Economic development can lead to improved population health and a more healthy population can also drive economic growth. Similarly, at the level of the individual, while greater wealth contributes to better health, good health is an important determinant of economic productivity. This study focuses on the Russian Federation. It begins to answer two important questions. What effect has adult ill-health, in particular NCD and injuries, had on the Russian economy and the economic outcomes of the people living there? If the excessive burden of adult ill-health in the Russian Federation were reduced, what economic benefits could result? The overarching message from the findings is unambiguous: poor adult health negatively affects economic well-being at the individual and household levels in the Russian Federation; and, if effective action were taken, improved health would play an important role in sustaining high economic growth rates.

Reasons for this trend include the prevalence of a large informal sector in many developing countries, relatively weak targeting performance of social assistance programs (especially in poor transition countries), and unavailability of broad based insurance mechanisms to protect individuals against loss of income due to unexpected illness.

But progress since then has been slow. While life expectancy in the ECA region today is close to the global average, the gap with its western neighbors has doubled, and other middle-income regions have all surpassed ECA. Some countries in the region are doing better, but full convergence with the world’s most advanced health systems is still a long way off. At the same time, survey evidence suggests that the health sector is the top priority for additional investment among populations across the region. The experience of high-income countries also suggests that popular demand for strong and accessible health systems will only grow over time. Yet these aspirations must be reconciled with current fiscal realities. In brief, health sector issues are a challenge here to stay for policy-makers across the ECA region. This report draws on new evidence to explore the development challenge facing health sectors in ECA, and highlights three key agendas to help policy-makers seeking to achieve more rapid convergence with the world’s best performing health systems. The first is the health agenda, where the task is to strengthen public health and primary care interventions to help launch the “cardiovascular revolution” that has taken place in the West in recent decades. The second is the financing agenda, in which growing demand for medical care must be satisfied without imposing undue burden on households or government budgets. The third agenda relates to broader institutional arrangements. Here there are some key reform ingredients common to most advanced health systems that are still missing in many ECA countries. A common theme in each of these three agendas is the emphasis on improving outcomes, or “Getting Better”.

With aging, the disease burden shifts toward noncommunicable diseases (NCDs) which can threaten healthy aging. However, securing the gains expected from the demographic dividend-where developing countries' working and nondependent population increases and per capita income thus rises- is both achievable and affordable through efficiently tacking NCDs with prevention and control efforts. This book looks primarily at cardiovascular disease (CVD) and tobacco use since they account for a disproportionate amount of the NCD burden-the focus is strategic, rather than comprehensive. The goal of this book is to encourage countries to develop, adopt, and implement effective and timely country and, where appropriate, regional responses that reduce both population-level risk factors and the NCD burden. The work develops (i) an NCD burden and risk factor profile for all countries and the region as a whole; (ii) a rationale for public policy and action for NCDs; (iii) a framework to guide the formulation of public policies and strategies for NCDs; (iv) a country profile, including capacity and ongoing NCD activities, as well as policy options and actions for NCDs that will help stimulate policy dialogue within and among countries; and (v) a regional strategy for NCD prevention and control where regional collaboration offers added value. The achievements of this book are (i) developing a framework for policy options to identify key areas for strategic country- and regional-level policy and actions; (ii) bringing together demographic and aging trends, disease and risk factor burden data, alongside analyses of capacities and accomplishments to tackle NCDs; and (iii) using these inputs to develop policy options for country and regional strategies.
