However, it is still difficult to define their implications concerning health maintenance and chronic disease prevention or even treatment. There are many difficulties in their study, with the first being the fact that this group of compounds includes such a diverse array of chemical structures and effects as part of the food products in which they coexist with many other nutrients and non-nutrients. However, today, it is generally accepted that they exert a vasoprotective effect, that they may modulate blood pressure and exert a regulatory effect both on lipid and glucidic metabolism, and that they have an immunomodulatory and gut microbial effect. It is through all these means that polyphenols may be of use in the prevention and treatment of chronic diseases as prevalent as obesity and cardiovascular disease or neurocognitive decline.

Although preclinical and observational studies have promoted the protective properties of polyphenols for a range of chronic diseases, evidence has shown that most dietary polyphenols have little bioavailability. Once ingested, most of them are metabolized by either the intestinal enzymes or by the gut microbiota and then undergo extensive phase-II metabolism reaching significant concentrations of conjugated metabolites. They remain in the systemic circulation and target systemic tissues where trigger biological effects. The polyphenol-derived metabolites produced in humans are dependent upon the composition of the gut microbiota and the subject genetics. Thus all the metabolites do not show the same biological activity in different individuals. To fully understand the health effects of polyphenols, further clinical investigations are required. Numerous clinical and epidemiological studies have led to the result that polyphenol intake may protect against chronic diseases such as cardiovascular and neurodegenerative diseases, cancer, or type 2 diabetes, to name some. Polyphenol intake estimation can be obtained through food frequency questionnaires and nutritional biomarkers, both having their own advantages and disadvantages. Although the association between these bioactive compounds and health seems irrefutable, many questions remain still unanswered. For instance, more studies are needed to identify possible interactions and effect-modulating variables, such as smoking habit, body mass index, sex, alcohol, hormones, other foods, etc. Moreover, intestinal microbiota seems to play an important role in the metabolism of polyphenols, but it is still unclear how.

Herbs, cocoa products, and darkly coloured berries, such as black elderberries, chokeberries, and black currants, are the richest dietary sources that contribute to the average intake of polyphenols of about 1 g/day. Polyphenols that are the most common in the human diet are not necessarily the most active in the body because their beneficial effects depend on the plant matrix in which they are incorporated and on processing methods and endogenous factors such as microbiota and digestive enzymes. Polyphenol-rich foods are considered as being potential functional foods due to antioxidant, anti-inflammatory, antimicrobial, immunomodulatory, anticancer, vasodilating, and prebiotic-like properties. This review will outline findings on the preventive effects of polyphenols on chronic diseases, the factors affecting polyphenol bioavailability and bioaccessibility, and new trends in functional food production.

Dietary phenolics are not only classified as human nutrients, but also play important roles in human health and are therefore called nutraceuticals. Types and contents of phenolics in different food system vary greatly and foods of plant origin including fruits, vegetables, legumes and some beverages are good sources of bioactive phenolics. Beside the edible part of plant foods, some agricultural by-products usually contain higher levels of polyphenols, particularly, flavonoids than products themselves. This book describes the major classes of dietary phenolics, and their occurrence in the commercially important agricultural by-products including peanuts skin, grape pomace, apples pomace, citrus processing by-products, and cranberry pomace.
