Book excerpt: Approximately 4 million U.S. service members took part in the wars in Afghanistan and Iraq. Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

The major three components of the study included: 1) a comprehensive literature review of veterans and grief; 2) development and implementation of a survey assessing need; and 3) recommendations to the medical center. More than 90% of veterans experienced the death of a close friend or relative and about one-quarter of them expressed interest in a veteran-specific grief program.

The purpose was to better understand how chaplain training prepared or failed to equip spiritual care providers to minister to service members during and/or after the initial experience that they identified as traumatic. The researcher asked nine open-ended questions to encourage and facilitate reflective responses. After grouping responses with similar answers, the researcher coded the chaplains' responses to determine themes that would describe and somehow encompass the phenomenon of serving as a chaplain in a war theater. Throughout the study, the researcher was guided by the theology developed primarily by Paul Tillich. Enriching Tillich's understandings were the writings of Pierre Teilhard de Chardin, SJ. Taken together, their writings and insights offered a spectrum of possibility that not only drew from the spiritual care training provider but also extended that training to the broad experience of service chaplains serving in war theaters"--Abstract.

This report is the third in a series of studies by IOM reviewing the protocols used by the VA and the Department of Defense to diagnose the health complaints of Gulf veterans. The committee reviews and makes recommendations concerning the adequacy of the medical protocol and its implementation by the VA, as well as the VA's outreach and education efforts aimed at informing Persian Gulf veterans and their care providers of the purpose and availability of this program. In addition, the report contains as appendixes the findings and recommendations of the previous reports, as well as those of two related IOM reports on Health Consequences of Service During the Persian Gulf War.

Book excerpt: Department of Veterans Affairs Office of Inspector General conducted a review to assess the Department's capacity to address combat stress in women veterans. It assessed women veterans use of VA health care for traumatic brain injury (TBI), posttraumatic stress disorder (PTSD), and other mental health conditions, and whether the Veterans Benefits Administration (VBA) properly adjudicated women veterans' disability claims for these conditions. It also assessed whether VBA developed and disseminated military sexual trauma (MST) training and reference materials and policies to claims processors and the feasibility of requiring MST training and testing as part of VBA's claims processor certification. The results show that, with variations in degree, female veterans generally were more likely to use VA health care. They were also more likely to continue using VA health care services, even years after separating from active military service, and to use it more frequently. VA generally diagnosed higher proportions of female veterans with mental health conditions after separation, but lower proportions were diagnosed with the specific mental health condition of PTSD and with TBI. These patterns corroborated our findings from data analyses and from review of claims files that higher proportions of female veterans generally were awarded disability for mental health conditions other than PTSD, and a higher proportion of men were generally awarded disability for PTSD and TBI. Data analyses of the study population indicated that VBA denied females more often for PTSD, and denied male veterans more often for a mental health condition other than PTSD, although the denial rates for male and female veterans for all mental health conditions were almost the same. The study did not find any evidence that claims processors applied VBA's current policies and procedures differently when evaluating male and female veterans' disability claims.

Unfortunately, the benefits become a necessity for many veterans who return home because of the mental and physical trauma that they sustain during their time being active duty. The U.S. Department of Veterans Affairs has created a healthcare system specifically for veterans that provides medical centers and outpatient clinics with a variety of services throughout the country. The trauma that veterans experience often requires them to have care that is specialized for them in areas like substance abuse and mental health. While they can receive care at VA locations, it is often not a choice for a veteran to travel the distance to that location or wait weeks or months for an appointment and are forced to get care in the civilian sector. Because of this, the VA has started to allow veterans to receive care at non-VA facilities and as a result, quality of care and specialization for veterans is very important. The services that are provided at those locations are reviewed for both availability and range of services for veterans. This thesis analyzes existing data on substance abuse treatment services and mental health services for both veterans and civilians in order to assess trends for services available and treatment options. Overall, treatment type availability has not changed since the Opioid Safety Initiative. VA facilities are not increasing their availability of alternative therapies, but telemedicine therapy has become more available nationwide. Veteran specific programs are slightly decreasing in non-VA mental health facilities unlike non-VA substance abuse facilities where the program availability is increasing. Increased access to alternative treatments can help medical institutions provide more diverse and better-quality treatment plans for veterans."--Abstract.
