Book excerpt: Le diabète de type 1 voit son incidence augmenter chez l'enfant depuis quelques décennies sans que l'on puisse précisément expliquer ce qui sous-tend ce phénomène. Sur un terrain génétique de susceptibilité, se greffe des facteurs environnementaux qui déclenchent, accélèrent, ou retardent le processus auto-immun qui aboutit à la destruction des cellules ß des îlots de Langerhans. Ces déterminants environnementaux peuvent être de différentes natures (nutritionnels, infectieux, toxiques...) et le changement de mode de vie des sociétés industrialisées qui s'est opéré depuis la seconde guerre mondiale pourraient contribuer à expliquer l'augmentation de la fréquence de cette maladie. Ce travail tente de répondre à deux objectifs. Il fait le point sur les connaissances actuelles en termes d'épidémiologie et de physiopathologie de cette maladie au travers d'une revue de littérature, reprenant les résultats d'études animales, d'études de cohorte ou encore d'études épidémiologiques comme EURODIAB ou DIAMOND. D'autre part, cet écrit vise à présenter les données du registre régional PEDIAB-LR, qui recense dans le Languedoc-Roussillon tous les nouveaux cas de diabète chez l'enfant depuis l'an 2000. Actuellement le seul en cours sur ce domaine sur le territoire français, ce registre apporte des informations sur l'évolution de l'incidence du diabète chez l'enfant au niveau régional. Il permet également d'apporter des éléments de réflexion sur les caractéritiques épidémiologiues des enfants diabétiques et de tester ainsi les différents hypothèses émises concernant les facteurs de risque de diabète chez l'enfant
