Ventilator associated pneumonia (VAP) is one of the most common hospital acquired infections among patients in the ICU, with an estimated incidence of 10 and– 20% in some institutions (CDC, 2012). VAP continues to be substantially associated with patient morbidity and mortality and also increases utilization of healthcare resources, longer hospital stays and higher health care costs (Sandrock and Daly, 2012). Numerous studies demonstrated a significant reduction of incidences of VAP following the introduction of a practice change in oral care (Cutler and Sluman, 2014). This paper will discuss methods of developing the use of ventilator care bundle with oral care, the importance of multidisciplinary approach like involving respiratory therapists to provide oral care to improve the compliance of oral care in mechanically ventilated intensive care unit patients (ICU), implementation of change as well as outcome of ventilator care bundles with oral care for the prevention of VAP in mechanically ventilated ICU patients. VAP has great significance to nursing because nursing staff are able to implement interventions that can prevent infection, thereby relieving the financial burdens on patients and organizations.

Book excerpt: From the moment nursing school is started, nurses are taught the importance of infection control and ways to prevent certain infections from occurring within the hospital setting. For many this knowledge becomes the cornerstone for prevention in the career practice. While others allow short cuts in their practice in order to obtain better time management and prioritize other practices, exposing patients to potential ancillary infectious processes. This form of practice can lead to detrimental patient outcomes. In the ICU, patients receiving mechanical ventilation are at increased risk of developing ancillary disease processes such as VAP. The incidence of VAP was reported to range 9-27% with a mortality rate that may exceed 50% for those receiving mechanical ventilation (Hoshijima and others, 2013, pages 349). One of the main causes of VAP is poor oral hygiene care with the most up to date evidence based practices. While nurses manage their time throughout the day, many will take short cuts and pass on something simple such as oral hygiene in order to have time for other responsibilities. This thought process can prove costly to the hospital in the mechanically ventilated patient population. In order to combat this thought process and provide proper oral hygiene, nurses need to value the cost of this disease process and the financial burden it takes on the hospital. By giving the nurse the information about VAP, the mortality rates and the costs related to it, they can in turn provide better oral hygiene with the latest evidence-based practices when tending to a mechanically ventilated patient.

Book excerpt: Ventilator-Associated pneumonia is known to be one of the most common complications for critically ill patients on mechanical ventilation. Developing a new oral care protocol is necessary to reduce or eliminate the development of this complication. Preventing or eliminating the development of ventilator-associated pneumonia will reduce hospital length of stay, decrease hospital cost, and decrease mortality rates which are caused by this illness. Research has been conducted on the use of chlorhexidine in routine oral care practice, which has shown to be effective in reducing or eliminating the development of ventilator-associated pneumonia. Chlorhexidine has been identified as an easy and inexpensive method in providing oral care with few adverse effects. Developing an appropriate implementation plan, while following the institutions policy and procedure, will provide and show the solution to addressing this complication. One barrier in implementing a change in oral care practice may be the nurses providing the care. Nurses view oral care on critically ill patients as a comfort measure and possibly compliancy can become an issue. Providing the education necessary to nursing staff is essential for the oral care practice to be successful. The aim of this proposal is to show key stakeholders the necessity for change and to implement a new oral care protocol. The proposal will have a written implementation plan explaining how this will be accomplished and an evaluation plan to obtain information in reference to the effectiveness of the proposal.

Book excerpt: Patients in the Intensive Care Unit (ICU) intubated with mechanical ventilators had a higher incidence of pneumonia than other patients, resulting in a higher mortality rate. Ventilator Associated Pneumonia (VAP) rates demonstrated a significant increase after a patient has been intubated for more than two days. Research indicates that bacterial colonization is a major cause of pneumonia resulting in higher VAP rates. A survey of multiple studies has indicated that a VAP care bundle that includes oral care for the patient can reduce the VAP rate to near zero. The VAP care bundle is composed of elevating the patient's head, brushing the patients' teeth with sterile water, and suctioning the fluid from various collection points to remove the potential bacterial colonization. Additional research indicates nursing staff are effective in implementing procedures learned during their initial training, or will readily adopt a new procedure when the training includes the evidence to support the expected results of the procedure. The implantation of this solution includes initial training of the staff, in addition to educational posters posted in critical locations. A committee designated to track the implementation of the program will adjust the implementation plan based on ongoing metrics collected. At the end of the implementation period, the committee will prepare a report detailing the results of the VAP care bundle for distribution in the facility of the implementation. In addition, the committee will oversee the development of a white paper detailing the project and the results for dissemination across the health care industry, particularly to similar ICU facilities.

In a critically ill patient, the development of VAP significantly increases their risk of mortality, length of hospital stay, and decelerates the speed of recovery. Research has shown a substantial correlation between poor oral hygiene in mechanically ventilated (MV) patients and the development of VAP. The implementation of an oral hygiene protocol and/or order-set increases nurse adherence to performing these interventions, improves quality of oral care practices, and ultimately reduces the incidence of VAP. The successful implementation of an oral care protocol requires thorough preparation. Components of implantation will include gathering of evidence, literature review, stakeholder presentation, involvement and education of intended staff, actual implementation, evaluation plan, and dissemination of evidence. The effective implementation of an oral care protocol in intensive care units for MV patients will decrease the incidence of VAP, reduce hospital stays, and decrease costs.

Implementing specialized education for critical care nurses with systematic follow-up actions can increase the compliance rate with all mentioned above positive outcomes. This project is focused on comprehensive literature review, clinical observation of current practices and policies, and proposal of improvement actions.

Book excerpt: Ventilator-Associated pneumonia (VAP) is defined as a Hospital Acquired Infection (HAI) that develops in patients who have been ventilated for more than 48 hours and who were not diagnosed with a pneumonia when intubated. VAP is associated with increased mortality, morbidity, lengths of stay, and increased cost of care. It is the leading cause of death of all hospital acquired infections carrying a mortality rate of 46%. VAP also plays a role in 30% of additional deaths of these critically ill patients due to hospital system failures that could have been avoided. Studies have shown that by implementing evidence based best practices called the VAP bundle on all ventilated patients the incidence of VAP may be decreased up to 45% (safer healthcare now, 2012). While oral care is a component of the VAP bundle, more study is needed regarding best practices for this care component. Current research indicates that a high level of compliance to a scheduled oral care regiment using an antimicrobial solution of 0.12% Chlorhexidine results in the greatest reduction in oral bio-burden (Bopp, Darby, Loftin, and Broscious, 2006). The objective of this study is to test this intervention in reducing bacterial load in the oral cavity and to support the intervention through increased staff compliance to the proposed oral care regimen. Data will be gathered and analyzed to better identify causative factors to VAP, and to monitor compliance to the proposed oral care protocol. The target group for intervention will be all adult patients admitted to the Intensive Care Unit (ICU). The target group for compliance will be all clinical staff in the ICU. The project will take place over a one year period and will include education, training, the oral care protocol, data collection, and reporting results to both clinicians and stakeholders. Expected results are increased staff compliance greater than 90% and a reduction in VAP of 50% at the end of the study.

Book excerpt: Ventilator- associated pneumonia (VAP) exist as a major source of morbidity, mortality, and expense with an estimated economic impact of a sole instance of VAP reaching from $11,897 to $15,0841 in the intensive care unit (ICU), even with therapies directed against it (Garcia, et.al, 2009). It is the second most common hospital acquired infection in the United States according to Sedwick, Lance-Smith, Reeder, and Nardi, (2012), and is responsible for one fourth of the infections in the ICU despite the development of VAP bundles. The Center for Medicare and Medicaid Services (CMS) recently listed VAP as one of the reasonably preventable diseases, along with other hospital acquired infections (HAIs) and soon hospitals may lose reimbursement for the care provided to these patients. An article by Arroliga et. al, (2012) reports on a study involving a multidisciplinary team of practitioners; an intensivist (physician), registered nurse (RN), respiratory therapist (RT), and infection control practitioner, participating on a task force with an objective of reducing VAP. The research involved the respiratory therapist completing the mouth care portion of the ventilator associated pneumonia (VAP) bundle, instead of the nurse, with a dramatic improvement in their mouth care compliance. However, a stronger foundation was seen throughout the literature the multidisciplinary team. Defeating ventilator-associated pneumonia requires more than just bundle compliance; it is a complex disease requiring a multifaceted team. This proposed solution involves the implementation of a multidisciplinary team; including the co-management team (nurse unit manager and the physician champion), clinical educator, a statistician (quality and compliance representative) respiratory therapist, pharmacist, and project co-coordinator. This team will be entrusted with the responsibility to evaluate current practices and coordinate new ones. The findings of this study not only reflects on improving patient outcomes related to oral care compliance on ventilator A MULTIDISCIPLINARY TEAM 3 associated pneumonia in an adult intensive care unit but the role of the multidisciplinary team; where nurses, respiratory therapists, physicians, and administrators come together, in a novel way to attack problems that are unsolvable by only one department. Then again, if the ideas and efforts of several departments, all of which are focused on the same problem, are synthesized, a synergistic effect can be achieved, and selected goals are more likely to be accomplished (Kaye et.al, 2000)). Key to this multidisciplinary team approach is clear access to hospital management and a culture change where everyone becomes a key stakeholder in the process.

VAP is the second most common nosocomial infection in the United States and is the leading cause of mortality and morbidity in Intensive Care Units (ICU). Preventive measures include proper hand hygiene, inclusion of evidence based practices, and education of healthcare professionals. Nurses have a vital role in the implementation of ventilator bundle practices and in reducing the risk of VAP in ICU. A time series study design with convenience sampling was utilized for the implementation of nurse initiated evidence-based practice guidelines to prevent VAP in the ICU. The study participants included all adult intubated patients on mechanical ventilation for more than 48 hours for 12 months. The study was conducted in a 22 bed ICU in a general hospital in California and a total number of 80 nurses were included in this study. Introduction of an intervention took an hour that included poster presentations and in-service education on evidence-based practices for the prevention of VAP. VAP committee, including the nurse manager, educators, ICU nurses, and an advanced nurse practitioner, were auditing the process of the project after the intervention. Evaluations of the effectiveness of VAP prevention best guideline practice is assessed by comparing the occurrence of VAP before and after implementation of the EBP guidelines. The recently implemented EBP guidelines to prevent VAP revealed a reduction in VAP rate, number of ventilator days, mortality and morbidity rates, subsequent hospital stay, and associated costs . Educational sessions provided for nurses showed an improved nursing practice related to VAP prevention strategies such as oral care, head of bed (HOB) elevation, suctioning, and hand hygiene. Nurses should be encouraged to participate in research studies in order to adopt best evidences into clinical practice, thus enhancing patient safety.
