Book excerpt: L’objectif de cette thèse était d’évaluer l’efficacité d’un programme de rééducation cognitive hiérarchisée de la mémoire de travail chez des patients souffrant de lésions cérébrales vasculaires ou traumatiques. Nous avons utilisé la méthodologie du cas unique avec lignes de bases multiples. Nous avons tout d’abord pu montrer l’efficacité et la spécificité de cette thérapie chez deux patients vasculaires présentant des séquelles d’aphasie. Dans les deux cas, nous avons constaté une généralisation dans la vie quotidienne. Dans l’un des deux cas, nous avons montré une spécificité composante-spécifique en alternant rééducation de la boucle phonologique et du calepin visuo-spatial. Puis nous nous sommes intéressés aux déficits de mémoire de travail après un traumatisme crânien sévère, par une étude de groupe chez 30 patients qui a mis en évidence un déficit des fonctions de l’administrateur central, contrastant avec une préservation des systèmes esclaves. Nous avons ensuite montré l’efficacité et la spécificité de notre programme de rééducation chez deux patients traumatisés crâniens sévères en phase chronique présentant des déficits isolés de la mémoire de travail. La dernière partie de notre travail était la validation d’un questionnaire visant à évaluer les plaintes liées à un déficit de mémoire de travail dans la vie quotidienne. Ce questionnaire, comprenant 30 questions (stockage à court terme, attention, aspects exécutifs) a été normalisé chez 313 sujets sains et validé chez un groupe de 69 patients cérébrolésés. La validité interne et concurrente était satisfaisante, et le questionnaire s’est avéré sensible aux effets d’une lésion cérébrale traumatique ou vasculaire

Notre objectif principal est de ractériser les effets d'une rééducation cognitive de la mémoire de travail (MDT) après lumatisme crânien (TCC), en validant (hl) une amélioration spécifique de la MDT avec aintien et (h2) une généralisation des effets aux processus de MDT. Nous observons aussi 1 transfert éventuel en vie quotidienne (h3). Une hypothèse secondaire concerne les lodifications fonctionnelles en imagerie. L'analyse des résultats confirme hl et h2. H3 est lus subtile à démontrer mais les résultats sont prometteurs. L'imagerie met en évidence une ctivation accrue du réseau d'intérêt. L'intégration de nouveaux patients permettra de ^produire ces travaux, d'en vérifier les liens ainsi que leur robustesse et niveau de preuve.

Book excerpt: Les gliomes de bas grade et les différentes thérapeutiques qu'impose leur traitement sont source de troubles cognitifs divers, que seules des investigations orthophoniques et neuropsychologiques poussées permettent d'objectiver. La mémoire de travail, notamment, est une fonction dont le déficit est souvent décrit chez les patients opérés en éveillé de gliomes infiltrants. Partant du constat que les pratiques d'évaluation et de rééducation sont très hétérogènes, nous avons décidé de tester des épreuves d'évaluation de la mémoire de travail et un matériel de rééducation cognitive spécifique. Les outils d'évaluation choisis, en référence au modèle de Baddeley, ont été testés sur 10 patients dont les résultats ont été comparés à une population appariée de 19 sujets-contrôles. L'efficacité de ce type de rééducation pour cette population a fait l'objet d'une étude préliminaire de petit groupe (6 patients) en méthodologie à deux bras en simple aveugle. Les résultats indiquent que les outils d'évaluation objectivent bien des troubles de la mémoire de travail, et que certaines épreuves sont sensibles à cette population. Leur prise en charge est primordiale pour permettre aux patients une réinsertion socio¬professionnelle. Peu d'études ont été publiées sur le sujet et les patients montrent une réelle envie de retrouver un cadre de vie « normal ».Notre étude porte sur un programme de rééducation de la mémoire de travail, sur son efficacité et sa spécificité. Nous l'avons testé auprès de deux patients cérébro-lésés à l'aide d'une méthodologie rigoureuse en ligne de base multiple selon les comportements. Nous avons montré l'efficacité de ce programme ainsi que sa spécificité puisque les résultats aux épreuves non-cibles n'ont pas progressé. Par ailleurs, un transfert en vie quotidienne a été observé lors des questionnaires. Des évaluations post-thérapeutiques à trois mois après l'arrêt de la rééducation restent à faire pour montrer la stabilité des résultats.Il reste à appliquer ce programme auprès d'une population plus large de patients cérébro-lésés afin de confirmer la significativité statistique de son efficacité.
