The broad scope of these volumes shapes a substantial answer to the question, "What is geriatrics and why should we be interested in it?" As I see it, there are at least five reasons. First, the scientific or intellectual reason: gerontology is the study of aging from the biologic, psychological, and social perspectives. There is increasing interest in the fascinating insights into the biologic mechanisms of aging, errors in protein synthesis, DNA repair mechanisms, alterations of the neuroendocrine system, changes in the immune system, genetic controls, and somatic mutations. Second, the demographic reason: this is the century of old age. There has been a 26-year gain in the average life expectancy. This gain compares with that acquired from 3,000 years B.C. (the Bronze Age) to the year 1900, which was about 29 years. Therefore, in one century, there has been a gain in the average life expectancy almost equal to 5,000 previous years of human history. In 1830, one of three newborn infants survived beyond 60 years of age.

The report appears in six chapters. After a brief introduction on health care reform and medical education, geriatric medicine and geriatricians are discussed in chapter 2. Some of the topics examined here include the historical development of geriatrics, physician certification, and the utilization and financing of health services. Chapter 3 analyzes past efforts to develop geriatrics and explores increases in geriatric faculty, geriatric fellowship programs and residencies, continuing medical education, and obstacles to the development of academic geriatrics. Chapter 4 assesses the demand for geriatricians and faculty, while in chapter 5, some strategies to strengthen physicians' geriatrics training are presented. Some of these strategies include revised financial policies, the revamping of service delivery, the strengthening of faculty development and academic programs, and recruiting and marketing ideas. The last chapter comments briefly on the 1993 Institute of Medicine Report. The recommendations made in the above report were grouped into five categories: (1) improved education in geriatrics; (2) leadership centers; (3) enhanced attractiveness of geriatrics; (4) revision in payment policies; and (5) research support. Medicare and Medicaid expenditures related to older adults are nearly a trillion dollars/year in the US. How our healthcare system cares for older adults, and how those older adults navigate an increasingly complex system, is of the utmost importance. According to the Institute of Medicine, physicians and other healthcare professionals receive an inadequate amount of training in geriatric medicine. Geriatric medicine is based on the concept of delivering person centered care with a focus on function and quality of life. It is essential that physicians, nurse practitioners, physician assistants, pharmacists, social workers and other health care professionals all be knowledgeable about the geriatric approach to care. Geriatric medicine varies from most other fields in medicine. While many specialties function on the basis of evidence-based literature, geriatricians and other clinicians caring for older adults must integrate relatively limited evidence with variable physiological changes and complex psychosocial determinants. Geriatricians are used to caring for 90 year olds with multiple chronic illnesses. Their variable physiology leads to uncertain responses to pharmacotherapy, and their personal goals and wishes need to be incorporated into any plan of care. Practicing geriatric medicine requires the ability to see patterns. But it goes one step further, as the rules are constantly in flux. Every patient is an individual with particular needs and goals. In order to provide true person centered care to older adults, one has to incorporate these factors into the decision making process. The proposed handbook is designed to present a comprehensive and state-of the-art update that incorporates existing literature with clinical experience. Basic science and the physiology of aging create a background, but are not the main focus.
