Book excerpt: La prise en charge des plaies est un enjeu de santé publique majeur qui risque de croître avec le vieillissement de la population dans les prochaines années. Plusieurs professionnels de santé sont impliqués: les hôpitaux, les spécialistes, les médecins, les infirmiers, les kinésithérapeutes et les pharmaciens. Malgré cette pluridisciplinarité, le respect des référentiels n'est pas appliqué ce qui entraine une prise en charge des patients inadaptée, qui entraine un allongement des soins, des pertes de chance de cicatrisation, une augmentation de la morbidité et des hospitalisations, et des dépenses. Ces difficultés de prise en charge sont liées à la différence entre les compétences des acteurs, à la transmission des données entre les soignants qui est parfois insuffisante, mais également au matériel. En tant que responsable des produits de santé, le pharmacien contribue à la sécurisation du circuit des produits de santé, à la pertinence et à l'efficience des prescriptions. Afin de trouver des solutions à ces actes parfois inadaptés, chronophages et coûteux, il semblait nécessaire de rechercher les éléments influençant la qualité de la prise en charge et les difficultés rencontrées. Notre étude a montré que seulement 4% des ordonnances reçues en officine offraient les garanties pour une prise en charges adéquates quel que soit le prescripteur. Il existait des imprécisions sur la fréquence (25%) et sur la durée (39%). Pour un bon protocole de soin, cela souligne le rôle du pharmacien, interlocuteur privilégié par les patients avec la pénurie médicale, ce d'autant plus qu'il peut être le lien entre les prescripteurs et les autres soignant dans la prise en charge des plaies.

Book excerpt: La prescription d'antibiotiques dans la prise en charge des plaies de main de l'enfant est une question pour laquelle il n'existe pas de référentiel ce qui entraine des pratiques hétérogènes et des sur-prescriptions. Notre objectif primaire était d'évaluer la prescription des antibiotiques dans une série rétrospective. Notre objectif secondaire était d'établir un logigramme de prescription et de montrer que si le logigramme était correctement utilisé, il permettrait de diminuer les prescriptions d'antibiotiques sans augmenter la survenue de complications infectieuses. Une étude rétrospective sur dossiers a été réalisée, analysant les plaies de main prises en charges au bloc opératoire pédiatrique de 2013 à 2018, relevant les caractéristiques propres à la plaie, la prescription d'antibiotiques et la survenue de complications infectieuses. Nous avons mis en place une évaluation des pratiques professionnelles sous la forme d'un logigramme de prescription antibiotique qui a été appliqué de façon prospective. Une étude prospective sur un an a été menée relevant les mêmes items que ceux de notre première étude. Nous avons alors comparé la prescription d'antibiotiques mais également la survenue de complications infectieuses et les dépenses dues aux prescriptions d'antibiotiques avant et après diffusion de ce logigramme. En comparant les items choisis avant et après diffusion du logigramme il apparaît que la prescription d'antibiotiques diminuait de 47,6%. Il n'était retrouvé aucune complication infectieuse dans les deux groupes. Sur le plan économique on observe une diminution des dépenses dues aux prescriptions d'antibiotiques ainsi qu'une économie du temps de travail infirmier. Ce logigramme semble donc être utilisable avec un réel intérêt écologique et économique dans notre CHU et exportable à d'autres structures.
