This document provides high-level methodical guidance for designing, developing, and strengthening of public health emergency operations centers. This interim document outlines the key concepts and essential requirements for developing and managing a public health EOC (PHEOC). The overall approach is generic and based on widely acknowledged elements of all-hazards emergency management. It provides an outline for developing and managing a PHEOC to achieve a goal-oriented response to public health emergencies and unity of effort among response agencies. The document will be revised as necessary. Practical guidance on specific aspects of the PHEOC framework will be developed and published separately. A public health emergency is here defined as an occurrence, or imminent threat, of an illness or health condition that poses a substantial risk of a significant number of human fatalities, injuries or permanent or long-term disability. Public health emergencies can result from a wide range of hazards and complex emergencies. Experience has shown that timely implementation of an EOC provides an essential platform for the effective management of public health emergencies. Public health emergencies involve increased incidence of illness, injury and/or death and require special measures to address increased morbidity, mortality and interruption of essential health services. For such emergencies, a multi-agency, multi-jurisdictional response is often required, working with the national disaster management organization. When normal resources and capacities are exceeded, support from outside the affected areas will also be required. External assistance could include national, cross-border, regional or international resources.

The protocols were prepared by WHO in collaboration with a large number of international agencies and experts with broad experience in the field of emergency management. Although all protocols follow a common format, each is specific to the circumstances, potential hazards, and immediate information needs that characterize a distinct type of emergency. Emphasis is placed on the exact information needed, the best sources of data and methods for rapid collection, and the specific questions that need to be answered in order to draw initial conclusions and direct immediate actions. Although the advantages of using experienced assessments teams are stressed, the book also explains how the protocols can be used to train general health workers as part of emergency preparedness. The book opens with an introductory protocol covering the aims and methods, responsibilities, complexities, and inherent difficulties of rapid health assessments. Addressed to health authorities as well as assessment teams, the chapter also includes abundant advice on preparedness for emergencies. Details range from the comparative need for speed in different types of emergencies, through a suggested format for presenting the results of assessments, to a list of common logistic, organizational, and technical errors. A protocol dealing with sudden population displacements offers guidelines for conducting rapid health assessments in all emergencies caused by sudden displacement of refugees or population groups within a country. Included are a sample checklist for rapid assessments and a sample form for weekly reports on morbidity and mortality. Subsequent protocols deal with the special situations of nutritional emergencies and chemical emergencies, including those caused by food contaminated with chemicals or toxins. The final protocol addresses the difficult task of conducting assessments in complex emergencies in which the cause of the emergency, as well as the assistance to the afflicted, is complicated by intense levels of political considerations. The protocol includes a form which has recently been used for rapid health assessment at local level in Bosnia and Herzegovnia. The book concludes with a brief summary of survey techniques, followed by a tabular presentation of reference values for assessing needs, hazards, and logistic requirements in developing countries.

The original Blue Book was a comprehensive publication used widely in health-care centers and government agencies to assist in the adoption of national guidance. It also provided support to committed medical directors and managers to make improvements and presented practical information on waste-management techniques for medical staff and waste workers. It has been more than ten years since the first edition of the Blue Book. During the intervening period, the requirements on generators of health-care wastes have evolved and new methods have become available. Consequently, WHO recognized that it was an appropriate time to update the original text. The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. This is no longer the situation. A wider range of people and organizations now have an active interest in the safe management of health-care wastes: regulators, policy-makers, development organizations, voluntary groups, environmental bodies, environmental health practitioners, advisers, researchers and students. They should also find the new Blue Book of benefit to their activities. Chapters 2 and 3 explain the various types of waste produced from health-care facilities, their typical characteristics and the hazards these wastes pose to patients, staff and the general environment. Chapters 4 and 5 introduce the guiding regulatory principles for developing local or national approaches to tackling health-care waste management and transposing these into practical plans for regions and individual health-care facilities. Specific methods and technologies are described for waste minimization, segregation and treatment of health-care wastes in Chapters 6, 7 and 8. These chapters introduce the basic features of each technology and the operational and environmental characteristics required to be achieved, followed by information on the potential advantages and disadvantages of each system. To reflect concerns about the difficulties of handling health-care wastewaters, Chapter 9 is an expanded chapter with new guidance on the various sources of wastewater and wastewater treatment options for places not connected to central sewerage systems. Health-related emergency disaster risk management (Health-EDRM) [1] refers to the systematic analysis and management of health risks surrounding emergencies and disasters; it plays an important role in reducing hazards and vulnerability along with extending preparedness, response, and recovery measures. This concept encompasses risk analyses and interventions, such as accessible early warning systems, timely deployment of relief workers, and the provision of suitable drugs and medical equipment, to decrease the impact of disaster on people before, during, and after disaster events. Disaster risk profiling and interventions can be at the personal/household, community, and system/political levels; they can be targeted at specific health risks including respiratory issues caused by indoor burning, re-emergence of infectious disease due to low vaccination coverage, and gastrointestinal problems resulting from unregulated waste management. Unfortunately, there has been a major gap in the scientific literature regarding Health-EDRM. The aim of this Special Issue of IJERPH was to present papers describing/reporting the latest disaster and health risk analyses, as well as interventions for health-related disaster risk management, in an effort to address this gap and facilitate major global policies and initiatives for disaster risk reduction.
