Book excerpt: Selon les promoteurs du système de fluorescence laser Diagnodent, cet appareil mesure la fluorescence qui se crée lorsque la lumière laser frappe des zones de déminéralisation ; il s'agit d'un appareil non invasif, simple à utiliser, qui fournit des données quantitatives. Une revue systématique des études corroborant son efficacité semble indiquer une bonne sensibilité et une excellence reproductibilité. Une nouvelle version, le Diagnodent Pen, a été évalué à travers une étude in vitro et une étude in vivo. Il en ressort que ses performances in vitro sont très intéressantes au niveau des surfaces occlusales avec une spécificité comparable à celle de l'examen clinique et une sensibilité plus élevé. Les résultats in vivo sont plus mitigés au niveau des surfaces occlusales et proximales. Cependant, d'autres études in vivo restent nécessaires pour confirmer les résultats obtenus par le Diagnodent Pen. Il semble offrir des renseignements supplémentaires mais il ne peut pas, à lui tou seul, remplacer l'examen visuel pour le diagnostic de la carie.

La recherche et le développement des différentes stratégies possibles de prévention primaire de la maladie carieuse en odontologie pédiatrique, doit définir le risque carieux individuel et spécifier les différents moyens mécaniques et chimiques disponibles en France, ainsi que leurs efficacités, sans oublier les conseils d’hygiène alimentaire et bucco-dentaire. Le développement et la synthèse des différentes recommandations a permis de réaliser des tableaux récapitulatifs par tranche d’âge et en fonction du risque carieux individuel du patient. Les conseils d’hygiène alimentaire et bucco-dentaire et les moyens thérapeutiques mécanique et chimique, fluoré et non fluoré, y sont ainsi proposés, permettant une prise en charge adaptée et individualisée de chaque patient. Dans le second tome de cette thèse, la création d'un logiciel destiné au praticien reprend les tableaux synthétiques développés dans le tome 1, afin de guider le chirurgien-dentiste dans ses décisions concernant la mise en place d'une stratégie de prévention.

Or, les répercussions des caries - immédiates et à long terme - sont nombreuses et potentiellement invalidantes. Il existe des recommandations pour la prévention de la maladie carieuse. Afin de réduire la prévalence de la carie de la petite enfance, il semble essentiel que les parents, premiers acteurs de la santé de leurs enfants, connaissent ces mesures préventives. L'objectif principal de ce travail est d'évaluer les connaissances parentales en matière de prévention de la carie dentaire chez les enfants en denture de lait et ainsi de déterminer les messages de prévention à transmettre. L'objectif secondaire est d'identifier, s'ils existent, des sous-groupes de population devant être plus particulièrement ciblés par ces messages de prévention. Nous avons réalisé une étude observationnelle descriptive, basée sur un questionnaire diffusé auprès de cent parents d'enfants âgés de moins de six ans, consultant dans l'un des cabinets de médecine générale alsacien participant. Globalement, notre travail a montré que les connaissances parentales dans ce domaine sont mitigées. Par ailleurs, nos données suggèrent d'informer plus particulièrement les hommes et les parents les plus âgés. Les connaissances parentales seraient facilement améliorables et le médecin généraliste, en tant que professionnel de premier recours, a un rôle essentiel à jouer.
