Book excerpt: Injury statistics report that 45% of all athletic injuries are ankle sprains, with 85% of those occurring at the lateral aspect of the ankle. Cryokinetics is a rehabilitation technique that uses cooling with the goal to decrease neural inhibition, thus allowing for greater pain-free range of motion. The present study evaluated how cryokinetics affected sensory perception, range of motion (ROM) and postural stability with previously injured (asymptomatic) versus uninjured ankles. Tactile sensation at the lateral ankle, dorsiflexion at the ankle, as well as postural stability (with eyes open and closed) before and after a cryokinetic protocol were evaluated. The results demonstrate how cryokinetics effects sensory perception, ROM and postural stability in relation to injury history. Two-point discrimination testing demonstrated a significant reduction in tactile acuity after cryokinetics. Functional range of motion testing demonstrated an interaction between a history of ankle injury compared to no injury history, where functional ROM was significantly greater after cryokinetics for previously injured ankles. Overall, vision had the largest impact on postural stability. However, there was an interaction across previous injury history, intervention and vision for median frequency of medial/lateral-Center of Pressure. The current results highlight that ankle injury history is a significant variable when assessing the affects of cryokinetics on functional ROM of ankles and that vision affects postural stability more than cryokinetics. Future research could investigate the effects of cryokinetics on acute care injury management of ankle sprains, and perhaps to ligamentous or soft tissue injuries at joints other than the ankle.

Assessments can be applied to degenerative dysfunction (e.g., falls or knee osteoarthritis in older adults) and sports-related injuries (e.g., ankle sprain or anterior cruciate ligament injury). Patients' body movements and daily activity functions can be compared to the state of pre-injury condition or to the level of healthy individuals. Some cutting-edge studies have gone a step further and used biomechanical performance to develop physical medicine and rehabilitation approaches and explore the mechanisms behind their effectiveness. However, such studies are still relatively rare. This research topic is intended to encourage more relevant projects to be published. This research topic aims to encourage researchers to use biomechanical performance to design advanced physical medicine and rehabilitation approaches, evaluate the effectiveness of the rehabilitation approaches, and explore the mechanisms by which rehabilitation approaches work for neuromusculoskeletal disorders. Some studies have developed stretching approaches for the rehabilitation of knee osteoarthritis in older adults by measuring biomechanical performance during functional activities. Some studies indicated that the mechanism of physical activity to reduce falls in older adults lies in its effectiveness in increasing proprioceptive sensitivity, and further indicated that rehabilitation of proprioception may be a key to reducing falls in the fall-prone older adult population. Some other studies analyzed biomechanical performance in ankle ligament injuries to understand when, how, and why ligaments fail. As a result, this research topic will expand the application of biomechanical performance to better understand and treat neuromusculoskeletal disorders.

Book excerpt: Musculoskeletal injuries sustained during physical training and sports-related activities are the greatest threat to military force readiness. Injuries sustained during physical training are one of the principal reasons for the inability of service men and women to be deployed and for the almost 25 million limited duty days accrued annually. Ankle sprains have been shown to be a leading cause for military disability and are a long-term health care issue, potentially leading to a medical discharge. Military parachuting studies have demonstrated the benefits of using an outside-the-boot ankle brace to reduce the risk for sustaining an ankle injury, but the use of ankle bracing on other performance measures in the military have not been established. The first chapter of this dissertation focused on reviewing existing literature that investigated the effects of ankle bracing on functional performance measures in civilian populations. It was concluded in the literature that ankle braces, used in healthy physically active populations, have little statistical impact on balance and functional performance measures. The second chapter of this dissertation focused on reviewing how ankle braces effect performance measures in military specific populations. At this point, only one study has specifically evaluated how these devices affect performance measures, and the remaining studies focused only on the reduction of ankle injuries. Due to the lack of literature available, the purpose of this study was to investigate the effects of ankle braces on dynamic balance and three functional performance measures in a Reserve Officers Training Corps. This randomized crossover study indicated significant results in which ankle braces do create negative effects for balance reach distances, max jump height, max jump distance, and obstacle course completion time. This is the first study to evaluate ankle braces in a military sample while wearing battle dress uniform, combat boots, and a loaded rucksack.
