Ses manifestations sont nombreuses, d'une simple hésitation à un mutisme, de la production de paraphasies à l'emploi de circonlocutions. Confronté au manque du mot, à l'oral comme à l'écrit, un patient aphasique met en place des stratégies pour aboutir à une production la plus juste possible. A partir de corpus de dénomination orale et écrite de 5 patients, nous avons décrit précisément les troubles du langage et leurs manifestations de surface, en dressant un inventaire des savoirs lexicaux préservés et des stratégies utilisées. Après avoir étudié l'efficacité de chaque stratégie, nous avons comparé les modalités orale et écrite. Il ressort de cette étude que les mêmes stratégies dénominatives sont mises en place à l'oral et à l'écrit, mais dans des proportions très différentes, et que tous les patients utilisent plus de stratégies à l'oral. D'autre part, seul dans cette modalité les patients emploient préférentiellement les stratégies les plus efficaces. Une analyse qualitative de l'ensemble de la réponse des patients (des productions déviantes ainsi que des stratégies dénominatives), permet de mieux comprendre les mécanismes de production mis en oeuvre. Ce type d'analyse livre à l'orthophoniste des pistes de rééducation directement inspirées des conduites dénominatives du patient, elle oriente la thérapie sur les déficits identifiés, en l'appuyant sur les compétences préservées et en tentant d'optimiser les stratégies dénominatives élaborées par le patient lui-même.

Book excerpt: Introduction et Objectif : plusieurs études ont identifié comme facteurs pronostiques du rétablissement de l'aphasie l'âge ou le sexe du patient, la latéralité et la taille de la lésion, le sous type d'AVC, la sévérité initiale de l' aphasie. Cependant, il reste encore des difficultés à prédire la récupération de l'aphasie chez les patients atteints initialement d' une aphasie sévère. Le but de cette étude était donc de localiser les lésions associées à une récupération dans une cohorte de patients aphasiques. Méthode : 100 patients aphasiques ont été inclus dans les 14 jours suivant un premier AVC hémorragique ou ischémique de l' hémisphère gauche. Ces patients avaient des séquences d' IRM analysables. La sévérité de l' aphasie a été déterminée en utilisant l'échelle d' évaluation de la sévérité de l'aphasie (ASRS) à la phase aiguë puis à 6 mois post AVC. Une aphasie est définie comme sévère par un score ASRS inférieur à 3. Une bonne récupération à 6 mois était définie par un ASRS de 4 ou 5. Les performances langagières ont aussi été analysées. Dans un premier temps, des images pondérées en diffusion axiale ou en T2* (d' hémorragie intra-cérébrale) ont été importées. Les régions des lésions d' intérêt ont été dessinées à la main, en aveugle par rapport aux données cliniques. Les images ont été ensuite normalisées comparativement à un modèle de cerveau standard dans un espace MNI et appliquées sur un modèle adapté. Les cartes de lésions normalisées ont été analysées selon une approche VLSM (voxel-based lesion-symptom mapping), qui superpose les informations comportementales, à l'information voxel sur les lésions binaires. Les cartes qui résultent de ces travaux ont été corrigées pour des comparaisons multiples. This raised the problem of memory and its relation to intelligence, which they examine in this title, originally published in English in 1973. Through the analysis primarily of the child’s capacity for remembering additive and multiplicative logical structures, and his remembrance of causal and spatial structures, the authors investigate whether memories pursue their own course, regardless of the intelligence or whether, in specified conditions, mnemonic improvements may be due to progress in intelligence.
