Book excerpt: Much evidence exists that nurses are leaving the public health sector for the private sector, or leaving the country to seek better working conditions and higher salaries. Studies conducted on the job satisfaction of nurses are proof that there is a need to know more about the factors that influence their sense of job satisfaction. Most of these studies focus on the general nursing context. Due to its unique circumstances, many studies abroad have identified the field of psychiatric mental health nursing to investigate job satisfaction of nurses. The minithesis is an attempt to fill the gap that exists in job satisfaction studies in South Africa of nurses in a provincial psychiatric hospital. The study was a cross sectional, correlational, survey design study. The instrument was a self-administered questionnaire, combining a quantitative questionnaire with one qualitative open-ended question. The study was conducted on nurses of all categories in a provincial psychiatric hospital in the Western Cape. Sixty- eight nurses participated in the study. The data was analyzed statistically using the SAS v9 statistical software and Statistical Package for Social Sciences (SPSS). The open-ended question was analyzed qualitatively. The results revealed that the participating nurses were dissatisfied with remuneration, recognition and appreciation, training and development, as well as benefits and incentives. Nurses were most satisfied with supervision and support, interpersonal relationships, and rendering patient care. The study identified the factors influencing job satisfaction and job dissatisfaction of nurses in a provincial psychiatric hospital. Recommendations were made based on the results of the research.

Book excerpt: Health planning is a critical component when responding to the health needs of low and middle income countries, characterised by particularly stringent resource constraints. The major communicable diseases such as AIDS, TB and malaria often appear in parallel with growing non-communicable diseases including heart disease and diabetes, and yet resources are often less than the levels recommended by the World Health Organisation for basic health care. The new edition of this well-respected text explains the importance of health planning in both developing regions such as Africa, and those in transition, such as Central and Eastern Europe. It stresses the importance of understanding the national and international context in which planning occurs, and provides an up to date analysis of the major current policy issues, including health reforms. Separate chapters are dedicated to the distinct issues of finance for health care and human resource planning. The various techniques used at each stage of the planning process are explained, starting with the situational analysis and then looking in turn at priority-setting, option appraisal, programming, implementation, monitoring, and evaluation. The book ends by examining the challenges facing planners in the 21st century, particularly in the light of growing globalisation. A major theme of the book is the need to recognise and reconcile the inevitable tension that lies between value judgements and 'rational' decision-making. As such, in addition to introducing techniques such as costing and economic appraisal, it also outlines techniques such as stakeholder analysis for understanding the relative attitudes and power of different groups in planning decisions. Each chapter includes a comprehensive bibliography (including key websites), a summary, and exercises to help the reader practise techniques and better understand the content. The book argues that all health professionals and community groups should be involved in the planning process for it to be effective, and will therefore appeal to anyone involved in planning.

Their motivation and attitudes have significant ramifications for patient health outcomes. In low- and middle-income countries, low performance and suboptimal behaviors of health workers have often been reported as contributing factors leading to adverse health outcomes. One possible solution to reduce suboptimal health worker practices is to increase health worker job satisfaction. Using secondary data from a performance-based financing project in health facilities, this dissertation examines the concept of work satisfaction among health workers in Bénin. First, I validate the instrument used — The Minnesota Satisfaction Questionnaire. I then use it to examine the level of satisfaction among midwives, nurses, and auxiliary nurses. I find that, on average, health workers are moderately satisfied. I then investigate the determinants of work satisfaction at both the individual and the health facility level, using a multilevel analysis to take advantage of the nested structure of my data. My findings indicate that tenure, gender, training opportunities, working conditions, and style of management are all factors that play a role in explaining work satisfaction. Finally, to understand how different kinds of health workers relate to their work environments, I conducted semi-structured interviews on a selected sample of health professionals. The qualitative evaluation of these interviews shows that these health workers perceive their work environments to be an important contributing factor to their satisfaction. This has potentially important implications on the level of the quality of care health workers can provide to their patients. As a whole, the results of my research provide a better understanding of work satisfaction among health workers in Bénin, and identify future research questions on a subject overlooked in low- and middle-income sub-Saharan African countries.
