Book excerpt: A study of safety reports was conducted to examine the hypothesis that fatigue and associated performance decrements occur in air transport operations, and that these are associated with some combination of factors: circadian desynchronosis, duty time; pre-duty activity; sleep; work scheduling; workload; and environmental deprivation. The findings are based on a selected sample of reported incidents in which the reporter associated fatigue with the occurrence. In comparing the fatigue reports with a control set, significant performance decrements were found to exist related to time-of-day, awareness and attention to duty, less significantly, final phases of flights. The majority of the fatigue incidents involved such unsafe events as altitude deviations, takeoffs and landing without clearance, and the like.
