Changes in the air ambulance (AA) industry's size and structure have led to differences of opinion about the implications for AA use, safety, and services. Some industry stakeholders believe that greater state regulation would be good for consumers. While states can regulate the medical aspects of AA, the Airline Deregulation Act preempts states from economic regulation of AA. Other stakeholders view the industry changes as having been beneficial to consumers and see no need for a regulatory change. Book excerpt: Emergency medical services (EMS) are a safe method of transporting patients from their injury site to the nearest hospital. Many conditions determine the safety of the patients during their travels. Previous research on EMS includes risks and benefits of ambulances and helicopters, but few studies compare these services' safety. This study compares the safety of ambulance and helicopter emergency medical services in the United States between 2000-2019. With this information, EMT personnel can make informed decisions on the best mode of transportation. Generating queries from the National Transportation Safety Board and the National Highway Traffic Safety Administration, data of fatalities and injuries among passengers affected during travel were collected. Based on the data, individuals travelling via ambulances were unaffected. Those affected from ambulance related crashes suffered more from injuries, rather than death. For the helicopter EMS, there were more deaths than injuries among passengers. Quantitatively, more patients were affected in ambulance-related crashes than helicopters. The high number of passengers affected is due to the availability of ambulances for emergencies. A straightforward comparison cannot be drawn at this time. The data must be quantified to a specific injury and accident rate to determine which transportation mode is the safest.

Over the past few years, with improvements in device size and cost, there has been increasing interest in exploring the utility of ultrasound in the prehospital environment. Much of the available literature on ultrasound in the emergency setting focuses on care delivered in emergency departments and intensive care units within the developed world. As a result, most resources are inappropriate and inadequate for doctors and non-physicians practicing in out-of-hospital environments that, by definition, are resource limited. This manual fills that gap by focusing on simplified discussions of ultrasound studies, ultrasound physics, and research that impacts out-of-hospital care in order to meet the needs of prehospital and austere providers. The manual discusses the use of ultrasound for diagnosis in out-of-hospital care, advanced noninvasive monitoring of patients, and safety in performing procedures common to the prehospital and austere environment. As is the approach for prehospital education, the chapters are complaint based and not diagnosis based where applicable. Chapters cover ultrasound image interpretation and basic physics; common image adjustments to improve image quality; unique challenges found in urban prehospital environments, austere/wilderness environments, tactical environments, and military special operations environments; and initial training, quality improvement/assurance programs, and credentialing. It also includes a section on procedures such as pericardiocentesis, vascular access, cricothyroidotomy, and others specific to austere providers.
