Book excerpt: This was a study to determine whether classical music played in the preoperative holding area decreased the anxiety of surgical patients. A group of patients who are exposed to music in a preoperative holding area will demonstrate a significantly lower degree of anxiety when compared to a group of patients who are not exposed to music in a preoperative holding area. The study was conducted in one area hospital using the preoperative holding area in a general service surgical suite. Two groups of 20 patients each were given two questionnaires using a pre-test-post-test method. The instruments used were the Anxiety Status Inventory and the Self-Rating Anxiety Scale. The hypothesis that patients who are exposed to classical music in a preoperative holding area will experience less anxiety than patients who are not exposed to music in a preoperative holding area was not supported by this study were not statistically significant.

Book excerpt: The purpose of this study was to use music to distract and relax pediatric patients during medical procedures. Procedural support via live music therapy interventions included a variety of activities such as music paired with breathing exercises, counting to music, developing sequence in song, and manipulation of musical and play objects. Because invasive and non-invasive are two radically different types of procedures, two separate experimental designs were implemented and administered to a total of 40 pediatric patients between the ages of 5 and 12, in the main area and the emergency department of a southeastern regional hospital in North Florida. Invasive procedures included intravenous starts, finger pricks, incision and draining, suturing, and removal of foreign material in the skin. Non-invasive procedures included X-rays, computed tomography scans, and breathing treatments. All subjects were randomly assigned to a control or experimental group. Demographics were collected on all pediatric patients; this information included gender, age, procedure, and length of procedure. For self-report, The Pain Intensity and Assessment Tool, modeled after the Wong-Baker FACES Pain Rating Scale, was administered pre and post treatment to measure each child's level of pain, and modified to measure each child's level of anxiety. Results indicated no significant difference in control versus experimental groups receiving either invasive or non-invasive procedures in the main hospital or emergency centers. Mean and standard deviation scores, however, showed a decrease in anxiety and pain levels when comparing pre to post procedures. A behavioral checklist was used post-intervention to record the number of times an anxiety-related behavior occurred. Regardless of procedure, groups receiving music intervention displayed fewer aversive behaviors than groups that did not. In addition, patient satisfaction with MT assistance during the medial procedures was 100%.

One of these therapies include the use of music, as literature has found that it reduces anxiety in patients undergoing surgical procedures that require sedative0́ ̄medications. In patients undergoing cataract surgery, the use of music during the procedure has been found to have a positive impact on patients, as their anxiety levels have been reduced. As a result, this quality improvement project focused on the reduction of anxiety with the use of music therapy, in patients undergoing cataract surgery in an outpatient surgical facility. Using the Hamilton Anxiety (Ham-A) scale, the anxiety level of0́ ̄104 patients was measured pre-and post-operatively. Overall, findings indicated that most of the Ham-A scores of the patients had decreased, and these patients also documented in the "Yes/No 1-Question Survey" that music therapy was therapeutic in reducing their anxiety0́ ̄from the pre-operative period to the time of discharge from the post-anesthesia care unit. However, 10 patients had no change in their anxiety level pre- and post-operatively. The findings of the EBP change project imply that perioperative music can reduce anxiety, and enhance patient outcomes in cataract surgery patients, and as music is a noninvasive, low-cost intervention, it can be easily applied in the perioperative environment.0́ ̄0́ ̄ Key words: Anxiety, Fear, Operating Room, Cataract Surgery, Non-Pharmacological Intervention, Hamilton Anxiety Scale, and Music.0́ ̄ " -- Abstract
