Yet these women are least likely to avail themselves of screening and early detection testing which has been previously demonstrated to be associated with prolongation of survival for women subsequently diagnosed with breast cancer. The goal of this project is to increase screening and early detection practices in low income women, forty years and older, who are enrolled in a statewide HMO. It compares the relative effectiveness of two interventions (a simple one using a letter of invitation and a more intensive "step-wise" intervention of two sequential letters and follow-up counseling and home visits). Both interventions are compared with a control group of women who continue to receive their "usual care". This report gives an account of program planning and process evaluation. The results of the baseline survey across groups are presented. A most significant finding was the difficulty of locating and contacting subjects who were literally "hard-to-reach". Detailed data are provided illustrating the scope of this problem. Baseline survey data analysis is in process and intervention among groups is being initiated.

Book excerpt: The economic burden of breast cancer for women under 50 in the United States remains largely unexplored, in part because young women make up a small proportion of breast cancer cases overall. To address this knowledge gap, we conducted a web-based survey to compare data from breast cancer survivors 18–39 years of age at first diagnosis and 40–49 years of age at first diagnosis.We administered a survey to a national convenience sample of 416 women who were 18–49 years of age at the time of their breast cancer diagnosis. We analyzed factors associated with financial decline using multivariate regression.Survivors 18–39 years of age at first diagnosis were more likely to report Stage II–IV breast cancer (P < 0.01). They also quit their jobs more often (14.6%) than older survivors (4.4%; P < 0.01) and faced more job performance issues (55.7% and 42.8%, respectively; P = 0.02). For respondents in both groups, financial decline was more likely if the survivor had at least one comorbid condition (odds ratios: 2.36–3.21) or was diagnosed at Stage II–IV breast cancer (odds ratios: 2.04–3.51).

Book excerpt: Breast cancer is the most common cancer diagnosed among all women in the US and the second leading cause of cancer death (DeSantis, Ma, Gaudet Newman, et al, 2019). Black women in the US are less likely to survive breast cancer with death rates 40% higher than White women (DeSantis, Ma, Gaudet Newman, et al, 2019; DeSantis, Fedewa, Goding Sauer et al, 2016). Black women younger than age 40 have the lowest survival rate of all groups, representing an ongoing and significant health disparity (Sheppard, Llanos et al, 2013).Background/Objective: To explore the experience of Black breast cancer patients.Methods: 25 African American breast cancer survivors ages 41-79 participated in a qualitative study. Age at diagnosis ranged from 31-70 years, with mean survival time since diagnosis ranging from 3 months to 24 years. Women participated in a semi-structured interview format. These studies were guided by the following primary research question: What are the risk and protective factors for African American women with breast cancer?Results: Interview data was analyzed using qualitative software NVIVO and coded for recurrent themes reported. African American cancer survivors drew on inner resources including spirituality, cognitive reframing, personal agency, and racial socialization. Coping resources in their environment included relational supports, medical resources, financial resources, and engaging in community health education and mentoring other survivors. African American cancer survivors reported individual and environmental risks in the categories of emotional distress, help-seeking barriers, medical mistrust and cultural silence about cancer. Younger women reported greater distress regarding relational and caregiving strain, negative impact on social network and functioning, and distress related to body image and fertility concerns.

International health organizations, bilateral aid agencies, and major foundations—which are instrumental in setting health priorities—also have largely ignored cancer in these countries. This book identifies feasible, affordable steps for LMCs and their international partners to begin to reduce the cancer burden for current and future generations. Stemming the growth of cigarette smoking tops the list to prevent cancer and all the other major chronic diseases. Other priorities include infant vaccination against the hepatitis B virus to prevent liver cancers and vaccination to prevent cervical cancer. Developing and increasing capacity for cancer screening and treatment of highly curable cancers (including most childhood malignancies) can be accomplished using "resource-level appropriateness" as a guide. And there are ways to make inexpensive oral morphine available to ease the pain of the many who will still die from cancer.

Today, it is not possible to deliver high-quality cancer care without using existing approaches, tools, and resources to address patients' psychosocial health needs. All patients with cancer and their families should expect and receive cancer care that ensures the provision of appropriate psychosocial health services. Cancer Care for the Whole Patient recommends actions that oncology providers, health policy makers, educators, health insurers, health planners, researchers and research sponsors, and consumer advocates should undertake to ensure that this standard is met.
