The feeling of weight and inconvenience caused by these tumors is sufficient in some cases to call for their removal, and the sufferer has a right to demand operation, even if life is not threatened, for surgery is not only to save life, but to contribute to its comfort; and there is no class of cases in which the decision of the patient has more weight than in these cases of fibroid tumors. I do not want to carry this thing any longer; it annoys me. I can not stoop to button my boots, andi want it re moved, said a woman to me. I had put her off several months, and she had abundant time to think the matter over. I thought she had a right to decide the question of operation. I removed the tumor, and she got well. With the possibilities revealed by this great advance, for fifteen years, the surgeons of civilization have been struggling toward perfection. In a little more than a decade wonders have been accomplished and surgery developed to a science. During this period of unprecedented activity the treatment of fibroids of the uterus has received a large share of attention and may now be said to have reached as high state of perfection as any other branch of surgery. The benign nature of these tumors, their slow development, their small mortality without treatment made patients reluctant to accept the radical operations. Hence minor medical and surgical means were sought and persistently cultivated to the limits of their possibilities, Ergot, electricity, the Battey-Tait operation and ligation of the broad ligaments are minor treatments which have relieved much suffering, cured not a few patients, and saved many from the more dangerous procedure of hysterectomy. In the development of these minor means, not only, but in the cultivation and improvement of the major surgical technique the writer has devoted a greater portion of his professional time for the last ten years. The object of these lectures is to place in permanent shape the outcome of this decade of work and to place in the hands of his friends and students a mirror, as it appears to him, of the present status of the treatment of fibroids of the uterus. Book excerpt: Excerpt from The Treatment of Fibroid Tumors of the Uterus In the first case I removed-eleven successive tumors, after which the patient recovered and had no return. On December 27, 1860, I visited Mrs. T. W. S., aged 31 years. Menstrua tion for the first four years was small in quantity and free from pain, but after wards was accompanied with extreme agony during the first day. She married at the age of 24 years, in one year after gave birth to a child, and had a mis carriage afterwards. The menses were regular, having only missed the last period. In April, 1858, She first discovered something wrong in not being able to pass water. A large quantity was drawn away by the catheter, which opera tion had to be repeated. Several physicians saw her, and diagnosticated mal position of the womb, and very violent efforts were made to restore it to its normal position, but without effect. June, 1860, she had a sudden attack of severe pain, which so entirely disabled her that she could not walk for several weeks, suffering much of the time with the most dreadful bearing-down pains. After a period of intermission the expulsive pains again returned with extreme violence, and, on November 28, 1860, while using the catheter, she felt a tumor in the vagina which she supposed to be the womb. The pain now dimin iehed, but the Size Of the tumor gradually increased, and it finally protruded through the os externum. During the above period she had copious discharges from the vagina of bloody clots and a greenish, slimy, and offensive fluid, changing usually about twenty-five napkins in a day.
