Dans la majorité des cas, une maturation du col est effectuée par prostaglandines. Cette méthode peut entrainer des contractions plus douloureuses que lors d'un travail spontané. Cette douleur peut avoir des conséquences sur la santé de la mère et du fœtus. L'objectif était de décrire les moyens utilisés par les sages-femmes lors de la prise en charge de la douleur pendant la maturation du col par prostaglandines. Matériel et Méthode : cette étude était descriptive, quantitative et multicentrique. Elle a été réalisée du 9 janvier 2019 au 1er mars 2019 au centre hospitalier universitaire d'Angers et au centre hospitalier du Mans. Les sages-femmes travaillant en salle de naissance dans ces établissements ont été invitées, par mail, à répondre à un auto-questionnaire en ligne. Résultats : pour un score de douleur inférieur à 3, ce sont les moyens non médicamenteux qui sont le plus proposés : la déambulation, le ballon, la douche/ bain ainsi que l'administration de paracétamol et de phloroglucinol. Pour un score supérieur à 6, ce sont l'injection de nalbuphine et la péridurale. Discussion : la prise en charge de la douleur lors de la maturation du col est variée. Cependant, on remarque que les moyens utilisés restent classiques. Les thérapeutiques alternatives sont peu utilisées malgré une demande plus forte de la part des femmes. Cette étude a permis aux sages-femmes d'exprimer leurs difficultés et les facteurs qui peuvent modifier leur prise en charge. Une marge de progression existe encore. Des outils doivent être mis à disposition des sages-femmes pour les aider.

Book excerpt: Introduction : En France, lors d'une indication de déclenchement artificiel, les prostaglandines E2 sont recommandées en première intention, pourtant elles semblent influencer négativement le vécu de la naissance. Objectifs : Evaluer la satisfaction des femmes concernant la prise en charge de leur déclenchement par prostaglandines E2 et décrire les paramètres influençant positivement leur satisfaction afin de l'optimiser. Population et méthode : Etude épidémiologique descriptive transversale auprès de 105 femmes déclenchées par prostaglandines E2 dans une maternité de type III, à l'aide de questionnaires. Résultats : Le score de satisfaction général était de 4,0 [4,0 - 5,0]. L'information et l'accompagnement avaient un impact significativement modéré sur la satisfaction générale, alors que celui de la prise en charge de la douleur était significativement faible. L'adaptation de l'information à la réalité et l'implication des femmes, ainsi que la disponibilité des sages-femmes semblaient être les points clefs de cette satisfaction. Discussion : Pour optimiser cette satisfaction, il parait nécessaire d'évaluer l'information donnée aux femmes selon le personnel médical et de l'adapter si besoin aux données scientifiques mais aussi d'étudier l'efficacité d'alternatives afin d'accroitre le sentiment d'implication des femmes. Connaitre la vision des sages-femmes sur cette prise en charge pourrait aussi permettre de dégager des axes d'amélioration. Conclusion : Les femmes étaient satisfaites de la prise en charge de leur déclenchement, mais des études supplémentaires pourraient permettre d'améliorer le vécu de la naissance de leur enfant dans le contexte d'un déclenchement, où la prise en charge psychologique par la sage-femme semble primer sur la prise en charge physique.

An ab stracted bibliography is a comprehensive abstracted bibliography focusing on the reactions and coping mechanisms of children and adolescents to the death of parents, siblings, friends, teachers, pets or even presidents. Publication~ dealing with both normative and pathological stages of bereavement are reviewed. Materials covering childhood concepts/attitudes toward death are included. Citations appear which explore child death education/counseling issues for parents and educators when they involve actual or antic ipated death. Articles are reviewed which deal with the importance of the adult role in the mourning process of the child. For example, decisions such as whether a child should attend a funeral or return to school are examined.
