroscopy confirmed the tetragonal phase, and while analyzing the Scanning electron microscopy images, the increase in grain size is confirmed with sintering temperature. If the tetragonal phase can be confirmed with no change in the grain size distribution and surface roughness, Ce-TZP can be considered aging resistant, which is an important property to consider Ce-TZP as a prospective dental implant material.

Book excerpt: Veneering with porcelain may negatively affect the marginal discrepancy of long-span computer-aided design and computer-aidedmanufacturing (CAD-CAM) implant-supported fixed restorations. Information regarding the precision of fit of CAD-CAM-fabricatedimplant-supported complete zirconia fixed dental prostheses (FDPs) before and after porcelain layering is limited.The purpose of this study was to evaluate the effect of porcelain layering on the marginal fit of CAD-CAM-fabricated complete-archimplant-supported screw-retained FDP with presintered zirconia framework comparing to titanium.An acrylic resin fixed complete denture prototype was fabricated on an edentulous training typodont model with 2 straight and 2distally tilted dental implants with multi-unit abutments. A 3-dimensional (3D) laser scanner was used to digitize the resin prototypeand the master model using scan bodies to generate a virtual 3D CAD framework. Five presintered zirconia and 5 solid titaniumframeworks were fabricated using the CAM milling unit. One-screw test was applied and an industrial CT scanner was used to scanthe framework-model complex to check the passive-fit of the frameworks on the master model. 3D virtual assessment of the marginaldiscrepancy was performed at the abutment-framework interface at maxillary left canine (Gap 2), right canine (Gap 3) and right firstmolar (Gap 4) abutments without prosthetic screws. Facial/buccal aspects of the teeth on frameworks were veneered with layeringporcelain and CT scanned again using the same protocol. Marginal discrepancy measurements were made for 4 groups; titanium (Ti)(control), porcelain-veneered titanium (Ti-P) (control), zirconia (Zr), and porcelain-veneered zirconia (Zr-P) framework groups. 3Ddiscrepancy mean values were computed and calculated, and the results were analyzed with a repeated measures 3-way ANOVAusing the maximum likelihood estimation method and Bonferroni adjustments to selected pairwise comparison t-tests (u03b1=.05).The 3D discrepancy was measurable at Gap 3 and Gap 4.
