Book excerpt: PURPOSE: Although moderate physical activity decreases the risk of arthritis related disability by 47%, pain and lack of confidence are often cited barriers to physical activity participation among older adults with knee osteoarthritis (OA). The purpose of this study was to examine changes in self-efficacy over the course of the Intensive Diet and Exercise (IDEA) trial in obese older adults with knee OA. METHODS: IDEA was a single-blind, randomized controlled trial with 454 obese older adults (M age=65.6 years) with radiographic evidence of knee OA. Participants were randomized to one of three interventions: exercise-only (E-only); intensive dietary weight loss-only (D-only); or intensive dietary weight loss-plus-exercise (D+E). Self-efficacy for walking ability, balance, walking duration and exercise adherence were assessed at baseline, 6 and 18 months. Baseline associations were tested using Pearson correlations, and group least squares means were compared using mixed linear models at 6 and 18 months, adjusted for multiple comparisons. Book excerpt: The work reported in this thesis was driven by the limited existing evidence investigating the effect of a combined intervention programme of dietary restriction and physical activity on overweight/obese older adults with knee osteoarthritis (OA). This thesis comprises three phases: 1) a systematic literature review to synthesise the effectiveness of a combined programme on the musculoskeletal function of this population; 2) the development of a protocol to evaluate the feasibility and acceptability of a combined programme; and 3) a feasibility study conducted using a mixed methods approach to collect data to inform a definitive trial. One pilot and two definitive trials (n=794) and two articles reporting additional data and outcome measures for one of the randomised controlled trials (RCTs) were included in the systematic review. Only mobility (a 6-minute walk test) at 6 months was possible to evaluate by meta-analysis and a pooled random effect of 15.05 (95% CI: -11.77 to 41.87) across two trials (with n=155 participants) did not support a combined intervention programme. Clear differences in relation to a reduced body weight and increased 6-minute walk test result in the intervention compared with the control groups were shown by narrative synthesis. However, the quality of the evidence was not clear. In addition, no UK studies were identified. A single-centre feasibility study, consisting of one arm (a combined programme) with an embedded qualitative component (a focus group for participants and a questionnaire for physiotherapy staff), was conducted at the Royal Orthopaedic Hospital and the University of Birmingham. The programme was found to be acceptable and feasible for the target population and physiotherapy staff involved in terms of integration and expansion. The results of the feasibility study may inform the design of future work to optimise diet and exercise interventions utilising a multimodal approach.

Book excerpt: PURPOSE: The purpose of this cross-sectional investigation was to determine the influences of alignment and obesity on the knee joint loads of overweight and obese older adults with knee osteoarthritis. Our primary hypothesis was that there would be a significant interaction between knee alignment and Body Mass Index (BMI) on the knee joint load, independent of gender and walking speed. METHODS: Baseline data from a subset of participants (157 out of 454) enrolled in the Intensive Diet and Exercise for Arthritis (IDEA) clinical trial were utilized for this study. Knee alignment was quantified on all subjects (N = 157) using a full length anteroposterior (AP) radiograph. Varus alignment was defined as an angle greater than 2°, neutral was 0-2°, and valgus was less than 0°. BMI was quantified in all subjects using each participant's height and weight. Internal knee moments, knee joint forces, and quadriceps, hamstrings, and gastrocnemius forces were calculated using a 3-D gait analysis with a 37-reflective maker set arranged in a Cleveland Clinic full-body configuration, a 6-Camera Motion Analysis system set to sample data at 60 Hz, and a torque-driven musculoskeletal model. Linear regression models were fitted for each kinetic outcome controlling for gender and walking speed.
