Book excerpt: The purpose of this study was to investigate the effects of a fluoride releasing orthodontic primer on demineralization adjacent to brackets. Twenty-two patients were recruited for this study. One premolar was randomly chosen as the experimental tooth, the contralateral tooth was the control. Teeth were visually analyzed for white spot lesions (WSLs). Knoop microhardness was used to determine hardness. Visual examination results showed no significant difference in the number of WSLs observed between Opal Seal and Transbond XT over the duration of this study. Solid conclusions could not be drawn from the results of microhardness testing. Therefore, it cannot be concluded that there is a difference in enamel hardness between Opal Seal and Transbond XT. However, prior to 90 days, teeth showed a significant difference in WSLs. Suggesting a protective effect of Opal Seal that diminished with time. Future studies are necessary to determine the clinical performance of this product.

Book excerpt: Objective: To quantify the effect of fluoride containing varnish on the relative proportions of Streptococcus mutans, Streptococcus sanguis, Streptococcus mitis, Lactobacillus sp. to total bacterial flora in the dental plaque. Subject and methods: 22 orthodontic patients were selected. The study group received an application of Duraphat varnish. Plaque samples were collected at baseline and 30 and 60 days after fluoride varnish application. The total cultivable microflora were counted on blood agar. Mitis-Salivarius agar served to enumerate different oral streptococci and, Rogosa SL agar plates to count lactobacilli. Results: S. mutans counts were statistically lower in the control group than in the experimental groups. Conclusion: Fluoride varnish helps to reduce the bacterial flora in plaque immediately after application but the long-term cariostatic effect of fluoride varnish probably is caused by the remineralization of early carious lesions.

Book excerpt: This study examined the effect of sodium fluoride varnish (V) and phosphate buffered saline (PBS) solution with or without demineralization (DM) prior to bracket bonding on subsequent enamel-bracket shear bond strength (SBS). Sixty maxillary third molars were used for the testing procedures after the roots were embedded in acrylic resin. Specimen were randomly assigned to the following four groups: 1) fluoride varnish-PBS only; 2) fluoride varnish-PBS with demineralization; 3) no fluoride varnish-PBS only; 4) no fluoride varnish-PBS with demineralization. Immediately after varnish or no varnish application, all teeth were placed in PBS for four hours to simulate a time of no food or beverage consumption according to the manufacturer’s protocols. All teeth were then brushed to simulate natural brushing removal. Teeth were then placed in their respective solutions of PBS only or PBS+DM for 7 days, and stored in an incubator at 37°C and subjected to a shaker set at 15 rpm to simulate oral circulation. A universal ten Cate demineralization solution was used. PBS was changed every day and DM three times daily. After 7 days, metal maxillary premolar brackets were bonded using a primer and resin adhesive to the mesial buccal surface of the molars. Following a 24-hour dark cure, SBS was determined with a universal mechanical tester, and adhesive remnant index (ARI) values were then assigned. There was a significantly (p=0.001) greater SBS with varnish application as compared to no varnish with bracketed teeth stored in the PBS+DM. An effect size of 0.372 indicates that approximately 37% of the SBS difference in the PBS+DM group can be attributed to varnish application. There were no significant differences in ARI results based on varnish application or storage medium. Overall results suggest that metal brackets bonded to teeth treated with fluoride varnish and stored in a medium of PBS+DM exhibited a statistically higher SBS, as compared to teeth with no fluoride varnish application.

Book excerpt: Purpose: To evaluate the efficacy of a fluoride-containing orthodontic primer in preventing demineralization adjacent to brackets and compare the quality of enamel on tooth surfaces that received interproximal reduction (IPR). Methods: Patients at the VCU Orthodontic clinic, 10 years and older, who consented to orthodontic treatment involving extraction of at least 2 premolars were recruited to this split-mouth randomized controlled trial. Brackets were bonded to premolars using one of two primers, Opal®Seal"!or Transbond"!T. IPR was performed, and the fluoride-containing primer was applied to the randomly selected experimental tooth. Extracted teeth were analyzed visually and lesion depths were calculated using micro-CT imaging to note the presence and extent of demineralization. The prevalence and depth of lesions were compared using Fisher's Exact test and Wilcoxon Signed-Rank Test, respectively. Results: A total of 18 teeth from 6 subjects were included in the following analyses. Micro-CT analysis revealed lesions on 89% of teeth treated with Opal®Seal"!compared to 67% with Transbond"!T, but this difference was not statistically significant (p-value=0.5765). There was also no statistically significant difference between the depths of the lesions (p-value=1.00), handedness (p-value=0.5765), hygiene (0.7804), or time in the mouth (p-value=0.5601). Visual exam showed no association with treatment (p-value=1.00), handedness (p-value=1.00), hygiene (p-value=0.1373), or the time the teeth were in the mouth (p-value=0.2987). The fluoride-containing primer also seemed to have no effect on the quality of the enamel on teeth treated with IPR. Conclusion: Fluoride-containing primers may not be effective for preventing demineralization on teeth treated with orthodontic appliances.

All rights not granted by the above license are retained by the author. Abstract: The chalky white appearance on enamel is known as White Spot Lesion (WSL). This initial caries lesion involves the dynamic process of demineralization and remineralization. Fluoride varnish has been shown to prevent WSL in post-eruptive tooth. The literature review summarized the available evidence on the various agents added to topical fluoride varnish, such as casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), xylito-coated calcium and phosphate, amorphous calcium phosphate (ACP) and tri-calcium phosphate (TCP) that proposed for enhancing the anticariogenic properties. It is clear that all agents has 5% sodium fluoride as major ingredient; however, there is conflicting evidence regarding the anticaries effect of these agents. The in vitro study evaluated the effect of different fluoride varnishes on remineralization of artificial enamel caries. Enamel blocks (n=60) were randomly divided into six groups (n=10) based on the topical fluoride varnish: (a) Group 0: Control, no application of varnish, (b) Group 1: Duraphat varnish, (c) Group 2: MI Varnish containing casein phosphopeptide-amorphous calcium phosphate, (d) Group 3: Embrace varnish containing xylitol-coated calcium and phosphate, (e) Group 4: Enamel Pro containing amorphous calcium phosphate, (f) Group 5: Clinpro White varnish containing functionalized tricalcium phosphate. The study concluded that Duraphat varnish alone achieve significant remineralization of enamel carious lesions. Clinpro White varnish had similar enamel remineralization effect as Duraphat varnish.
