Dr. Macklin's pioneering work in ethics and global health spans more than two decades. The articles in this volume range from a chapter in a book published in 1989 to a journal article currently in press. The essays fall into two broad categories: policy and practice, and multinational research. Topics in the first category include cultural beliefs and attitudes regarding family planning, long-acting contraception, abortion, and more broadly, policies and practices affecting women's health. Two essays dealing with justice focus on HIV/AIDS: how developing country governments might distribute medications fairly to all who are in need; and what obligations do industrialized countries and world leaders have to provide affordable medications to developing countries. A theme that runs throughout the essays is a defense of the universality of ethical principles, despite cultural differences that exist around the globe. The section on multinational research includes articles on international ethics guidance documents, such as the Declaration of Helsinki; discussion of the obligations of researchers and sponsors when they conduct research in developing countries; what constitutes exploitation when research is conducted in resource-poor countries; and, as in the first section of the book, the application of universal ethical principles to the global research enterprise. The author criticizes the view that double standards in research are acceptable: one standard for rich countries, and a lower standard for developing countries. Several essays deal with sensitive and controversial ethical aspects of research on reproductive health and HIV/AIDS.

Book excerpt: This book documents the massive deprivation of human rights resulting from governmental censorship, manipulation, and control of reproductive health and sexuality information. The introductory chapter applies a human rights perspective to reproductive health to show that women must have full and impartial information to be able to choose services which further their goals rather than governmental policies. Examples of different types of state manipulation are provided, and demographic, biomedical, and reproductive health paradigms of contraceptive delivery programs are described. Chapter 2 identifies the binding obligations imposed on governments by the international principle that women have a right to appropriate reproductive health information. The third chapter provides a global overview of such topics as health expenditures, fertility rates, infertility, literacy and education, infant and child mortality, maternal mortality, child spacing, contraceptive usage, unmet need, abortion, HIV/AIDS, and sexually transmitted diseases (STDs). Chapters 4-13 present country reports for Algeria, Brazil, Chile, Ireland, Kenya, Malawi, Pakistan, the Philippines, Poland, and the US. The country reports reveal the overwhelming need of women to have access to this information and the innumerable ways in which governments control such access. The country reports also describe factors such as religion, culture, tradition, state of development, and influence of foreign donors which have an impact on access to information. Each country report ends with specific recommendations, and the concluding chapter defines seven obligations of national governments imposed by the right to information contained in international law and contains recommendations of ways nongovernmental organizations can use these obligations to lobby governments for improvements.

An example of this is the recent Convention on Bioethics endorsed by the Council of Europe. The aim of the essays contained in this book is to highlight the differences between existing regulations in several countries, and to stress how necessary it is to elaborate a legal framework that could be shared by the widest range of national legislations. For there is no denying that technological advances in the fields of both biology and medicine, as well as progress in surgical treatments, mean that jurists the world over are faced with a common scientific reality. The task of the jurist must therefore be to engage in a comparative analysis so as to overcome the differences in national legislations.

It continues to be professionalized and institutionalized as a maturing discipline at the intersections of philosophy, medicine, law, social sciences, and humanities. Increasingly bioethics - along with its foundational values, concepts and principals - has been exported to other countries, not only in the developed West, but also in developing and/or Eastern countries. Bioethics thus continues to undergo intriguing transformations as it is globalized and adapted to local cultures. These processes have occurred rapidly in the last two decades, with relatively little reflection and examination. This volume brings together contributors from a wide variety of disciplines to take a critical, empirical look at bioethics around the globe, examining how it is being transformed - at both local and global levels - in this process of cross-cultural exporting and importing. One concern is to identify sociocultural forces and consequences which may positively or negatively affect ethics and social justice goals. This book thereby offers the first comparative anthropology and sociology of globalizing bioethics in the field, exploring the global dissemination, local adaptations, cultural meanings and social functions of bioethics theories, practices and institutions and comparing developed and developing countries.
