Book excerpt: Introduction : une unité de médecine aigue dédiée aux patients de post-urgences a ouvert à l'IHU, mêlant infectiologues et urgentistes qui veillent à l'application des protocoles de prise en charge standardisée des principaux syndromes infectieux. L'objectif principal de cette étude est de surveiller le respect des protocoles ainsi que le retrait des cathéters veineux périphériques ou urinaires inutiles dans cette unité. L'objectif secondaire est d'évaluer la suite de prise en charge des patients après leur sortie de l'hôpital en établissant un lien avec le médecin traitant. Méthodes : il s'agit d'une étude monocentrique, observationnelle, analytique et prospective, réalisée dans le service de médecine aigue post-urgences de l'IHU de Janvier à Juillet 2017. Résultats : 614 patients ont été inclus dont 414 patients atteints de maladies infectieuses et 200 patients atteints de troubles neurologiques ou gastroentérologiques non infectieux. Parmi les patients de maladies infectieuses, 65% présentaient un syndrome entrant dans un protocole. Les protocoles étaient plus respectés dans le service qu'aux urgences (74% vs 62%, p=0,001). A l'entrée, 83% des patients avaient un cathéter sanguin dont 51% inutiles, et 10% des patients avec une sonde urinaire dont 32% non justifiées. A la sortie d'hospitalisation, le médecin traitant recevait la lettre de liaison dans 84% des cas. Une réévaluation du traitement était nécessaire dans 13% des cas et 10% des patients ont eu besoin d'une nouvelle hospitalisation au décours. Discussion : grâce à une prise en charge standardisée, notamment en établissant une filière courte pour que les patients des urgences soient hospitalisés en médecine aiguë post-urgences, en appliquant les protocoles thérapeutiques, en chassant systématiquement les cathéters inutiles, et créant un lien avec le médecin traitant à la sortie d'hospitalisation, nous pensons pouvoir améliorer la prise en charge des patients durant leur parcours de soins à l'hôpital.

Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.
