Book excerpt: This paper explores the perspectives of Latino focus group participants regarding colorectal cancer (CRC) screening and gender. In the United States (US), the Latino community screens for CRC much less than other groups. Latino men can screen at low rates while their CRC mortality rates are high. The present focus group participants corroborated findings in which Latino study participants perceive threats to masculinity to be a barrier for men in CRC screening. Using Grounded Theory to analyze the focus group transcripts, a theory emerges that explores the potential role that Latino men's female partners can have to encourage their screening. There exists evidence that Latino men can benefit from their partners' support to complete their CRC screening. In theory, a woman's encouragement is successful in this context because she can mitigate the perceived threats to masculinity. Further research should test the strategy of female partner support for Latino men's CRC screening.

Furthermore, without a physician recommendation for early screening, the patient's chances of early detection are substantially lowered. This is why it is imperative to examine the complex factors that contribute to minorities who have lower CRC screening rates compared to Caucasians.

Book excerpt: Introduction: Colorectal cancer (CRC) screening disparities continue to persist among Latinos despite increasing CRC screening rates within the past decade. To help address this disparity, a promotor-led intervention was implemented in community-based settings, connecting community members with a partnering Federally Qualified Health Center (FQHC) to access CRC screening. Purpose: The study examines screening completion at six-month follow-up and identifies socio-demographic variables that may impact adherence to CRC screening. Methods: The Juntos Contra el Cáncer/Together Against Cancer (JUNTOS) program enrolled 214 male and female Latinos between the ages of 50 to 75 years who have not been screened for CRC. The study was conducted in San Diego, California, and implemented a 2.5-hour promotor-led educational workshop describing CRC screening benefits and CRC screening options. Promotores provided instrumental (e.g. assist with appointment scheduling with partnering clinic) and social support (e.g. encouragement). Data collection occurred between December 2016 and March 2019. A total of 173 participants were included in the final analyses. Socio-demographic variables and barriers to CRC screening comparisons were made between those who completed a screening at six months (adherent) and those who did not complete a screening at six months (non-adherent) using descriptive statistics, Chi-squared, and logistic regression analyses. Results: Overall, out of the 173 total participants, 117 participants completed screening (67.6%) and 56 participants (32.4%) did not complete screening by six months. There were no significant group differences in any of the demographic variables (p > 0.05). Medicare/Medicaid enrollees were more likely to get screened than those with no health insurance (OR = 3.11, 95% CI [1.22, 7.96], p = 0.02). Significant associations (p

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: Author's abstract:Background: The findings from previous studies on the socio-demographic and socio-economic factors associated with non-adherence to Colorectal Cancer Screening (CRCS) in populations of health disparities are inconsistent, and few studies have utilized an integrative approach to deliver a CRCS intervention to at-risk adults. Methods: A cross-sectional study was conducted using data from the 2009-2010 Southeast Georgia Cancer Alliance Integrative project survey to describe the rates of negative patient deviation (non-adherence) to CRCS. Results: Almost 70% of at-risk adult participants were non-adherent to CRCS. Participants under 45 years of age were 1.8 times as likely to report a negative deviation compared to participants 45 years of age and older. Males were 1.7 times as likely to be non-adherent to CRCS compared to females. Obese participants were 7.8 times as likely to be non-adherent to CRCS compared to underweight, normal weight and overweight participants. A strong correlation existed between county of residence and negative deviation to CRCS. The results of the study support that age, gender, BMI and county of residence were significant factors that showed strong associations to non-adherence to CRCS. Conclusion: The findings suggest, understanding the relationships that exist between non-adherence to CRCS and factors that determine health outcomes are essential to reducing the mortality and morbidity of CRC.

The book examines how disparities in treatment may arise in health care systems and looks at aspects of the clinical encounter that may contribute to such disparities. Patients' and providers' attitudes, expectations, and behavior are analyzed. How to intervene? Unequal Treatment offers recommendations for improvements in medical care financing, allocation of care, availability of language translation, community-based care, and other arenas. The committee highlights the potential of cross-cultural education to improve provider-patient communication and offers a detailed look at how to integrate cross-cultural learning within the health professions. Differences in health by racial and ethnic status could be increasingly consequential for health policy and programs. Such differences are not simply a matter of education or ability to pay for health care. For instance, Asian Americans and Hispanics appear to be in better health, on a number of indicators, than White Americans, despite, on average, lower socioeconomic status. The reasons are complex, including possible roles for such factors as selective migration, risk behaviors, exposure to various stressors, patient attitudes, and geographic variation in health care. This volume, produced by a multidisciplinary panel, considers such possible explanations for racial and ethnic health differentials within an integrated framework. It provides a concise summary of available research and lays out a research agenda to address the many uncertainties in current knowledge. It recommends, for instance, looking at health differentials across the life course and deciphering the links between factors presumably producing differentials and biopsychosocial mechanisms that lead to impaired health.

Book excerpt: The Centers for Medicare & Medicaid Services (CMS) have been moving from volume-based, fee-for-service payment to value-based payment (VBP), which aims to improve health care quality, health outcomes, and patient care experiences, while also controlling costs. Since the passage of the Patient Protection and Affordable Care Act of 2010, CMS has implemented a variety of VBP strategies, including incentive programs and risk-based alternative payment models. Early evidence from these programs raised concerns about potential unintended consequences for health equity. Specifically, emerging evidence suggests that providers disproportionately serving patients with social risk factors for poor health outcomes (e.g., individuals with low socioeconomic position, racial and ethnic minorities, gender and sexual minorities, socially isolated persons, and individuals residing in disadvantaged neighborhoods) may be more likely to fare poorly on quality rankings and to receive financial penalties, and less likely to receive financial rewards. The drivers of these disparities are poorly understood, and differences in interpretation have led to divergent concerns about the potential effect of VBP on health equity. Some suggest that underlying differences in patient characteristics that are out of the control of providers lead to differences in health outcomes. At the same time, others are concerned that differences in outcomes between providers serving socially at-risk populations and providers serving the general population reflect disparities in the provision of health care. Systems Practices for the Care of Socially At-Risk Populations seeks to better distinguish the drivers of variations in performance among providers disproportionately serving socially at-risk populations and identifies methods to account for social risk factors in Medicare payment programs. This report identifies best practices of high-performing hospitals, health plans, and other providers that serve disproportionately higher shares of socioeconomically disadvantaged populations and compares those best practices of low-performing providers serving similar patient populations. It is the second in a series of five brief reports that aim to inform the Office of the Assistant Secretary of Planning and Evaluation (ASPE) analyses that account for social risk factors in Medicare payment programs mandated through the Improving Medicare Post-Acute Care Transformation (IMPACT) Act.

Few, however, realize that these qualities--when taken to the extreme--can cause emotional constriction, substance abuse, depression, aggression, and violence in many men. Further, even though most men are not violent, decades of research has shown that masculinity is distinctly related to sexual and gun violence and men's poorer health. Considering how girls and women have benefitted from decades of conversations on navigation of their gender in a changing world, similar processes are urgently needed for boys and men.
