Book excerpt: Antimicrobial resistance (AMR) is the ability of microorganisms to fight antimicrobial compounds, reducing the efficacy of treating diseases in humans, animals, and plants. AMR risk is outpacing human population growth, owing to misuse of antimicrobials in large quantities in food systems, and is a serious threat to food security and sustainable development. FAO, with the World Health Organization (WHO), the World Organization for Animal Health (OIE), and the United Nations Environment Programme (UNEP), is supporting countries in developing and implementing their One Health National Action Plans on AMR. The eventual aim is to ensure sustainable use of antimicrobials to minimize AMR risks, in alignment with the Global Action Plan on AMR. The scope of the evaluation covers FAO’s entire work on AMR up to early 2020 and its role in the global AMR architecture. It examines FAO’s organizational and institutional set-up for AMR work. FAO has a strong mandate to work on AMR, implementing activities in 45 countries and providing far-reaching support on AMR National Action Plans (NAPs). FAO’s technical expertise is a key comparative advantage in its work on AMR. It is underpinned by the strong scientific grounding of FAO’s work, engendered in its AMR working groups and supported by its collaboration with research centers, universities, and the Tripartite organizations. Nevertheless, the work is relatively recent and, given the long impact pathways, it has had limited results. A comprehensive strategic and programmatic approach would increase the likelihood of achieving results in combating AMR. FAO should prioritize its work in a long-term strategy on AMR that recognizes the seriousness of the threat and is fully integrated into the Organization’s Strategic Framework. The strategy should set out FAO’s long-term role in combating AMR and that of its divisions and offices, as well as its approach at the country and regional level. FAO should consolidate its work on AMR through a strong programmatic approach with a central coordination and management structure that links with the Regional Offices and is supported by dedicated core funding.

Drug-resistant infections have the potential to become leading causes of death. AMR may force tens of millions more people into extreme poverty, hunger, and malnutrition, and the associated economic losses are projected at several percent of gross domestic product. However, we can prevent this from happening – if we act quickly. This document outlines the FAO Action Plan on Antimicrobial Resistance 2021–2025 which serves as a roadmap for focusing global efforts to address AMR in the food and agriculture sectors. The aim of this plan is to help accelerate progress in developing and implementing multi-sectoral National Action Plans to tackle AMR by calling attention to strategic priorities and areas of expertise for FAO support. The action plan was developed by a multidisciplinary FAO team to ensure that all relevant dimensions – including terrestrial and aquatic animal health and production, crop production, food and feed safety, genetic resources, natural resource management, risk communication, and behavior change - are considered, with attention to regulatory frameworks, standards, norm-setting and bottom-up processes of collective action. By working together, food systems, livelihoods, and economies will be better protected from the destabilizing forces of untreatable illness.

Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. Book excerpt: In 2015 the World Health Assembly endorsed the Global action plan on antimicrobial resistance (GAP-AMR) calling on countries to develop, implement and monitor national action plans on AMR. In 2019, FAO, WHO and WOAH developed a monitoring and evaluation (M&E) framework for the GAP-AMRto track progress towards the plan’s five global objectives, designed with a One Health perspective and reflecting the cross-sectoral nature of AMR with indicators across human and animal health, plant and food production and the environment. Countries are expected to develop an M&E plan for their NAP, tailored to their context and priorities. This includes developing indicators appropriate to the country’s own circumstances, aligned with the proposed core indicators of the GAP-AMR M&E framework, as far as possible. This country M&E guidance document was developed as a reference for countries to support the development and delivery of AMR NAPs. It provides assistance on how to establish an M&E plan for their AMR NAP, building on existing national reporting systems and recommended indicators from the GAP-AMR M&E framework.

More than ever before, we know that addressing the interlinked and multi-faceted challenges posed by antimicrobial resistance requires that we work together – across sectors, government, academic disciplines, civil society, the private sector, and the multilateral system – to advance a One Health approach. This strategic framework on antimicrobial resistance represents an important milestone in the decades-long history of collaboration between the Tripartite organizations - a collaboration that now is even stronger as a result of our close engagement with the United Nations Environment Programme. The framework sets out for the first time what our organizations – as leaders in the multilateral system on the human, animal, plant, and environmental health – will do jointly to support countries’ efforts to scale up national responses to antimicrobial resistance. Countries and partners are strongly encouraged to replicate and amplify the One Health approach used in the framework, based on their own contexts and needs.

Media were therefore identified as one of four priorities during two global consultations for raising awareness about Antimicrobial Resistance (AMR), organized in 2022 by the Quadripartite organizations, the Food and Agriculture Organization of the United Nations (FAO), the United Nations Environment Programme (UNEP), the World Health Organization (WHO) and the World Organisation for Animal Health (WOAH). The priorities for developing a common approach to awareness-raising, including for the media, are to engage target audiences in a participatory approach; emphasize AMR and ensure that the messages are simple, consistent, and clear; agree on common definitions, language, and terms for AMR in all sectors; and ensure coordination of awareness among sectors. In response, the Quadripartite has developed this practical toolkit for engaging media in AMR. The aim is to equip individuals, organizations and networks with tools for media engagement on AMR at subnational, national, regional and global levels to increase public awareness of AMR, including policy-makers, civil society and communities.

Some human populations escaped tropical infections by migrating into cold climates but then had to procure fuel, warm clothing, durable housing, and crops from a short growing season. Waterborne infections were averted by owning your own well or supporting a community reservoir. Everyone got vaccines in rich countries, while people in others got them later if at all. Antimicrobial agents seemed at first to be an exception. They did not need to be delivered through a cold chain and to everyone, as vaccines did. They had to be given only to infected patients and often then as relatively cheap injectables or pills off a shelf for only a few days to get astonishing cures. Antimicrobials not only were better than most other innovations but also reached more of the world’s people sooner. The problem appeared later. After each new antimicrobial became widely used, genes expressing resistance to it began to emerge and spread through bacterial populations. Patients infected with bacteria expressing such resistance genes then failed treatment and remained infected or died. Growing resistance to antimicrobial agents began to take away more and more of the cures that the agents had brought.

Increasing temperatures, ocean warming and acidification, severe droughts, wildfires, altered precipitation patterns, melting glaciers, rising sea levels and amplification of extreme weather events have direct implications for our food systems. While the impacts of such environmental factors on food security are well known, the effects on food safety receive less attention. The purpose of Climate change: Unpacking the burden on food safety is to identify and attempt to quantify some current and anticipated food safety issues that are associated with climate change. The food safety hazards considered in the publication are foodborne pathogens and parasites, harmful algal blooms, pesticides, mycotoxins and heavy metals with emphasis on methylmercury. There is also, a dedicated section on the benefits of forward-looking approaches such as horizon scanning and foresight, which will not only aid in anticipating future challenges in a shifting global food safety landscape, but also help build resilient food systems that can be continually updated as more knowledge is assimilated. By building a more widespread and better understanding of the consequences climate change has on food safety, it is hoped that this document will aid in fostering stronger international cooperation in making our food safer by reducing the global burden of these concerns.
