Book excerpt: Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation's Health was released in September 2019, before the World Health Organization declared COVID-19 a global pandemic in March 2020. Improving social conditions remains critical to improving health outcomes, and integrating social care into health care delivery is more relevant than ever in the context of the pandemic and increased strains placed on the U.S. health care system. The report and its related products ultimately aim to help improve health and health equity, during COVID-19 and beyond. The consistent and compelling evidence on how social determinants shape health has led to a growing recognition throughout the health care sector that improving health and health equity is likely to depend â€" at least in part â€" on mitigating adverse social determinants. This recognition has been bolstered by a shift in the health care sector towards value-based payment, which incentivizes improved health outcomes for persons and populations rather than service delivery alone. The combined result of these changes has been a growing emphasis on health care systems addressing patients' social risk factors and social needs with the aim of improving health outcomes. This may involve health care systems linking individual patients with government and community social services, but important questions need to be answered about when and how health care systems should integrate social care into their practices and what kinds of infrastructure are required to facilitate such activities. Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation's Health examines the potential for integrating services addressing social needs and the social determinants of health into the delivery of health care to achieve better health outcomes. This report assesses approaches to social care integration currently being taken by health care providers and systems, and new or emerging approaches and opportunities; current roles in such integration by different disciplines and organizations, and new or emerging roles and types of providers; and current and emerging efforts to design health care systems to improve the nation's health and reduce health inequities.

None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. The questions the volume seeks to answer are: o How are public health services in Europe organized? Are there good practices that can be emulated? What policy options are available? o How much is spent on public health services? Where do resources come from? And what was the impact of the economic crisis? o What do we know about the public health workforce? How can it be strengthened? o How is the quality of public health services being assured? To establish high-quality and affordable transport systems, cities must ensure their financial sustainability to fund new investments in infrastructure while also funding maintenance and operation of existing facilities and services. However, many cities in developing countries are stuck in an "underfunding trap" for urban transport, in which large up-front investments are needed for new transport infrastructure that will improve the still small-scale, and perhaps, poor-quality systems, but revenue is insufficient to cover maintenance and operation expenses, let alone new investment projects. The urban transport financing gap in these cities is further widened by the implicit subsidies for the use of private cars, which represent a minority of trips but contribute huge costs in terms of congestion, sprawl, accidents, and pollution. Using an analytical framework based on the concept of "Who Benefits Pays," 24 types of financing instruments are assessed in terms of their social, economic and environmental impacts and their ability to fund urban transport capital investments, operational expenses, and maintenance. Urban transport financing needs to be based on an appropriate mix of complementary financing instruments. In particular for capital investments, a combination of grants †“from multiple levels of government†“ and loans together with investments through public private partnerships could finance large projects that benefit society. Moreover, the property tax emerges as a key financing instrument for capital, operation, and maintenance expenses. By choosing the most appropriate mix of financing instruments and focusing on wise investments, cities can design comprehensive financing for all types of urban transport projects, using multi-level innovative revenue sources that promote efficient pricing schemes, increase overall revenue, strengthen sustainable transport, and cover capital investments, operation, and maintenance for all parts of a public transport system, "from the sidewalk to the subway."

In Piety and Public Funding historian Axel R. Schäfer offers a compelling answer to this question by chronicling how, in the first half century since World War II, conservative evangelical groups became increasingly adept at accommodating their hostility to the state with federal support. Though holding to the ideals of church-state separation, evangelicals gradually took advantage of expanded public funding opportunities for religious foreign aid, health care, education, and social welfare. This was especially the case during the Cold War, when groups such as the National Association of Evangelicals were at the forefront of battling communism at home and abroad. It was evident, too, in the Sunbelt, where the military-industrial complex grew exponentially after World War II and where the postwar right would achieve its earliest success. Contrary to evangelicals' own claims, liberal public policies were a boon for, not a threat to, their own institutions and values. The welfare state, forged during the New Deal and renewed by the Great Society, hastened—not hindered—the ascendancy of a conservative political movement that would, in turn, use its resurgence as leverage against the very system that helped create it. By showing that the liberal state's dependence on private and nonprofit social services made it vulnerable to assaults from the right, Piety and Public Funding brings a much needed historical perspective to a hotly debated contemporary issue: the efforts of both Republican and Democratic administrations to channel federal money to "faith-based" organizations. It suggests a major reevaluation of the religious right, which grew to dominate evangelicalism by exploiting institutional ties to the state while simultaneously brandishing a message of free enterprise and moral awakening.

Catchphrases such as “metropolitan areas are the engines that pull the national economy” turn out to be fairly accurate. But the same advantages of metropolitan areas that draw investment also draw migrants who need jobs and housing, lead to demands for better infrastructure and social services, and result in increased congestion, environmental harm, and social problems. The challenges for metropolitan public finance are to capture a share of the economic growth to adequately finance new and growing expenditures and to organize governance so that services can be delivered in a cost-effective way, giving the local population a voice in fiscal decision making. At the same time, care must be taken to avoid overregulation and overtaxation, which will hamper the now quite mobile economic engine of private investment and entrepreneurial initiative. Metropolitan planning has become a reality in most large urban areas, even though the planning agencies are often ineffective in moving things forward and in linking their plans with the fiscal and financial realities of metropolitan government. A growing number of success stories in metropolitan finance and management, together with accumulated experience and proper efforts and support, could be extended to a broader array of forward-looking programs to address the growing public service needs of metropolitan-area populations. Nevertheless, sweeping metropolitan-area fiscal reforms have been few and far between; the urban policy reform agenda is still a long one; and there is a reasonable prospect that closing the gaps between what we know how to do and what is actually being done will continue to be difficult and slow. This book identifies the most important issues in metropolitan governance and finance in developing countries, describes the practice, explores the gap between practice and what theory suggests should be done, and lays out the reform paths that might be considered. Part of the solution will rest in rethinking expenditure assignments and instruments of finance. The “right” approach also will depend on the flexibility of political leaders to relinquish some control in order to find a better solution to the metropolitan finance problem.

Book excerpt: Historically, governments have played the predominant role in owning and operating infrastructure facilities such as schools, hospitals, roads, bridges, railways, ports, telecommunications networks, and water and electricity supply facilities. However, fiscal policy constraints, growing acceptance of the user-pays principle, and a recognition that there are generally greater incentives for efficiency in the private sector, have driven increased private involvement in the provision of both economic and social infrastructure. A new Commission Staff Working Paper reports on the experiences of a number of countries using different approaches to funding public infrastructure projects. The countries covered in the study are Australia, Canada, France, Germany, New Zealand, Sweden, the United Kingdom and the United States. In most countries, general government investment in infrastructure has declined in recent years. Nevertheless, overall investment in infrastructure has remained fairly steady, although volatile in some countries. Total Australian investment in infrastructure was just below 6 per cent of GDP in 2006-07. Sub-national governments undertook 76 per cent of public infrastructure investment, with government trading enterprises (GTEs) accounting for around half of this.
