Book excerpt: Objectif : le but de cette étude épidémiologique transversale multicentrique était de déterminer les facteurs prédictifs de déséquilibre glycémique chez les enfants atteints de diabète type 1 (DT1), en s'intéressant particulièrement au statut socioéconomique. Méthodes : les enfants atteints de DT1, diagnostiqués depuis plus d'un an, suivis par des pédiatres spécialisés en diabétologie dans 7 centres hospitaliers d'Occitanie en France, ont été inclus du 2 novembre 2017 au 3 mai 2018. Les données cliniques et démographiques ont été obtenues rétrospectivement à partir des dossiers des patients. Le statut socio-économique (SSE) était évalué par un score de défavorisation individuelle, le score EPICES (Evaluation de la privation et des inégalités de santé dans les centres de santé), et un score de défavorisation géographique, l'EDI (European deprivation index, indice européen de défavorisation). Pour étudier l'association entre les scores de défavorisation et l'HbA1c, nous avons utilisé un modèle multivarié de régression linéaire, ajusté sur le sexe, l'âge, la durée de diabète, l'IMC et le schéma de traitement insulinique. Résultats : au total, 33% (n=376) des patients avaient un score EPICES =30 et 23% (n=268) se situaient dans le cinquième quintile de l'EDI. Nous avons recherche les facteurs ayant le plus d'impact sur l'equilibre glycemique (HbA1c) parmi les modalites de la surveillance glycemique, du traitement, ainsi que le vecu de la maladie dans une population d'enfants. Cette etude longitudinale et retrospective porte sur 45 enfants diabetiques de type 1 pre-puberes avec une bonne adhesion au suivi et au traitement, pris en charge en pediatrie au CHRU de Lille et au CH de Cambrai. Dans notre population de patients au traitement correctement administre, l'intensification du schema d'insuline n'etait pas correlee a une meilleure HbA1c. Le degre d'aptitude du patient et de son entourage a adapter les doses d'insuline quotidiennes constitue un facteur essentiel d'equilibre glycemique. Des etudes prospectives, randomisees et a plus grande echelle apparaissent ainsi necessaires pour determiner la surveillance glycemique optimale."

Book excerpt: Objectif : mettre en évidence les facteurs prédictifs du contrôle de l'équilibre glycémique au cours de la première année post découverte du diabète de type 1 (DT1) chez l'enfant. Méthodes : étude monocentrique rétrospective incluant toutes les découvertes de DT1 du service de diabétologie pédiatrique de Toulouse découvert entre juin 2019 et juin 2020 et suivies durant la première année. La variable d'intérêt principale était la moyenne de l'HbA1c sur le suivi. Les variables clinico-biologiques, sociales, la prise en charge et le suivi étaient évaluées pour déterminer celles associées au contrôle glycémique à la fin de la 1er année. Résultats : 104 patients pédiatrique DT1 ont été inclus avec une HbA1c moyenne sur le suivi de 7,35±0,74. En analyses uni puis multivariées le temps dans la cible = 60% au 2ème suivi était significativement associé à un meilleur contrôle glycémique à 1 an. Conclusion : notre étude retrouve qu'un temps dans la cible non à l'objectif au 2ème suivi est un facteur prédictif de mauvais équilibre glycémique. Ce déséquilibre rapide permettrait de proposer un suivi rapproché et une intensification de la prise en charge plus précoce. Notre étude ne retrouve pas d'autres facteurs prédictifs en analyse multivariée.
