Book excerpt: The present study examines Emergency Department nurses' perceptions of infant pain management and their perceptions of consulting and engaging with certified child life specialists (CCLS). Participants consisted of 29 registered nurses that currently work in pediatric emergency departments. Participants completed an online survey that assessed participants' perceptions of the child life specialist role, use of nonpharmacological pain management, and their knowledge and attitudes regarding pediatric pain and pain management. Results revealed that pediatric nurses' knowledge and attitudes toward pain management in children varied by level of education and amount of experience working in pediatrics. Findings also suggest that most nurses recognize the significant impact that child life specialists have on the psychosocial wellbeing of patients and families but might not recognize fully how child life interventions can improve the physical well-being of patients. Study implications for practice are discussed.

The children I nursed who experienced pain are the stimulus for both my interest in the issue of pain and latterly in writing this book. Some of the children I cared for stand out as milestones in terms of provoking thought and in questioning practice -James, Ralph and Felicity. These children were tough and brave and taught me a lot about the human aspect of pain and suffering. However, as children, especially vulner able children in hospital, they would not have had to have been as tough or as brave if! had been more knowledgeable about practice issues such as distraction, imagery, massage and so on or had a better understanding of the complexity of pain. Nursing should add to a child's life. By developing knowledge and skills within pain management and prevention nurses are in a strong position to ensure that they, the child and their family are in control of the pain and that the pain is not in control of the child. Developing these skills is ultimately rewarding both personally and professionally and nurses should be encouraged to scrutinize their practice carefully and identify areas where improvements could be made. Listening carefully to what children and their families say about their pain and in identifying the needs of the individual child is an essential part of holistic and comprehensive nursing care.

Book excerpt: ABSTRACT: The purpose of this mixed methods study was to describe the cognitive processes/knowledge sources used by Emergency Department (ED) nurses in decision-making activities regarding triage and pediatric pain assessment and management. Deficiencies persist in ED pediatric pain assessment, and management methods or approaches that might help resolve these deficiencies have not been identified previously. Methodology triangulation with sequential use of qualitative- quantitative methods provided a rich description of knowledge sources and cognitive processes used by ED nurses relative to pediatric pain assessment decisions. Based on qualitative results, a set of vignettes was developed to assess ED nurses. Data analysis using ordinal logistic regression with a cumulative logit model identified patient and nurse variables which influence triage acuity decisions. Five common themes emerged from the qualitative data; 1) Age of the child is important, 2) Behavior can tell a lot, 3) Really looking at the patient, 4) Things that help make decisions, and 5) Things that hinder decisions. Ordinal logistic regression analysis of the quantitative data identified predictor variables of infants compared to school-age children, Hispanic ethnicity, moderate number of years of ED experience (11 -20 years) and years of education that were associated with higher triage levels .The implications of this new knowledge include changes in ED triage nurse practice towards pain assessment, and increased awareness of the need for education in use of pain assessment tools. Additional implications include education related to pain management practices by ED physicians and pain medication protocols at triage. This information may enhance triage and care of the pediatric patient experiencing pain, expand the knowledge base of emergency nursing, identify areas in which to implement changes, assist in improving care provided to children experiencing pain, and provide direction for future education, training, and research.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Not treating pain in pediatrics can cause problems in the future. This project will discuss the nurses views on why pediatric pain management is under managed and treated in the emergency room setting. An educational series will be taught to emergency room nurses and physicians on the importance of how and why pediatric pain should be managed. Once an education session is done and they pass a post test survey, a new implementation project will be rolled out in the emergency room. All pediatric patients between the ages of 7years and 12 years of age, who require a venipuncture or intravenous catheter will receive a topical anesthetic 30 minutes prior to their painful procedure. The nurse who is treating the patient, will monitor their pain level using the FACES Pain Scale before, during and after the procedure in hopes that at least 80% of the patients will have a pain level of less than 6 during the procedure. Once results are collected and distributed to the appropriate leaders, the hope will be that the new procedure for treating pain management will be distributed house wide in the facility to all pediatric patients.
